A hospital group CEO at ₹80 crore revenue with no dedicated marketing leadership called me last quarter. The question: "Do I hire a full-time CMO or engage a Fractional CGO?"
My honest answer was: it depends on 5 factors, and for 3 of the 5 factors for that specific organisation, the Fractional CGO was the better decision. But let me walk through the framework, because the answer genuinely does depend.
The 5 decision factors
Factor 1 — Revenue scale and marketing complexity
Below ₹50 crore revenue, single specialty or single location: the full-time CMO hire is not justified. The marketing programme is not complex enough to require full-time C-suite leadership. An agency (ICG or equivalent) with a senior account lead is adequate.
₹50–150 crore, multi-location or multi-specialty: this is the Fractional CGO zone. The programme is complex enough to require strategic leadership (multi-location CPQL architecture, board-level reporting, cross-specialty campaign management), but not so large that a full-time CMO with a 10+ person team is necessary.
Above ₹300 crore, large multi-hospital group: the full-time CMO with an in-house team is justified. The programme volume (₹3–6 crore/month in marketing spend, 15+ location campaigns, complex HIS integrations) requires dedicated full-time leadership.
Factor 2 — Existing marketing team maturity
If the existing marketing team is 2–4 people with solid execution capability but no strategic direction: Fractional CGO. The team executes; the CGO sets strategy and owns board accountability.
If there is no existing marketing team at all: the organisation needs both strategy and execution. A Fractional CGO + full execution agency (ICG) covers both. A CMO hire + building a team takes 12–18 months to reach operational capability.
Factor 3 — Speed of the growth mandate
"We need to grow patient volume by 40% in 18 months" is a mandate that requires execution capability from day 1 — not a 6–9 month CMO hiring process. The honest answer for the CEO who called me
₹80 crore revenue, 4-hospital group, existing 3-person marketing team, board pressure on marketing ROI, no attribution infrastructure, 18-month growth mandate.
The Fractional CGO model was right for 4 of the 5 factors: revenue scale (₹50–150 crore zone), execution team maturity (exists, needs strategy not more execution), growth mandate speed (urgent — 4 weeks to operational vs 12+ months for CMO hire), and portfolio benchmark need (their marketing team had no benchmark data, ever).
The one factor that tilted slightly toward the CMO: they wanted someone fully embedded in the organisation culture, attending daily leadership meetings, physically present in the office. The Fractional CGO model's monthly strategic session + quarterly board presentation does not replicate full-time presence.
My recommendation: Fractional CGO for the first 18 months (while the attribution infrastructure is built and the CPQL improvement is demonstrated), then evaluate the CMO hire decision with the benefit of 18 months of CPQL data that will inform the job description, the KPIs, and the performance framework for the full-time hire.
The CEO took the recommendation. The engagement started 3 months ago. CPQL is improving.
