In local search, the practices that win the map results tend to be the ones with the deepest review histories, because star ratings no longer separate anyone. So the most useful measure of how competitive healthcare SEO is in a city is how many reviews you are up against. Our State of US Healthcare Marketing 2026 report turns that into a review-depth index, comparing each specialty's city median with its national median.
New York scores 1.52, the highest of the 20 metros, and 8 of its 9 specialties sit above the national median. It has the deepest (or joint-deepest) review profiles of any metro in dental, dermatology and physical therapy.

Dermatology: the deepest specialty market in the study
New York dermatology has the highest median review count of any specialty-and-city group in our benchmark, higher even than any city's dental median. The median practice has 513.5 Google reviews; the next-deepest dermatology metro has less than half of that. 50% of New York dermatology practices have 500 or more reviews and only 5.4% have fewer than 50. Ratings average 4.59 stars, close to the national 4.62, so the gap is volume, not satisfaction.
The access picture is unusual too: 59.5% of New York dermatology practices list a weekend day, against 27.5% nationally. In most cities weekend hours are a way to stand out. In New York dermatology they are close to expected.
How long it takes to reach the New York median
Review depth translates directly into time. The median dental practice in our national subsample adds about 5.2 reviews a month. A practice starting from zero, collecting at a steady pace, would need this long to reach the typical New York competitor:

Our arithmetic: New York median ÷ monthly review pace, rounded up. Competitors keep adding reviews, so real catch-up is slower.
Dentistry shows the scale of the task. Only 0.8% of New York dental practices have fewer than 50 reviews, and 40.9% have 500 or more. A new practice cannot out-review that field in its first year, so it needs other ways to be found.
And the clicks are expensive
Some deep review markets offset the organic barrier with cheap clicks. New York does not. In our 2026 planning estimates for Manhattan, general-dentistry clicks cost about $21.10 (the 2nd most expensive of 20 metros), orthodontic $35.40 and cosmetic dentistry $58.90. Medical dermatology is the most expensive of 19 metros at $17.60, and cosmetic dermatology about $52.80.
Fertility is the relative bargain. New York requires large-group insurance plans to cover IVF, so many patients search for a clinic in their network rather than shop on price, and IVF clicks run about $14.90 with a new consultation at about $420, well below Miami or Austin.
The practical upshot: in New York, waste is expensive. Tight geographic targeting, specific procedure keywords and landing pages that convert matter more here than in any other metro we track.
Where a smaller practice can still compete
- Newer cash-pay categories. New York medical weight loss clinics have a median of 34.5 reviews, below the national 42; 57.9% have fewer than 50.
- Neighborhood relevance. Google weighs distance. A practice that dominates its own neighborhood does not need to out-review Manhattan's largest groups.
- Narrow procedures. Pages built around one condition or procedure can rank and earn AI-answer citations where a general "dermatologist near me" page cannot.

- Size the gap honestly. Compare your review count with the New York median for your specialty, not the national one.
- Win the neighborhood first. Concentrate profile work, local pages and ads on the few ZIP codes your patients actually come from.
- Treat reviews as a standing program with a monthly target well above the national median pace, and a reply to every review.
- Buy clicks precisely. At New York prices, broad keywords burn budget; measure cost per booked patient weekly.
- Specialize your content. Procedure- and condition-level pages are where a smaller practice can outrank a bigger one.
