How do I choose a med spa marketing agency?

Choose a med spa marketing agency by testing five things: whether it understands aesthetic treatments and their rules, how it measures booked consultations, who owns your accounts, how it handles patient data, and whether it can show a specific 90-day plan for your market. Shortlist two or three agencies, score them on the same criteria, and decide on evidence rather than the pitch.

Guide for US practice owners · Published October 15, 2026

TL;DR
  • Write your goals first: which treatments, which locations, how many consultations a month. An agency can only be judged against a target.
  • Score every shortlisted agency on the same criteria, using a simple weighted scorecard.
  • Insist on booking-level reporting and accounts held in your name, with the agency given access.
  • Ask for a BAA and a written approach to tracking on booking and intake pages before any campaign goes live.
  • Every practice welcome — retainers from $499/mo, Goals-Driven engagements, Performance-Linked Payout Models available.

“They were able to get all my ideas and work with me over a period of three to four months and create this amazing website. It's super customized, very modern, and it incorporates all the elements that I had wanted — the patient portal, nice pictures, a very interactive website, patient reviews. I would highly recommend their company to anyone who wants to make an excellent website.”

Before you shortlist

What should I decide before talking to agencies?

Decide what you want marketing to change, by when, and what you can spend. Agencies pitch best when you give them a clear written brief, and you can only compare their answers fairly if every agency responds to exactly the same one.

A one-page brief is enough. Include:

  • Priority treatments: for example injectables, laser hair removal, body contouring or a weight-loss program, ranked by margin and capacity.
  • Locations and service area: the neighborhoods or suburbs you realistically draw clients from.
  • Current numbers: monthly inquiries, booked consultations, show rate and rebooking, even if rough.
  • Budget: the total for agency fee plus ad spend, and how long you can commit before judging results.
  • Constraints: provider availability, the treatments you cannot take more of, and any claims your medical oversight team has asked you to avoid.

If you are still deciding whether to hire an agency at all, read med spa marketing agency vs in-house first.

Should I pick a single-channel specialist or a full-service agency?

Pick a single-channel specialist when one channel clearly limits growth and the rest works. Pick a full-service agency when the problem spans the booking path, such as ads that generate inquiries nobody follows up, or a website that does not convert the traffic you already get.

Single-channel specialists, for example a paid search shop or a local SEO consultant, can go deep. The cost is coordination: you become the person who connects ads, website, reviews and front-desk follow-up, and each vendor reports its own numbers in its own way.

A full-service agency owns more of the path and can be held to one outcome, booked consultations. The risk is breadth without depth. Test it by asking who on the team actually runs each channel and how often they review it.

Many med spas find it simpler to work with one accountable lead agency plus their own front-desk process. Whichever you choose, make one party responsible for the end-to-end report, so no one can claim credit for leads that never booked.

Scorecard

What criteria should I use to compare med spa agencies?

Use a weighted scorecard so the decision rests on the same criteria for every agency. Score each from one to five after the pitch and reference calls, multiply by the weight, and compare totals. Adjust the weights to your goals, but keep them fixed once the shortlist is set.

CriterionWhat a strong answer looks likeSuggested weight
Aesthetic-treatment knowledgeTalks fluently about consultation-led treatments, seasonality, memberships and the role of the supervising medical professionalHigh
MeasurementReports booked consultations and revenue by source, reconciled with your scheduling systemHigh
Compliance habitsHas a review process for claims, before-and-after photos and testimonials; signs a BAAHigh
Account ownershipAd accounts, analytics, Google Business Profile and website held in your nameMedium
Specific planA first 90-day plan for your treatments and market, not a generic deckMedium
Team and communicationNamed point of contact, clear response times, a fixed reporting rhythmMedium
Contract termsReasonable minimum term, clear notice period, clean handover clauseLow to medium

Price is deliberately not on the scorecard. Compare it after scoring, so a cheap quote does not hide a weak plan. Our guide on what a med spa marketing agency costs explains how to compare fees.

The pitch call

What questions should I ask a med spa agency on the first call?

Ask questions that force specific answers about your med spa, not general claims about the agency. Good agencies answer with process, examples of how they would approach your treatments, and honest limits. Vague answers about "growth" or "brand awareness" are a signal to probe further.

