Direct answer

Google's algorithm for local search rankings is built on three pillars:

TL;DR

Google's algorithm for local search rankings is built on three pillars:

The Google Business Profile Ranking Factors

Google's algorithm for local search rankings is built on three pillars:

1. Relevance: Does your GBP listing match what the searcher is looking for? A dermatology clinic with "dermatology" as its primary category and "acne treatment", "laser skin treatment" as service keywords will rank better for dermatology searches than a generic "health clinic."

2. Distance: How close is your clinic to the searcher's location (or the location they are searching about)? This is partly fixed — you cannot change your address. But service area configuration, GMB authority signals, and review density can compensate for distance.

Written summary on the call. EMQ, intent tier, IVR leak, funnel drop-off. No commitment.

3. Prominence: How well-known and trusted is your clinic according to Google's signals? This is driven by review count and rating, website authority, citations (your NAP — name, address, phone — appearing correctly across directories), and engagement signals (direction requests, phone calls, website clicks from your GBP).

Of these three pillars, Relevance and Prominence are the ones you actively manage. Here is how.

Step 1: Claim and Verify Your Google Business Profile

If you have not already: go to business.google.com, search for your clinic name, and either claim an existing listing or create a new one. Verification requires a postcard sent to your physical address (5–7 business days) or, in some cases, phone or email verification.

For multi-location chains: each location needs its own GBP listing, its own verification, and its own management. Do not try to manage 10 locations from a single GBP — this violates Google's policies and risks all listings.

Step 2: Choose the Right Primary Category

This is the single most impactful decision in your GBP setup. The primary category tells Google what type of business you are and determines which searches your listing is eligible to appear for.

Healthcare-specific category guidance:

SpecialtyCorrect Primary Category
DermatologyDermatologist
Aesthetics / Skin treatmentsMedical Spa OR Skin Care Clinic
IVF / FertilityFertility Clinic
Hair TransplantHair Transplantation Clinic OR Hair Removal Service
DentalDental Clinic OR Dentist
Mental Health / PsychiatryMental Health Clinic OR Psychiatrist
Multispecialty HospitalHospital
AyurvedaAyurvedic Clinic
Orthopaedics (single specialty)Orthopedic Surgeon OR Orthopedic Clinic

The mistake to avoid: Choosing "Medical Clinic" or "Health Clinic" as your primary category when a more specific option exists. Google rewards specificity. A clinic listed as "Dermatologist" will outrank one listed as "Medical Clinic" for dermatology searches.

Secondary categories: Add up to 9 secondary categories for each relevant specialty or service. A dermatology clinic offering laser treatments can add "Laser Hair Removal Service" and "Medical Aesthetics" as secondary categories.

Step 3: Complete Every Profile Section

Google uses the completeness of your profile as a relevance signal. An incomplete profile ranks lower than a complete one, all else being equal.

Profile sections to complete (in priority order):

Business Name: Use your exact legal business name. Do not add keywords to your business name (e.g., "Dr. Sharma's Clinic — Best Dermatologist in Delhi") — this violates Google's policies and can result in listing suspension.

Address: Use the exact address format that appears on your local government registration. Consistency between your GBP address and your website contact page and all online directories is critical.

Phone Number: Use a trackable number if your IVR system supports it — this connects your GBP calls to your ad attribution. Use a number that forwards to your main clinic line.

Website URL: Link to your main website homepage or a location-specific landing page for multi-location chains.

Business Hours: Keep these accurate. Outdated hours lead to patients showing up when you are closed — directly damaging reviews and trust.

Description (750 characters max): Include your primary specialty, location, and the specific conditions or treatments you offer. Example:

"[Clinic Name] is a specialist dermatology clinic in [location], offering acne treatment, laser hair removal, chemical peels, PRP for hair loss, and anti-ageing treatments. Our team of dermatologists has [X years] combined experience and has served [X+] patients across [city/region]. Consultations available Monday–Saturday, with evening appointments available."

Services: Add each service you offer individually. Include the service name, description, and price (if you are comfortable disclosing it). This directly affects which service-specific searches your listing appears for.

Attributes: Select every relevant attribute — "wheelchair accessible," "online appointments," "accepts walk-ins," "serves Hindi," etc. These filter options are used by patients to narrow searches.

Step 4: Upload High-Quality Photos and Videos

Google shows that profiles with photos receive significantly more direction requests and calls than profiles without. Photo quality signals directly to the relevance and prominence scores.

Photo types to upload:

Photo TypeRecommended QuantityWhat to Show
Exterior3–5Building facade, entrance, parking, signage
Interior5–8Waiting room, consultation rooms, treatment areas, equipment
Team3–5Doctor headshots, team group photo
Treatment/procedures5–10Clinical photos (no patient identifiable information)
Logo1Official clinic logo, white or transparent background
Cover photo1Best exterior or interior shot, 16:9 ratio

Important: No before-and-after patient photos without written consent. No identifiable patient images at all unless written, documented consent exists.

