What 3,000 med spa reviews reveal: patients love the care, and struggle at the front door
We classified about 3,000 Google reviews from a random subsample of 115 US med spas in our 555-spa study into one primary theme each, with sentiment. Patients overwhelmingly praise the people and the results. Their frustration collects in one place: getting booked and getting answered. This is part of our State of Med Spa Google Presence 2026 study.
- About 98% of reviews about staff, results and comfort are positive. The clinical experience is rarely the problem.
- Booking and communication is the only theme where negative reviews outnumber positive ones: about 54% negative (a small slice of all reviews).
- Price, before/after outcomes, scheduling and upselling each carry a 16–20% negative minority.
- The winnable gap is operational: answering the phone, replying to messages and honouring bookings.

Staff and bedside manner (about 48% of all reviews) and results (about 34%), both more than 97% positive. Pain and comfort and cleanliness were also overwhelmingly positive.
How were the reviews analysed?
Up to 50 recent Google reviews per med spa were collected through SearchApi, and 2,990 reviews were each classified by Google Gemini (gemini-2.5-flash) into one primary theme with a sentiment. Counts are directional; an independent re-run reproduced the key finding, and a human spot-check of 100 reviews is in progress. The review subsample covers 115 med spas; themes are directional.
How can a med spa fix booking and communication problems?
Answer calls and messages quickly (or call back the same day), confirm which services each location offers before booking, check product availability ahead of appointments, make package scheduling easy and follow up personally on any complaint.