  1. Which two treatments would you promote first for us, and why those?
  2. How will you know a lead became a booked consultation, and a consultation became a treatment?
  3. Who reviews ad copy and landing pages for claims, and how do before-and-after photos get approved?
  4. Where will tracking pixels sit, and how do you keep booking and intake pages HIPAA-safe?
  5. Will you sign a Business Associate Agreement?
  6. Whose name are the ad accounts, analytics and Google Business Profile in?
  7. What will you have done by day 30, day 60 and day 90?
  8. What would make you recommend we spend less, or pause a channel?

The last question is revealing. An agency that can name the conditions for spending less is more likely to manage your budget carefully.

Compliance check

How do I check an agency's compliance habits?

Ask how the agency handles patient data, advertising claims and reviews, then check the answers against the public rules. You are looking for an agency that knows the rules exist, builds a review step into its work, and tells you to confirm details with your counsel rather than improvising.

  • Tracking and patient data: HHS's Office for Civil Rights has issued guidance on online tracking technologies, and tracking on patient portals and other authenticated pages is still covered. A careful agency avoids sending identifiable information from booking and intake flows to ad platforms and signs a BAA. See the HHS guidance on online tracking.
  • Reviews and testimonials: the FTC's rule banning fake reviews and testimonials took effect in October 2024, and its Endorsement Guides were revised in 2023. Ask how the agency requests reviews and whether it ever writes or incentivizes them.
  • Treatment claims: the FTC expects health claims to be truthful and backed by competent and reliable scientific evidence, and state boards may set their own advertising rules. Ask who signs off claims before they go live, and confirm specifics with your counsel.

Due diligence

How do I verify what an agency tells me?

Verify with references, a sample report and a short trial of communication. Claims in a pitch are easy to make; a reference call with a current client and a real, anonymized monthly report show how the agency actually works when campaigns are live.

Ask for a reference from a med spa or aesthetic practice of similar size and ask the reference three things: how quickly the agency responds, whether reports match what they see in their own scheduling system, and what happened the last time results dipped. Then ask the agency for an anonymized sample report. Check that it shows booked consultations, not only clicks and impressions.

Finally, look at the agency's own marketing. If it cannot keep its website accurate, its claims modest and its contact details consistent, it will struggle to do that for you. If you see warning signs, our list of med spa marketing agency red flags helps you decide whether to walk away.

Decide and start

How should I make the final decision and start well?

Pick the agency with the highest weighted score whose fee fits your budget, then agree the first 90 days in writing before signing. A good start depends more on shared goals and clean tracking than on how fast campaigns go live.

In the agreement, list the goals for each month, the reports you will receive, the accounts you will own and the handover terms if you part ways. Plan a kickoff that covers your treatment priorities, provider availability, approved claims and photo policy, and how leads reach your front desk.

FAQ

Common questions

What is the most important thing when choosing a med spa marketing agency?

Measurement. An agency should report booked consultations and treatment revenue by source, reconciled with your scheduling system. Without that, you cannot tell which spend works or what to cut. Knowledge of aesthetic treatments and a clear compliance review process for claims, photos and reviews come close behind.

Should a med spa hire an agency that only works with med spas?

Not necessarily. What matters is real experience with consultation-led aesthetic treatments and healthcare rules, such as patient data, claims and reviews. A healthcare-focused agency with med spa experience can be a good fit. Judge by the plan it proposes for your treatments, not by its label.

How many agencies should a med spa compare before choosing?

Two or three is usually enough. Give each the same one-page brief, score them on the same weighted criteria, speak to a reference for each, and compare fees only after scoring. Comparing more agencies tends to add pitches, not clarity.

Should a med spa marketing agency sign a BAA?

If your med spa is a HIPAA covered entity and the agency will handle protected health information, such as lead details tied to treatments or call recordings, HIPAA requires a Business Associate Agreement. Not every med spa is a covered entity, so confirm your status with counsel. Ichelon Consulting US signs a BAA with every US client.

How long should a med spa give a new agency before judging it?

Allow about 90 days for paid campaigns to settle and tracking to prove itself, and longer for SEO. Agree monthly goals at the start, so you can judge progress each month rather than waiting for a single verdict at the end of the quarter.

A note on this guide: it explains marketing practice, not legal advice. Rules on privacy, advertising and insurance change and vary by state, so confirm anything compliance-related with your own counsel.