Videos: Upload at least one 30–60 second clinic tour video. Videos significantly increase engagement signals.

Step 5: Build Your Review Base

Reviews are the most visible prominence signal in local search. The two metrics Google weighs most heavily: total review count and average rating.

The review velocity target: Minimum 5 new reviews per month for an active clinic. A clinic receiving 5 new reviews per month will grow its review base significantly faster than one that asks once at launch and then forgets.

Angryturtle · Auto Review UploaderNMC-compliant review acquisition + response workflow at scale. Content Studio pre-checks every reply against NMC · ASCI · DPDP · ART Act.

When to ask: The optimal moment is immediately after a positive patient outcome — the end of a successful consultation, the day after a procedure when the patient messages to say they are happy, after a follow-up visit showing good progress.

How to ask (WhatsApp):

"Hi [Name], I am so glad to hear your [treatment] is going well! If you have a moment, would you mind sharing your experience on Google? It really helps other patients who are looking for the right clinic. Here is the link: [Google review link]. Thank you!"

The Google review link: Get your direct review link from your GBP dashboard. It shortens the review process to one click.

What you cannot do: Offer incentives for reviews (vouchers, discounts, gifts). Google prohibits this and will remove reviews if it detects a pattern.

Step 6: Manage Questions and Answers (Q&A)

The Q&A section of your GBP allows anyone to ask a question — including the business owner. This is an underused feature.

Pre-populate your own Q&As: Log into your GBP and ask the questions patients most commonly ask you, then answer them yourself. This:

  • Makes your listing more informative
  • Reduces phone calls for routine questions
  • Signals to Google that your listing is actively managed

Common healthcare Q&As to pre-populate:

  • "Do you offer consultations for [primary condition]?"
  • "What is the consultation fee?"
  • "Do you have parking available?"
  • "Are you open on weekends?"
  • "Do you accept insurance / cashless treatment?"
  • "How do I book an appointment?"

Answer each concisely and accurately. Update answers when details change.

Step 7: Post Weekly on GBP

GBP Posts are short updates (100–300 words) that appear directly on your listing. Clinics that post regularly show higher engagement rates and maintain better local rankings than those that post infrequently.

Post types:

  • Update: New doctor joining, new equipment, new treatment available
  • Event: Health camp, free consultation day, awareness event
  • Offer: Introductory package (compliant with healthcare advertising guidelines)
  • Health tip: Educational content about a condition or treatment you offer

Posting cadence: Weekly minimum. Posts expire after 7 days for most types — so weekly posting means your listing always has fresh content.

The compliance note: Avoid unsubstantiated health claims ("We cure all skin conditions") or testimonial-style content that implies guaranteed outcomes. Indian Medical Council and Google's healthcare advertising policies both restrict this.

Step 8: Build Citations (NAP Consistency)

A citation is any mention of your clinic's Name, Address, and Phone number (NAP) on another website. Consistent citations across directories reinforce your local relevance to Google.

Essential citation directories for Indian healthcare clinics:

  • Practo
  • JustDial
  • Lybrate
  • 1mg
  • Sulekha
  • HealthGrid
  • IndiaMart (for medical equipment businesses)
  • Local chamber of commerce / association websites
  • Your professional body's member directory (IMA, IADVL, etc.)

The consistency rule: Your NAP must be identical across every citation and your website. "Dr. Sharma Dermatology Clinic" vs "Dr. Sharma's Dermatology Clinic" vs "Sharma Dermatology" are three different entities to Google. Pick one exact format and use it everywhere.

Step 9: Track Your GBP Performance

Check these monthly from your GBP dashboard:

MetricWhat it tells you
Search queriesWhat patients searched when they found you
ViewsHow many times your listing appeared
Direction requestsPatients looking up how to reach you
Phone calls from GBPDirect patient calls from the listing
Website clicksPatients clicking through to your site

Benchmark (established clinic, metro city):

  • 500–2,000 monthly views
  • 50–200 monthly calls
  • 100–400 direction requests

Declining calls month-over-month without a clear seasonal reason is a signal to check ranking positions and recently received reviews.

Frequently asked

Questions readers ask
about this topic.

For a new clinic with no GBP history: 3–6 months with consistent optimisation. For an existing clinic with an unclaimed or incomplete profile: improvements can appear within 4–8 weeks of optimisation. For highly competitive queries in large cities: 6–12 months.

In competitive searches, the tie-breaker is usually review count and velocity. Check their review count versus yours. If they have significantly more reviews, that is almost always the primary ranking factor. Additionally, their website authority (backlinks) may be higher, supporting their local ranking.

No. Each physical location must have its own GBP listing with its own address. Attempting to list multiple locations under one GBP violates Google's guidelines and will result in listing penalties.

Claim it through the "Claim this business" process in Google Business Profile Manager. You will need to verify ownership, which requires access to the listed phone number or address. If you cannot complete verification, contact Google Business Profile support.
