- Portfolio mean zero-click rate is 58.6 percent. Roughly six of every ten Indian healthcare searches now end without a click on any organic result. The 2024 baseline on the same corpus was 41 percent.
- Symptom queries lead at 71.4 percent zero-click. Cost queries 64.2 percent. Doctor-near-me queries 62.7 percent. Brand queries the lowest at 32.1 percent.
- AI Overview is the largest single cause. AIO appears on 27.6 percent of the corpus and lifts zero-click on those queries by +24 percentage points on average.
- People-Also-Ask is now the highest-coverage feature. 82.4 percent of the 3,102 queries in this zero-click corpus carry a PAA panel (portfolio-weighted) — higher than featured snippets (34.2 percent) and local pack (47.1 percent). The separate PAA Capture Report measured 88.8 percent on its own 2,384-query corpus; the two figures come from different query sets.
- Commercial intent still clicks. Brand, navigational, insurance and compare-to queries carry zero-click rates below 52 percent — this is where organic CTR still matters.
- Success metrics must migrate. Clicks stay valid on brand and commercial queries. AIO citation rate and PAA capture rate replace clicks as the primary KPIs on informational and commercial-informational queries.

Five numbers to anchor the rest of the report
Methodology
This study aggregates Google Search Console impressions and clicks for 3,102 Indian healthcare and healthcare-adjacent queries drawn from ICG's Search Intelligence Engine, cross-referenced with weekly SERP-feature scrapes for the same query set. The reporting window is 1 April to 31 August 2026. Every query in the sample carried at least 100 impressions in the 90 days preceding the window, is India-geo dominant (at least 85 percent of impressions from IN), and is non-branded except in the Brand / navigational cluster which is deliberately preserved for comparison.
Zero-click definition. Zero-click rate is calculated as (impressions - clicks) / impressions on the query subset. This is a conservative definition — it treats an impression served on a query with a PAA panel where the user expanded a PAA but did not click any organic result as a zero-click event. It does not treat AI Overview citation appearances as clicks (they are not clicks; they are impressions with no click credited to any URL). SERP-feature presence rates are calculated from weekly scrape data in India geo, incognito, no personalisation, at three time-of-day slots.
Specialty clustering. Queries were tagged into ten specialty clusters based on primary intent, not clinical specialty. Symptom queries include "why does my hair fall out" and similar. Cost queries include "how much does IVF cost in Delhi". Treatment explainer queries include "what happens during a root canal". This intent-based clustering is more useful for content-strategy decisions than a clinical-specialty clustering, because the SERP behaviour follows the intent shape, not the specialty label.
Anonymisation. No client names, brand names, per-URL identifiers or query strings appear anywhere in this report or its published dataset. Every figure is an aggregate across a specialty cluster or across the whole sample.
What this report does not claim. This is a portfolio sample, not a random sample of the Indian healthcare search market. Absolute zero-click rates in the wider market may be higher or lower depending on the property mix. The relative pattern across specialty clusters and SERP features is, however, robust — every specialty and every feature is measured on the same corpus with the same methodology.
Finding 1 · Portfolio-weighted zero-click rate is now 58.6 percent
The number: 58.6 percent of impressions across the 3,102-query sample generated no click on any organic result. That figure is up from a 2024 baseline of 41 percent on the same corpus and up from 47 percent in the 2025 mid-year snapshot. The two-year climb is driven almost entirely by AI Overview roll-out and expanded People-Also-Ask coverage.
The shift is uneven across intent clusters. Informational queries — symptoms, treatment explainers, recovery and aftercare, how-to — all sit above 52 percent. Commercial queries with clear transactional intent — brand, compare-to, insurance, doctor-near-me — sit below 63 percent, with brand and compare queries dropping into the 30-48 percent band. The middle band of commercial-informational queries (cost, best-of listicles, best-clinic-for) sits at 53-64 percent zero-click. This tri-band split is the practical map for reallocating content-team effort.

The specialty spread runs from 71.4 percent (symptom queries) to 32.1 percent (brand queries). Symptom queries are the SERP-feature layer's ideal target — informational intent, well-defined answer, low commercial risk for Google to satisfy in the SERP. Brand queries are the opposite — the user is looking for a specific destination and the SERP mostly gets out of the way. Everything in the middle follows a predictable rule: the more commercial and specific the intent, the lower the zero-click rate. Content teams should read this table as an effort-allocation map, not as a lament about lost clicks.
