Direct answer
International SEO technical audits surface the same 20 issues across most Indian healthcare sites: missing hreflang, wrong canonicals, broken international sitemap, geo-targeting misconfiguration. Here's the complete checklist.
TL;DR
ICG's international SEO audits across Indian healthcare brands consistently surface the same 20 technical issues. Most sites have 8-14 of them simultaneously. None of them are visible from the homepage; they require structured technical audit to find. And until they're fixed, content quality and link building deliver disappointing results because the technical foundation is broken.
This article is the complete 20-point checklist ICG runs on every international SEO engagement.
Foundation layer (Points 1-5)
1. hreflang implementation
Are hreflang tags present in the <head> of every international-facing page? Are they correctly formatted (<link rel="alternate" hreflang="en-AE" href="..." />)? Do they include self-referential links? Do they include x-default?
Common failure: hreflang missing entirely, or implemented only on home page, or with wrong region codes (en-UAE instead of en-AE).
2. hreflang bidirectionality
Does every page version link to every other version? UAE page must link to UK page; UK page must link to UAE page; both must link to Indian (default) version. Unidirectional hreflang is broken hreflang.
Common failure: Forward-only references (Indian page lists international versions, but international pages don't list each other).
3. Canonical URL correctness
Does every international page have a self-referential canonical (<link rel="canonical" href="this-page" />)? Or does the canonical point to the India version (which would tell Google to demote the international page)?
Common failure: All international pages canonical to India version → international pages effectively de-indexed.
4. International sitemap
Is there a sitemap that includes international URLs with <xhtml:link> hreflang annotations? Submitted to GSC for each target country property?
Common failure: Single sitemap with no international annotations; no separate GSC properties for international targeting.
5. GSC international targeting
For each target country, is there a separate GSC property (preferred via Domain property) with hreflang international annotations submitted? Country-specific performance reporting set up?
Common failure: Single GSC property reporting blended India + international metrics with no separation.
Content + page layer (Points 6-10)
6. Language consistency
Does the content language match the hreflang declaration? An en-AE page must be in English. An ar-AE page must be in Arabic. Language detection misalignment is a common bug.
Common failure: Page declared ar-AE (Arabic UAE) but content is in English with no Arabic translation.
7. Country-relevant content
Does the international-targeted page have content that's genuinely relevant to the target country? Or is it the India page with a different URL but identical content?
Common failure: Duplicate content across international versions; Google penalises this as low-quality.
8. Local currency display
Does the cost transparency content show prices in local currency for the target market? UAE page shows AED; UK page shows GBP; USA page shows USD.
Common failure: All pages show INR; international patients have to mental-arithmetic to convert.
9. Local credentials emphasis
Are credentials relevant to the target market prominently displayed? UK page emphasises UK-fellowship-trained doctors; US page emphasises US board certifications; UAE page emphasises JCI accreditation.
Common failure: Generic credential listing; no market-relevant prioritisation.
10. Country-specific FAQ schema
Does each international page have FAQPage schema with country-specific Q/A? Generic Q/A is weaker than market-specific ("how do UAE patients arrange visa for India IVF treatment").
Common failure: Generic Indian FAQs duplicated to international pages.
Technical infrastructure layer (Points 11-15)
11. Server location declaration
Where is the website hosted? Does the server location signal align with international targeting? Indian hosting for India targeting is fine; Indian hosting with international targeting requires explicit hreflang to override server-location signals.
12. Page speed for international visitors
International visitors typically have higher latency to India-hosted servers. CDN configuration matters. Page Speed Insights → Mobile → International market simulation.
Common failure: Page speed acceptable from India but >5 seconds from UAE / UK / US due to no CDN.
13. Mobile rendering
International patients overwhelmingly research on mobile. Is mobile rendering perfect? Is mobile content equivalent to desktop?
Common failure: Mobile content stripped down; international visitors get inferior experience.
14. Schema markup richness
Beyond FAQPage, is there appropriate schema for Service (the treatment), Organization (the hospital), Person (the doctor), MedicalProcedure (specific procedures)?
Common failure: FAQPage only; missing other schema types.
15. Internal linking architecture
Do international pages link to each other (UAE IVF page links to UAE oncology page)? Do they link to the relevant Indian content for context? Internal linking signal is critical for cluster ranking.
Common failure: International pages are silo'd from each other.
Off-page + monitoring layer (Points 16-20)
16. International backlink portfolio
Are there backlinks from target-market domains? UK .uk domains? UAE .ae domains? USA .com domains in healthcare context? International link signal is the biggest gap for most Indian healthcare brands.
Common failure: All backlinks from .in / generic .com; international backlink portfolio empty.
17. AI citation tracking
Are LLM citations being tracked for international queries? Is AIO Intel monitoring Perplexity + ChatGPT + Gemini for target-market queries?
Common failure: No LLM citation visibility; can't measure AEO performance internationally.
18. International rank tracking
Are rankings tracked per target market? Same keyword may rank position 3 in India but position 47 in UAE — same page, different markets. Rank tracker needs market-by-market configuration.
Common failure: Only Indian rankings tracked; international performance invisible.
19. International GSC performance review
Is monthly GSC performance reviewed per target country? Impressions, clicks, position trends by country?
Common failure: Only India performance reviewed; international clicks not noticed.
20. International conversion attribution
Is there end-to-end attribution from international SEO clicks through to international patient bookings? Beacon configured for cross-border attribution?
Common failure: International traffic counted but conversion attribution broken; no understanding of channel ROI.
ICG's audit process
ICG runs this 20-point audit as the opening week of every international SEO engagement. Most Indian healthcare brands fail 8-14 points. Fixing the foundation layer (points 1-5) before content scale-up is non-negotiable.
