Two of the thinnest specialties in the country, three in the top six
Philadelphia's overall rank of 14th of 20 is an average of extremes. Between its deepest specialty and its thinnest, the gap relative to national norms is about 3.1-fold, the kind of spread that makes a single "how competitive is Philadelphia" answer misleading. The chart shows each specialty's Philadelphia median review count against the national median.

The aesthetic end is crowded. Philadelphia med spas have a median of 195 reviews (5th of 19 metros), and plastic surgery practices 115.5 (6th of 20). The wellness and manual-therapy end is not: IV therapy and chiropractic both rank last of 20, and medical weight loss sits 17th.
Philadelphia chiropractic: thin on reviews and on the basics
Chiropractic is the clearest opening in our Philadelphia data. The median practice has 54 Google reviews against 125 nationally, and 47.5% have fewer than 50, the highest share of any metro. The gaps go beyond reviews:
- Only 84.7% of Philadelphia chiropractors list a website on their Google profile, the lowest share of 20 metros (national 95%).
- 18.6% list a weekend day, the 2nd-lowest share (national 30.8%).
- The average rating is 4.67 stars, against 4.77 nationally.
Dentistry, a large share of Philadelphia searches in our data, sits in the middle. The median dental practice has 293.5 reviews, 13th of 20 metros, and 31% have passed 500, about the national share. The weak spot is again the basics: 94% of Philadelphia dental practices list a website on Google, the lowest share of 20 metros, and 17.2% list a weekend day (national 25.5%).
IV therapy looks similar on reviews: a median of 23, with 66.3% of clinics under 50. As plain arithmetic, a chiropractor adding 8 reviews a month would pass the Philadelphia median in about 7 months; the same pace would take about 16 months against the national figure. That ignores competitors' own new reviews, but the order of magnitude is the point.
Mid-priced clicks, and a cheap click that is not a cheap patient
Our 2026 planning estimates put Philadelphia general-dentistry clicks at about $10.20 (10th of 20 metros, 1 = most expensive), orthodontics at $18.60 and cosmetic dentistry at $33.20. Medical dermatology runs about $7.80 and cosmetic dermatology $21.10, 10th of 19.
Fertility is the instructive case. A Philadelphia IVF click is one of the cheapest in our fertility benchmark, about $11.20 (2nd cheapest of 16), yet a new patient consultation is estimated at about $620, 8th most expensive of 16 and about 2 times the lowest metro estimate ($310). Pennsylvania is not on RESOLVE's list of states with an infertility insurance law, which may mean more people click and compare clinics before they book. Judging an IVF campaign by its click price would miss that.
What this means for your practice
- Ignore the city average. Look up your own specialty's Philadelphia row; it may be the deepest or the thinnest in the country.
- Chiropractors and IV clinics: move first. Fix the profile basics (website, hours, weekend days) and start a review routine while the field is thin.
- Med spas and plastic surgeons: plan for a long review program, and use paid search to stay visible meanwhile.
- Fertility clinics: measure cost per consultation, not per click. In a state without a coverage law, follow-up and price clarity decide bookings.
- Re-check yearly. Thin markets fill up; the first practices to build reviews keep the lead.
