Google Local Services Ads (LSA) are pay-per-lead ads that appear at the top of some local searches, and in the US they are open to a set of healthcare categories including dentists, primary care physicians, dermatologists, chiropractors and physical therapists. To run them, the practice and each provider must pass Google's screening, which for healthcare includes background checks, liability and malpractice insurance, state licenses and National Provider Identifier (NPI) verification. Local Services Ads appear above or among the results for some local searches, such as "dentist near me" or "chiropractor in [city]". Each ad shows the practice name, rating, review count, hours and a Google verification badge. A searcher taps to call or message. Unlike Search ads, there are no keywords to choose and no landing page: the ad is built from your LSA profile.

Which healthcare practices are eligible

Google's US screening and verification page lists these healthcare categories:

  • Allergist, audiologist, chiropractor, dentist, dermatologist, dietitian
  • Occupational therapist, ophthalmologist, optometrist, orthodontist, orthopedic surgeon
  • Physical therapist, plastic surgeon, podiatrist, primary care physician

Categories and availability change and vary by location, so check Google's current list and the sign-up flow for your ZIP codes. Specialties not on the list, such as med spas, pediatric subspecialties or behavioral health, rely on Search ads and organic local search instead.

Most delays come from mismatches: a provider's name spelled differently on the license, insurance certificate and NPI record, or an old practice address in NPPES. Fix those before you apply.

Step by step

Setting up Local Services Ads for a practice

  1. Confirm availability. Check that your category is offered in your area in the LSA sign-up flow.
  2. Clean up records. Make names and addresses consistent across state license lookups, NPPES, insurance certificates and your Google Business Profile.
  3. Create the profile. Choose your categories and the services you actually offer. Turn off services you don't want leads for. Set your service area realistically.
  4. Add providers. Each listed provider goes through their own checks. Start with the providers who drive new-patient demand.
  5. Upload documents and complete checks. Respond quickly to any follow-up requests from Google.
  6. Link your Business Profile and reviews. Reviews and rating affect ranking and how the ad looks.
  7. Set hours and budget. Only run during hours someone answers the phone. Set a budget and bidding approach you can review weekly.
  8. Train the front desk. LSA leads arrive as calls and messages. Answer fast, book in the call and mark lead outcomes in the LSA dashboard.

What decides your ranking and what you pay

Google describes LSA ranking as an auction that weighs your bid together with lead likelihood and profile quality:

  • Bid: the most you're willing to pay for a lead. You may pay less.
  • Responsiveness: missed calls can lower your ranking. Google may show estimated message response times on ads once you have enough message leads.
  • Reviews and rating: higher ratings and more reviews help ranking and stand out on the ad.
  • Profile quality: response time, good images, completed verification checks and complete business information.
  • Search context: the service searched for, time, location and other factors.

Charging and credits

You pay for valid leads. Google assesses leads at first contact and doesn't charge for leads it judges invalid. In other categories, Google may later credit low-quality leads automatically, but its help page states that lead credits aren't available for health care verticals. Calls about services you don't offer, or from outside your area, can still cost money, which is why tight service and area settings matter.

Measuring LSA properly

  • Mark every lead's outcome in the dashboard (booked, not booked, not a patient enquiry).
  • Reconcile booked leads with attended first visits in your practice management system every month.
  • Compare cost per booked visit with your Search campaigns for the same service and area. That comparison, not cost per lead, decides where the next dollar goes.
Privacy

Handling LSA leads with patient privacy in mind

LSA leads are often a person's first contact with your practice, and they may share health details straight away. A few habits keep that information where it belongs:

  • Reply to message leads with a short invitation to call or book, not with questions about symptoms or history.
  • Move the conversation into your phone system and practice management or booking software as soon as possible.
  • Check whether call recording is switched on in your account and decide, with your privacy lead, whether to keep it.
  • Don't copy lead details into spreadsheets or tools that aren't covered by your HIPAA policies and BAAs.

What's changing

LSA is moving into Google Ads

Google has announced that Local Services Ads are transitioning into Google Ads as Performance Max campaigns with pay-per-lead goals. According to Google's help page:

  • August 2026: initial migration for selected US home and storefront service categories.
  • Late 2026: expansion to service-area businesses without storefronts and accounts with custom bidding or booking setups.
  • 2027: non-US accounts and remaining business categories.

The pay-per-lead model stays, campaign settings transfer automatically, and the verification badge carries over for eligible verified businesses. Weekly budgets become daily averages, manual bidding goes away, and LSA performance reports won't move to Google Ads, so download your historical LSA reports before your account migrates.
