Direct answer

A workable, data-informed monthly Instagram calendar template for IVF clinics — pillars, cadence, format ratio and the KPI check every account manager should run before publishing.

TL;DR

A workable, data-informed monthly Instagram calendar template for IVF clinics — pillars, cadence, format ratio and the KPI check every account manager should run before publishing.

Most IVF clinic Instagram calendars we audit are built on a Google Sheet, filled in by a junior on the last Friday of the month, and forgotten by the second week when a policy change or a competitor moment forces a scramble. This piece lays out the calendar template ICG uses inside Prism Pulse — our Instagram analytics and content-strategy console for healthcare — and the specific decisions an account manager should make before locking the month.

This is a B2B guide for the agency or in-house team running the account, not a patient-education piece. If you are a clinic founder reading this, forward it to your marketing lead — the value is in the workflow, not the topic list.

Prism Pulse monthly content calendar for an IVF clinic, colour-coded by Reel Feed and Story
The Prism Pulse content calendar view — one month at a glance, colour-coded by format, editable by the account team.

Start with the four content pillars every IVF Instagram account needs

An IVF Instagram account that tries to post about everything ends up ranking for nothing and enquiring from nobody. The tightest working structure we have seen across 30+ IVF clinics is four pillars, each doing a distinct job in the funnel.

Pillar one is education — myth-busting Reels, procedure explainers, cost transparency, timeline pieces. This pillar drives saves and time on profile. It builds the informed audience that eventually enquires. Weight it heaviest for a young account.

Pillar two is patient stories — consented, medico-legally reviewed narratives from actual patients. This pillar drives enquiries. It is the single highest-converting format across almost every IVF account we track. Weight it heaviest for an account with reach but low enquiry pull.

Pillar three is clinician credibility — the specific doctors, their credentials, a look at the lab, the tech, the accreditations. This pillar drives trust once a prospective patient has already found you. It rarely drives cold reach, but it closes warm leads.

Pillar four is community and moments — awareness days (World IVF Day, PCOS Awareness Month), clinic milestones, team introductions. This pillar drives shareability and human warmth, which is the antidote to feeling clinical. Weight it light — around 15% of the month.

Cadence: how much to publish without burning the team

The single most common mistake we see is over-publishing. A clinic that publishes 30 pieces a month with variable quality gets worse reach than the same clinic publishing 18 with tight quality. The Instagram ranking system rewards engagement per piece, not volume, and it punishes accounts whose average performance is dragged down by low-effort filler.

A workable cadence for an established IVF clinic is 16-22 posts a month total, split roughly 60% Reels, 25% Feed carousels, 15% Stories. That is 10-13 Reels a month, 4-5 Feed carousels, 2-3 Stories a week on top. A young account (under 5K followers) can start lighter at 12-14 posts a month and lean 70% Reels. A mature account (over 50K followers) usually plateaus at 22 posts and shifts weight toward patient stories and clinician credibility.

The service-to-KPI map that drives the calendar

Once you have the four pillars and a working cadence, the next question is which specific themes to weight this month. The Programming view inside Prism Pulse does this by holding the last 90 days of posts against the KPI each drove — reach, saves, enquiries, or low pull.

Content themeBest atWeight when
Success-story ReelsEnquiriesEnquiries below monthly target
Myth-busting Reels (cost, age, timelines)ReachFollower growth has plateaued
Procedure explainer carouselsSavesSave rate is trending down month over month
Doctor-led explainers (video)Trust + enquiriesBounce from bio to website is high
Lab and tech toursTrustNew tech or accreditation to announce
Awareness-day postsCommunityWeight light — always at 10-15%, never more
Discount or offer postsLow pullAlmost never. If a promotion is running, use one anchor post, not five.

The calendar is not a template you paste each month. It is a weighting decision informed by which KPI is under pressure. That is the difference between a data-driven calendar and a spreadsheet.

The monthly build workflow — 90 minutes done well

Building the calendar for a well-run IVF account takes 90 minutes if the last month's data is already tagged. Longer if you are cleaning up historical taxonomy for the first time.

  1. Read the last month's Prism Pulse Overview (10 minutes). Which KPI moved, which stayed flat, which slipped. This is the input.
  2. Open the Programming view (10 minutes). Which pillars drove which KPI. Note the two winners and the two draggers.
  3. Draft the pillar weighting for next month (15 minutes). Adjust from the default 40/30/20/10 split based on the KPI under pressure.
  4. Slot posts into the calendar (30 minutes). Cluster patient stories on high-intent days (Mondays and Thursdays perform best for IVF enquiries). Slot educational Reels on Sundays and Wednesdays. Keep Fridays for team or community pieces.
  5. Book Story slots and any live sessions (10 minutes). Stories are a weekly rhythm, not a monthly plan — leave the specific captions for the week they run.
  6. Send the calendar to medico-legal review (15 minutes buffer). Every patient story needs signed consent and clinical approval. Every doctor-led piece needs the doctor's approval on the specific script. Never skip this.