Finding 3 · AI Overview is the largest single driver of zero-click lift
The number: AIO appears on 27.6 percent of the corpus and lifts zero-click rate on those queries by roughly +24 percentage points versus the same queries in a matched no-AIO week.
SERP feature presence and zero-click contribution
AI Overview has the largest per-query effect. People-Also-Ask has the largest cumulative effect because it appears on more than three times as many queries. The two features together account for roughly 80 percent of the observed zero-click lift since 2024. Local pack, featured snippets and knowledge panels each contribute a smaller but non-trivial share. Video carousels have the smallest zero-click lift per appearance — a video click usually opens a new destination outside the SERP, so it registers as a click rather than a zero-click event.
Finding 4 · Commercial intent still clicks — this is where CTR still matters
The subset: brand queries (32.1 percent zero-click), compare-to queries (47.6 percent), insurance and cashless queries (51.5 percent), and how-to queries (52.9 percent) are the four clusters where organic click-through rate remains a primary success metric.
The efficient strategy allocation follows from this split. Sixty percent of ranking effort should sit on informational queries — engineered for AIO and PAA capture, measured by citation and capture rate. Forty percent should sit on commercial-and-brand queries — engineered for organic click-through, measured by top-3 rank density, CTR and conversion. Teams that do not differentiate the two effort tracks tend to over-invest in click-optimisation on informational queries where clicks are not available, and under-invest in click-optimisation on commercial queries where clicks are the whole point.
Finding 5 · Zero-click does not mean zero value — measure citation and capture instead
A URL cited across three PAA slots and one AI Overview on a symptom query might drive zero direct clicks that day. But that citation footprint feeds three second-order signals: topic-authority accumulation (which materially lifts ranking on adjacent commercial queries in the same cluster), brand-recall surface area (users see the same source cited across multiple SERP surfaces), and third-party AI-assistant referenceability (the same content becomes eligible for citation in Copilot, Perplexity and ChatGPT web).
The correct measurement upgrade is a three-metric dashboard. AIO citation rate on the target set. PAA capture rate on the target set. Commercial share of voice on the transactional subset. Clicks remain a valid tail metric on the commercial subset, but the primary KPIs on the informational subset are now citation-shaped, not click-shaped.
Finding 6 · The 2024-to-2026 trend is a 17-point climb — the next 12 months will continue
The trajectory: zero-click rate on this corpus was 41 percent in the 2024 baseline snapshot, 47 percent in the 2025 mid-year snapshot, and 58.6 percent in the Aug-2026 snapshot. Linear extrapolation is unwise, but the direction is unambiguous — zero-click rates will keep climbing on informational healthcare queries as AI Overview coverage expands (currently 27.6 percent, likely 40-50 percent by mid-2027) and as third-party AI-assistant traffic siphons additional intent away from Google entirely.
The strategic response is to invest in citation and authority now, on the informational side, and to invest in click-through optimisation on the commercial side. Waiting until 70 percent portfolio-wide zero-click to migrate measurement infrastructure is waiting too long.
What this means for healthcare marketers
Four takeaways worth acting on this quarter.
1 · Segment your queries by intent cluster and report zero-click per cluster
The portfolio-wide 58.6 percent number is unhelpful for decision-making. The per-cluster split (32-71 percent) is what tells you where to put content-team effort. Every healthcare property should report zero-click at the intent-cluster level, monthly.
2 · Move informational content KPIs from clicks to citation and capture
AIO citation rate and PAA capture rate are the correct KPIs for informational cluster pages in 2026. Clicks are a tail metric. Teams still reporting only clicks on symptom, treatment-explainer and recovery-aftercare pages are measuring the wrong thing.
3 · Concentrate click-optimisation effort on brand, compare-to, insurance and commercial-informational queries
These four clusters still generate real click volume with reasonable CTR. This is where meta-title testing, description testing, structured-data experiments and rich-result targeting continue to pay back.
4 · Set a 12-month citation-rate target, not just a click target
A defensible 12-month target is 15-20 percent AIO citation rate on target informational queries and 18-25 percent PAA capture rate on the same set. Both are measurable, both are actionable, both align with where the SERP is going.
