Entity SEO Agency India — Knowledge Graph Optimization for Healthcare Brands

TL;DR
 • Entity SEO is the discipline of making a brand, a doctor, a hospital or a medical concept recognisable as a distinct entity to search engines and LLMs — not just a string of characters, but a node in the Knowledge Graph with clear relationships to specialty, location, credentials and source citations.
 • In the AI-answer era, entity strength has become the highest-leverage lift for citation eligibility. AI Overviews, ChatGPT, Perplexity and Gemini all lean on entity disambiguation before they select a source; a brand that is not disambiguated is not chosen.
 • ICG's entity SEO work covers: Person schema and Physician credential markup, MedicalBusiness / MedicalClinic / Hospital schema, MedicalCondition and MedicalProcedure entity linking, Wikidata / Wikipedia signalling where eligible, and same-name-string disambiguation for brands sharing a name with another entity.
 • Entity SEO is not a content deliverable. It is a structured-data plus authority-signal plus off-site-mention discipline. Every ICG engagement runs it as a parallel workstream, tracked inside SIE's Rank OS Authority stage.
 • Retainers from Rs 20,000/month · scope set after the Rank OS baseline.

Definition

What entity SEO actually is

Traditional keyword SEO teaches a search engine that a string of characters ("hair transplant clinic Delhi") appears on a page. Entity SEO teaches the search engine, and every LLM downstream of it, that a specific real-world thing exists — a clinic, a doctor, a procedure, a hospital — and that this specific real-world thing has a set of verifiable relationships to specialty, location, credentials, patient outcomes and third-party mentions. The unit of optimization is not the keyword. It is the entity, and the graph of relationships around it.

The shift matters because AI answer systems ground their citations on entities before they rank text. When Google's AI Overview picks a source for "best LASIK surgeon in Bangalore", it is not scoring pages against a keyword phrase alone; it is asking: is there an entity that matches (a) LASIK as a MedicalProcedure, (b) Bangalore as a geographic entity, and (c) a Physician entity credentialled to perform LASIK? A brand whose website surfaces all three — via Person schema on the surgeon, MedicalProcedure schema on the procedure page, and a MedicalClinic entity tied to the city — is a candidate for that citation. A brand whose website has none of that markup, however well its keywords are optimised, is not.

Entity SEO is the workstream that closes that gap. It is a structured-data heavy discipline, but it is not schema markup alone. The full workstream covers three layers: on-site entity declaration (schema.org markup), off-site entity signal (Wikidata entries, Wikipedia mentions where eligible, high-authority third-party references), and same-name disambiguation (making sure a clinic sharing a name with another business, or a doctor sharing a name with another physician, is separated cleanly enough that search engines pick the correct entity every time).

Entity types we work

The entity types that matter for a healthcare brand

Entity typeWhat it representsWhy it matters for AI citation
Person / PhysicianEach named doctor with credentials, specialties, hospital affiliations, alumniOfChatGPT and AI Overviews both weight named-expert authorship heavily. A Physician entity is often the difference between an unattributed brand citation and a named-source citation.
MedicalBusiness / MedicalClinic / HospitalThe clinic or hospital as a real-world business, tied to address, specialty, area servedThe entity that connects the brand to its city and specialty for local-intent AI citations.
MedicalProcedureNamed procedures the brand performs, with prep, recovery, alternatives, contraindicationsThe entity AI Overviews match against when a patient asks a procedure-detail query. Without it, the page competes only on keyword surface, not on entity match.
MedicalConditionNamed conditions the brand treats, with symptoms, causes, differential diagnosesFeeds "what causes X" and "how is Y treated" queries that dominate AI Overview volume in healthcare.
DatasetStructured outcome data — success rates, recovery timelines, pricing ranges — sourced and datedDatasets are cite-magnets. AI Overviews prefer sourced numbers over prose claims; a Dataset entry with a real numeric value and a real citation earns citation more often than a paragraph making the same claim in prose.
Article + Person + about-entityEvery clinical article wired to both an authoring Person and an "about" MedicalCondition / MedicalProcedure entitySignals to the model that the article is authored by a credentialled person and speaks to a specific medical entity, not a generic keyword topic.
Worked example

A worked example: how entity declaration reads to a model

Consider a dermatologist page for a fictional Dr Anaya Rao in Mumbai. Without entity declaration, a search engine sees a doctor page. With entity declaration, it sees a graph: a named person, tied to a specialty, tied to a clinic, tied to a city, tied to a set of procedures she performs.

{
 "@context": "https://schema.org",
 "@type": "Physician",
 "@id": "https://[client]/team/anaya-rao#person",
 "name": "Dr Anaya Rao",
 "medicalSpecialty": "Dermatology",
 "alumniOf": [{ "@type": "CollegeOrUniversity", "name": "AIIMS Delhi" }],
 "worksFor": { "@type": "MedicalClinic", "@id": "https://[client]#clinic" },
 "knowsAbout": ["Melasma", "Acne scars", "Laser resurfacing"]
}

Six lines of markup carry three separate signals: this is a physician (not a generic person), she practises dermatology (not general medicine), and she works for a specific MedicalClinic entity. When an AI Overview considers whether to cite this brand for "best melasma treatment Mumbai", the entity graph gives it a positive match on specialty, location, and topical expertise — three of the five factors that dominate AI citation selection.

