Very, at least on Google reviews. Houston ranks 2 of 20 US metros in our 2026 review-depth index (1.41, behind New York), and it has the deepest review profiles in 3 of 9 specialties. In our benchmark of 513 Houston practices, the median dental practice has 408 Google reviews against 319.5 nationally. Paid clicks are cheaper than in most large metros.

Houston is the second-deepest market of 20

"Competitive" in medical SEO mostly means one thing in the map results: how much review history the practices around you already have. Ratings barely separate anyone, because almost every practice sits between 4.7 and 5.0 stars. Review depth does. To compare cities fairly across specialties with very different review counts, the State of US Healthcare Marketing 2026 uses a review-depth index: each specialty's Houston median divided by its national median, then the median of those ratios.

Houston scores 1.41. Put plainly, the typical Houston practice carries about 41% more Google reviews than a practice in the same specialty in the average large metro. 6 of its 9 specialties sit above the national median.

Hardest corners

Where Houston is hardest: elective and wellness care

Houston has the deepest review profiles of every metro we measured in IV therapy and wellness, medical weight loss and hair restoration. These are cash-pay categories where patients compare several clinics before booking, and Houston clinics have built review profiles to match. A hair restoration clinic in Houston has a median of 121 reviews; nationally the figure is 33. Only 30.3% of Houston medical weight loss clinics have fewer than 50 reviews, against 53.8% nationally.

Dentistry is deep too. The Houston median of 408 ranks 3rd of 20 metros, and 38.2% of practices have 500 or more reviews. Dermatology, at 223, is close to double the national median, and plastic surgery (151) ranks 2nd.

What that means in time: a new dental practice that collects 15 reviews a month, roughly three times the national median pace in our dental study, would need about 28 months to reach the Houston median. That is why few Houston practices win the map pack on SEO alone in year one.

Room to move

Where there is still room

Not every Houston category is crowded. Physical therapy clinics have a median of 99.5 reviews, below the national 122, and chiropractic sits near the national figure at 120. In these categories, a disciplined review routine and a well-built Google Business Profile can move a practice into the top tier far faster.

Access is the other gap. Houston dental practices are more likely than average to list a weekend day (38.2% against 25.5%), so Saturday hours are less of a differentiator there. But only 4.3% of Houston plastic surgery practices and 23.8% of dermatology practices list weekend hours. A practice in those specialties that offers weekend consults, and shows it accurately on Google, stands apart in the results without waiting years for reviews.

Paid search

The paid side is cheaper than the reviews suggest

Houston's organic barrier is high, but its click costs are not. In our 2026 planning estimates, a general-dentistry click in Houston costs about $8.60, the 13th most expensive of 20 metros, and well under half the San Francisco figure. Orthodontic clicks run about $15.80 and cosmetic dentistry about $28.40. Medical dermatology is about $6.60 a click and cosmetic dermatology $17.20.

Fertility is the exception. Houston IVF clicks are about $20.80 (the 5th most expensive of 16 metros) and a new patient consultation about $720. Texas requires group insurers only to offer IVF coverage, not to include it, so more Houston patients shop on price and packages, which pushes bids up.

The combination matters for planning. Where reviews are deep and clicks are moderate, paid search is the fastest way to be seen while the organic profile matures.

  1. Benchmark against Houston, not the country. A review count that looks healthy nationally can be below the Houston median in your specialty.
  2. Plan reviews as a 12- to 24-month program in dental, dermatology and the cash-pay categories, with a monthly target and an owner reply on every review.
  3. Use paid search as the bridge. Houston clicks cost less than in most large metros; track cost per booked patient, not cost per click.
  4. Compete on access where peers are closed: weekend consults in dermatology and plastic surgery, published accurately on Google.
  5. Pick your battles. In physical therapy and chiropractic, local SEO can pay back much sooner than in hair restoration or weight loss.

How we know: source, sample, date and limits

Google benchmark: public Google Business Profile data for 513 Houston practices in 9 specialties, found through Google searches and collected in September 2026, part of the 9,452-practice, 20-metro sample in the State of US Healthcare Marketing 2026. Review-depth index: for each specialty, the Houston median review count divided by the national median; the index is the median of those ratios. Click costs: planning estimates from the dental, dermatology and IVF benchmark reports (January to August 2026, client data plus third-party cost-per-click signals).

  • Specialty groups range from 19 to 123 Houston practices; small groups (hair restoration, medical weight loss) carry more uncertainty.