Ninety minutes a month. If the calendar takes four hours, either the last month's data was not tagged or the pillar weights are being renegotiated every cycle. Both are fixable.

The mistakes that kill IVF Instagram accounts

Three specific mistakes recur across accounts that stall. First, the founder wants to post about every awareness day, which dilutes the pillar weight and confuses the algorithm about who the audience is. Push back — pick two awareness days a month, not eight.

Second, generic discount posts get greenlit under enquiry pressure. They pull low-intent traffic that unfollows the next week and pulls the account's baseline engagement rate down for months. Prism Pulse's Programming view flags these as "low pull" for a reason.

Third, the calendar gets built without a review of last month's Comparison view. If reach dropped 15% last month, publishing the same mix again will produce the same drop. The calendar is the response to the data, not a schedule for its own sake.

Time-of-day and day-of-week patterns specific to IVF audiences

IVF audiences behave differently from general healthcare audiences. Prospective patients tend to research at night, often between 9pm and midnight after the household has settled, and often on weekends. Reels published Sunday evening consistently outperform Tuesday morning across the accounts we have tracked over the last 12 months.

Enquiry Reels — the ones designed to trigger a DM — do best when published Sunday evening or Monday morning, catching the top-of-week decision moment. Educational Reels do best mid-week (Tuesday to Thursday) when the audience is scrolling more casually. Saturday afternoon is a quiet slot; use it for community pieces that do not need to drive an action.

The specific pattern for your account will differ, and the Prism Pulse Content view will surface it inside the first 60 days. Do not lock your day-of-week rules to what a Twitter thread claims is best. Look at your own data, tag your posts by publish day and time, and let the Programming view show you the actual pattern.

Where the calendar plugs into the rest of the clinic's marketing

The Instagram calendar does not stand alone. It should feed WhatsApp broadcasts (the enquiry pathway), Google Business Profile posts (the local-SEO pathway), and the clinic's website blog (the long-tail SEO pathway). Prism Pulse handles the Instagram layer of this multi-channel calendar. For the Google Business Profile layer, ICG's Angryturtle handles the same analytics-plus-calendar pattern for GBP posts.

If the clinic is also running paid social, the calendar for organic posts and the ad-creative pipeline should be built in the same session — organic Reels that perform well become the seed for paid creative iteration, and the workflow across both is where most of the compounding lives. Treat the Instagram calendar as the source-of-truth creative pipeline, not as an afterthought that gets stitched together at the end of every month.

Frequently asked

Questions readers ask
about this topic.

Most established IVF clinics we work with hit the right cadence at 16-22 posts a month across Reels, Feed and Stories combined — roughly one Reel every other day plus two Stories a week. Higher cadence rarely improves reach and often burns the creative team.

A working starting ratio is 60% Reels, 25% Feed, 15% Stories. Reels drive reach, Feed drives saves on educational content, Stories drive DM enquiries from the already-warm audience. Prism Pulse's Formats view will tell you within 60 days whether your specific account should shift the mix.

Plan the pillars monthly, plan the specific posts weekly. IVF clinics that lock in every post six weeks out find themselves publishing stale content when a policy change or a competitor moment demands agility. Monthly frame, weekly refresh is the durable pattern.

Patient stories require signed consent that specifies Instagram usage rights, medico-legal review of the specific caption and creative, and a clear line on which clinical claims are permissible. Every clinic we work with has a written content-consent SOP — if yours does not, that is the first artefact to build before you scale content.

The Programming view shows which content pillars drove which KPI over the last 90 days. Next month's calendar starts from that map — patient stories weighted heavier if enquiries are lagging, educational Reels weighted heavier if saves are the constraint. The calendar is data-informed, not a monthly guess.

Guaranteed-outcome claims, before/after comparisons of embryos or scans without extensive medico-legal review, and generic discount posts that pull low-intent traffic. All three attract engagement but hurt the enquiry mix and can trigger consumer-protection complaints in India.

Reach improvements from a Programming-informed calendar typically show up in weeks 3-6. Enquiry-mix improvements — where the same reach delivers more qualified DMs — usually show up in months 2-3 as the audience composition shifts.