The equivalent page without entity markup passes exactly the same prose to the same crawler, and yet earns citation at a fraction of the rate. This gap is what entity SEO exists to close.

Off-site entity signal

Beyond the site: Wikidata, Wikipedia and the off-site entity graph

On-site schema is a declaration. Off-site presence is corroboration. A search engine will accept an on-site Physician entity, but it grows more confident in that entity when the same doctor is referenced by third-party sources with matching credentials — a hospital directory listing, a peer-reviewed journal author page, a Wikidata entry, or a Wikipedia mention where the doctor or the institution is genuinely notable.

The eligibility bar for Wikidata and Wikipedia is real and non-trivial. ICG only pursues Wikidata entries where the entity has verifiable off-site coverage (published research, credible independent news coverage, institutional recognition). Where eligibility exists, a Wikidata entry is one of the strongest off-site entity signals available — it is directly ingested by Google's Knowledge Graph and referenced by multiple downstream LLM systems during their retrieval steps. Where eligibility does not exist, we do not fake it; the strategy shifts to high-authority listings, credentialled directory mentions, and third-party editorial coverage instead.

Deliverables

What an entity SEO engagement delivers

Entity audit

Full inventory of Person, MedicalBusiness, MedicalProcedure and MedicalCondition entities that should exist on the site, versus what actually exists. Ranked by AI-citation impact per entity.

Schema architecture

Entity IDs, cross-references, worksFor / affiliation links, knowsAbout tagging, and a graph-JSON pattern that keeps the entity model consistent across every page rather than duplicating half-declarations.

Named-expert programme

Bylines, credentials markup, author pages, and Person-schema wiring for every clinical article on the site. Tied to ICG's <x-named-byline> component in production.

Off-site strengthening

Wikidata / Wikipedia work where eligible, high-authority directory placements, third-party editorial pitches on legitimately newsworthy stories.

Disambiguation pass

Same-name conflicts resolved. If a brand shares a name with another entity in a different vertical, the schema, About page, and off-site listings are wired to make the disambiguation unambiguous to a model.

Ongoing monitoring

SIE's Rank OS Authority stage recalculates every week. New doctors, new procedures, new locations get onboarded into the entity graph as they appear rather than accumulating as debt.

CPQL benchmarks · ICG vs market

29–51% below the market
median CPQL, specialty by specialty.

Most healthcare marketing agency pitches sell on CPL. ICG sells on CPQL — Cost Per Qualified Lead — the cost of a lead that actually shows up for a consultation. The difference is often 3–4×. Here is what our 150+ healthcare clients actually pay.

Specialty Market median CPQL ICG median CPQL Difference
IVF & Fertility₹2,400₹1,180−51%
Aesthetic Dermatology₹1,400₹950−32%
Plastic Surgery₹3,500₹2,200−37%
Hair Transplant₹2,100₹1,350−36%
Ophthalmology (LASIK)₹1,800₹1,080−40%
Psychiatry₹2,200₹1,260−43%
Dental (general)₹1,100₹780−29%

Medians from ICG's public CPQL benchmark dataset (Q2 2026), drawn from ICG's live portfolio across Delhi NCR, Mumbai, Bangalore, Hyderabad, Pune, Chennai, and tier-2 cities. Adjust for city: metro CPQLs run 20–35% higher than tier-2 across all specialties.

Frequently asked

Entity SEO — in plain answers

Entity SEO is the discipline of engineering how search engines and LLMs recognise a brand, doctor, procedure or condition as a distinct real-world entity, tied to a graph of verifiable relationships — specialty, location, credentials, third-party mentions.

Keyword SEO optimises pages against character strings. Entity SEO optimises against the real-world things those strings refer to. AI answer systems ground on entities before they score text, which is why entity signals now often outweigh classical keyword factors.

Only if the brand or the doctor legitimately meets Wikipedia notability guidelines. Where eligibility exists, a Wikipedia entry is one of the strongest off-site entity signals. Where it does not, ICG does not pursue it — the strategy shifts to Wikidata, high-authority directories and legitimate editorial coverage.

Google's internal database of entities and their relationships. It is what allows search to answer "who founded X" or "which hospital does Y practise at" without ranking a page. Entity SEO influences whether a brand ends up as a node in that graph.

Schema-side entity declarations start showing signal within 4–6 weeks of deployment. Off-site strengthening (Wikidata, editorial coverage) takes longer — typically 3–6 months to move the needle on Knowledge Graph presence.

Person / Physician, MedicalBusiness / MedicalClinic / Hospital, MedicalProcedure, MedicalCondition, and Dataset. Layered together with cross-references, not deployed in isolation.

Retainers from Rs 20,000/month, custom-scoped after the Rank OS baseline diagnostic.
