Which US cities have the most expensive Google Ads for medical practices?
In our 2026 benchmarks, the San Francisco Bay Area and Manhattan are the most expensive metros for practice Google Ads: $22.50 and $21.10 per general-dentistry click, and $16.20 and $17.60 per medical-dermatology click. Nashville and San Antonio are the cheapest at $3.80 to $4.90. Fertility breaks the pattern: Miami ($28.40) and Austin ($26.30) top the IVF table.

Dentistry gives the cleanest city comparison because our US Dental Marketing Benchmark 2026 covers all 20 of the largest dental search markets. The most expensive metro pays more than five times the cheapest for the same general-dentistry click.
Google Ads cost per click, general-dentistry keywords, 2026 planning estimates. Orthodontic CPC runs about 1.7–1.8× these figures; cosmetic about 2.7–2.9×.
Dental, dermatology and IVF side by side
Dentistry and dermatology follow the same map: the coastal, high-income metros cost most, the Sun Belt and Southeast least. Fertility does not. Its most expensive clicks are in metros where IVF is mostly paid out of pocket.

What drives the gap between cities
Competition and patient value
Click prices are set in an auction, so they rise where more practices bid on the same searches and where each new patient is worth more. Manhattan, the Bay Area, Boston and Los Angeles combine dense practice competition, high incomes and large cash-pay segments. That lifts every bid, and cosmetic searches most of all: cosmetic dentistry clicks run $51 to $63 in those four metros.
Insurance coverage
Fertility shows how coverage changes searching. Where insurance covers IVF, patients mostly search for a provider. Where it does not, they compare packages, prices and financing, and clinics bid hard on those searches. The fertility benchmark puts IVF clicks at $9 to $17 in coverage-mandate metros and $18 to $34 elsewhere, with Miami ($28.40), Austin ($26.30) and Atlanta ($22.10) at the top.
Your own account
These are market reference points. Your own cost per click also depends on keyword choice, match types, ad quality, landing pages and schedule. A well-built account in an expensive metro can beat a careless one in a cheap metro on cost per booked patient, which is the number that matters.
What this means for your practice
- Check your metro row in the dental benchmark (or the dermatology and IVF reports) before setting a budget.
- In top-tier metros, narrow the keyword set. Pay for high-intent searches near your location; skip broad research terms.
- Lean on organic where clicks are dear. A strong Google profile and reviews earn map visibility you do not pay per click for.
- Judge on cost per booked patient, not cost per click. A $20 click that books is cheaper than five $4 clicks that do not.
- Review quarterly. Auction prices move with season and local competition.

Why are IVF Google Ads more expensive in Texas and Florida?
In our fertility benchmark, clicks cost more in metros where insurance does not cover IVF, because patients compare packages and prices and clinics bid on those searches. Miami ($28.40), Austin ($26.30) and Atlanta ($22.10) top the table; covered metros such as Denver ($10.80) and Chicago ($12.40) are far lower.
Is $20 a day enough for Google Ads in an expensive city?
At $20 to $22 per click, $20 a day buys about one general-dentistry click in San Francisco or Manhattan, against four or five in Nashville. In high-cost metros a small budget is usually better spent on a narrow set of high-intent keywords or on reviews and the Google profile first.
