What are Google Local Services Ads?
Google Local Services Ads (LSAs) are a Google ad format where verified local businesses pay per lead, usually a call or message, rather than per click. They appear above other results for some local searches and show a verification badge, rating and hours. Healthcare categories are available in some US markets, and eligibility depends on the category and location.
- Charged per lead (calls, messages, bookings), not per click.
- Businesses pass Google's verification, which can include license and background checks.
- Ranking leans on proximity, reviews, responsiveness and business hours.
- Healthcare availability varies by category and city; check eligibility in the sign-up flow.
- Every practice welcome — retainers from $499/mo, Goals-Driven engagements, Performance-Linked Payout Models available.

“They were able to get all my ideas and work with me over a period of three to four months and create this amazing website. It's super customized, very modern, and it incorporates all the elements that I had wanted — the patient portal, nice pictures, a very interactive website, patient reviews. I would highly recommend their company to anyone who wants to make an excellent website.”
How Local Services Ads work
LSAs run from a separate interface from standard Google Ads search campaigns. The business creates a profile, chooses its service categories and areas, sets a weekly budget, and goes through Google's screening and verification. For licensed professions this can include license verification, insurance checks and background checks, depending on the category and state. Verified businesses show a badge in the ad.
Instead of keywords and bids per click, LSAs match the business to searches in its chosen categories and areas. Google charges when a potential customer contacts the business through the ad. Advertisers can dispute leads that do not fit, such as spam or wrong services, and Google may credit them under its rules. Google has said ranking considers factors such as proximity to the searcher, review score and count, responsiveness to calls and messages, business hours, and whether there have been serious complaints.
Google has expanded healthcare categories over time, but coverage is uneven. Some specialties are available in some cities only.
How it applies to practice marketing
- Answer every call: missed calls hurt responsiveness and waste paid leads. Staff the phones during the hours you set.
- Keep hours honest: showing as open when no one picks up hurts both patients and ranking.
- Grow reviews: LSA reviews and your Google Business Profile reputation both matter.
- Dispute and track: review leads weekly, dispute invalid ones, and record which leads become booked appointments.
- Watch PHI: call recordings and messages contain patient information, so set up recording and storage with HIPAA in mind.

Common mistakes and limits
LSAs give less control than search campaigns. You cannot choose keywords, write ad copy or target specific procedures in detail, so high-value services may still need standard search ads. Cost per lead varies widely by specialty and city and changes over time, so judge results by cost per booked appointment, not by lead volume.
A common mistake is treating LSA leads as booked patients. Many are price questions or out-of-area calls. Another is letting verification lapse when a license or insurance document changes. Eligibility, badges and dispute rules are set by Google and change often, so check the current Local Services Ads help pages.

Common questions
Are Google Local Services Ads available for medical practices?
For some categories in some US markets. Google has opened certain healthcare categories to Local Services Ads, but availability depends on the specialty and the city. Practices can check eligibility in the Local Services Ads sign-up flow.
How do practices pay for Local Services Ads?
Practices pay per lead, such as a phone call or message from a potential patient, not per click. They set a weekly budget, and they can dispute leads that do not fit, such as spam or wrong services, under Google's crediting rules.
How do Local Services Ads rank businesses?
Google has said ranking considers factors such as distance to the searcher, review rating and number, how responsive the business is to calls and messages, business hours, and serious complaints. Answering calls reliably matters a great deal.
Are Local Services Ads better than Google search ads for practices?
They serve different jobs. LSAs are simple and pay per lead but offer little control over keywords and copy. Search ads offer targeting by procedure and landing page. Many practices test both and compare cost per booked appointment.
A note on this guide: it explains marketing practice, not legal advice. Rules on privacy, advertising and insurance change and vary by state, so confirm anything compliance-related with your own counsel.
