Registration 1: State Medical Council / National Medical Register

What it is: Your personal registration as a qualified practitioner. Everything else references this.

Who needs it: Every doctor practising in India.

Documents:

  • MBBS/MD/MS degree certificate (original + certified copy)
  • Internship completion certificate
  • Character certificate from your medical college
  • Passport photographs (4-6)
  • Application fee: ₹500-2,000 depending on state

Timeline: 4-8 weeks. States with online portals process faster.

2026 update: The NMC's National Medical Register (NMR) is being progressively populated. Verify your NMR entry is current — insurance empanelment and hospital credentialing are increasingly cross-referencing the NMR.


Registration 2: Clinical Establishments Act Registration

What it is: Registration of the clinic as a clinical establishment under the Clinical Establishments (Registration and Regulation) Act 2010, or the equivalent state Act.

Who needs it: All clinical establishments including solo OPD clinics. The Central Act applies in 11 states and all Union Territories. Other states have parallel legislation.

Documents:

  • Proof of premises (ownership deed or rental agreement)
  • Doctor's medical council registration certificate
  • Clinic layout plan (architect-signed in some states)
  • Equipment list and services offered
  • Application fee: ₹2,000-15,000 depending on category and state

Timeline: 4-12 weeks. Delhi: 6-8 weeks. Karnataka: 4-6 weeks. Maharashtra: 8-12 weeks.

Categories: Establishments are classified (typically A, B, C) by bed strength. A solo OPD clinic without beds is the lowest category and has lighter documentation requirements.


Registration 3: PCPNDT Registration (If You Use Ultrasound)

What it is: Registration under the Pre-Conception and Pre-Natal Diagnostic Techniques Act 1994. Mandatory for any clinic or facility using ultrasound equipment — including portable units.

Who needs it: Any facility with an ultrasound machine: gynaecology clinics, IVF centres, radiology centres, hospitals, and any clinic using portable ultrasound for clinical assessment.

Documents:

  • Form A (application)
  • Doctor's medical council registration
  • Proof of qualification in sonography (for the operating doctor)
  • Proof of premises
  • Equipment details: make, model, serial number, purchase invoice
  • Form C proforma (for maintaining records of all scans)

Post-registration obligations: Every patient who undergoes ultrasound must fill Form F. The clinic must maintain a register of all examinations. Annual renewal required in most states.

Timeline: 4-8 weeks. The PCPNDT authority is at the district level — processing varies significantly by district.

Penalty for non-compliance: Criminal penalties — imprisonment up to 3 years and fines. One of India's most actively enforced healthcare regulations.


Registration 4: Trade Licence

What it is: Local municipal corporation licence to operate a business at the premises.

Who needs it: Every clinic.

Documents:

  • Proof of premises (rental agreement or ownership deed)
  • Identity proof of proprietor/partners/directors
  • NOC from landlord (if rented)
  • Application fee: ₹1,000-5,000 depending on municipality and floor area

Timeline: 2-4 weeks in most municipalities with online portals.


Registration 5: Shops and Establishments Registration

What it is: State government registration under the Shops and Establishments Act. Required for all establishments that employ at least one person.

Who needs it: Any clinic with a receptionist, nurse, or lab technician.

Documents:

  • Proof of business address
  • Identity proof of employer
  • Employee details (numbers, working hours, wages)
  • Application fee: ₹500-2,000

Timeline: 1-3 weeks. Most states have online portals with near-immediate certificate generation.


Registration 6: Pollution Control Board Consent

What it is: Consent to Establish (CTE) and Consent to Operate (CTO) from the State Pollution Control Board. Required for any clinic generating biomedical waste.

Who needs it: Any clinic using syringes, blood-soaked materials, surgical instruments, or lab specimens — which effectively means most clinical practices beyond pure-OPD consultation.

Documents:

  • Proof of premises
  • Clinical Establishments Act registration
  • Details of biomedical waste generation and disposal method
  • Agreement with a CBMWTF (Common Biomedical Waste Treatment Facility) for waste collection

Timeline: CTE: 4-8 weeks. CTO: issued after inspection, an additional 4-8 weeks in some states.

CBMWTF note: Under the Biomedical Waste Management Rules 2016 (amended 2019), all healthcare facilities must segregate waste into colour-coded containers and use an authorised CBMWTF for treatment and disposal. The CBMWTF agreement is a prerequisite for PCB registration — sign this early.


Registration 7: GST Registration

What it is: Registration under the Goods and Services Tax.

Who needs it: Clinics with annual turnover above the threshold (₹20 lakh for services in most states; ₹10 lakh in special category states). Even below the threshold, registration is often useful for Input Tax Credit on capital equipment.

Healthcare GST note: Medical services provided by a registered clinical establishment are GST-exempt (Schedule II, Entry 74 of the CGST Act). However, cosmetic procedures, diagnostic services, and pharmacy sales may attract GST depending on classification. Verify with a healthcare-specialist CA before assuming full exemption.

Documents:

  • PAN (clinic or proprietor)
  • Aadhaar (proprietor or directors)
  • Proof of business address
  • Bank account details
  • Digital signature (for company registrations)

Timeline: 7-10 working days via the GST portal.

  2. Execute the CBMWTF agreement first. It is a prerequisite for PCB and the agreement process takes time to arrange.
  3. Verify PCPNDT requirements with your district authority before purchasing any ultrasound equipment.
  4. Engage a healthcare-specialist CA for GST and entity structure. The healthcare exemption has specific conditions that a generalist CA may miss.
  5. Appoint an operations manager or senior clinic staff member to track each registration status weekly and follow up proactively.

2026 update: what changed for clinic registration this year

Three shifts in 2026 quietly reshaped how a new medical clinic gets registered in India, and most founders are still working off 2024 checklists that miss them.

  1. Clinical Establishments Act (CEA) expansion. Two more states notified the CEA in early 2026, taking the operational count past 15 states plus all UTs. If you registered your entity in 2023-24 and skipped a CEA filing because your state was outside the net, re-check before your next renewal - the local health authority now expects a provisional certificate on file.
  2. PCPNDT Form F digitisation. The Form F ultrasound register moved fully online in the largest states this year. Manual registers are still accepted during inspection, but the appropriate authority increasingly cross-checks digital submissions against footfall - a mismatch flags the clinic for audit.
  3. GST threshold clarity for consultation-only clinics. Pure OPD consultation remains exempt, but any diagnostic add-on (path lab tie-up, imaging, IV infusion room) pulls the clinic into GST at the aggregate turnover trigger. Model your 24-month revenue mix before deciding to voluntarily register.

None of these are dealbreakers. They are timing traps - the kind that add three weeks to a launch calendar because a founder discovered them on day 40 instead of day 1.

What most founders get wrong in the first 30 days

  • Filing the trade licence before the CEA provisional. Municipal bodies in Delhi, Bengaluru and Hyderabad now ask for the CEA acknowledgement as a prerequisite. Reverse the order and you queue twice.
  • Booking the clinic address on a residential lease. Shops & Establishments and the Pollution Control Board both need a commercial or mixed-use address; converting mid-registration costs 4-6 weeks.
  • Ignoring biomedical waste sign-up. A CBWTF (Common Biomedical Waste Treatment Facility) tie-up is a document dependency for CEA in most states - not a post-launch task.

Operating without medical council registration is professional misconduct under NMC provisions. PCB non-compliance attracts significant fines and can result in forced closure. The risks are serious — prioritise getting registered before opening.
