What is cost per lead?
Cost per lead (CPL) is marketing spend divided by the number of leads that spend produced in the same period. A lead is an enquiry from a potential patient, such as a phone call, form submission, chat or booking request. CPL is the most common early metric for paid campaigns, but it measures enquiries, not patients, so it needs booking and attendance data beside it.
- Formula: spend ÷ leads for one channel and period.
- Define a lead clearly: unique, real enquiries, excluding spam, duplicates and existing patients.
- A qualified lead definition helps compare channels fairly.
- Read CPL with booking rate and cost per booked appointment.
- Every practice welcome — retainers from $499/mo, Goals-Driven engagements, Performance-Linked Payout Models available.

“They were able to get all my ideas and work with me over a period of three to four months and create this amazing website. It's super customized, very modern, and it incorporates all the elements that I had wanted — the patient portal, nice pictures, a very interactive website, patient reviews. I would highly recommend their company to anyone who wants to make an excellent website.”
How cost per lead is calculated
Take the spend on a channel for a period and divide it by the leads the channel produced. Spend can be media only, or include agency fees and tools; state which. The bigger question is what counts as a lead. Raw counts include spam form fills, short or misdialed calls, duplicate enquiries from the same person, existing patients calling about appointments and job seekers. Including these lowers CPL without adding patients.
Many practices therefore track a qualified lead: a unique, real potential patient enquiring about a service the practice offers, in its area. Cost per qualified lead (sometimes called CPQL) gives a fairer comparison across channels. Call tracking helps by filtering short calls and tagging outcomes.
CPL is the top of a chain: cost per lead, then cost per booked appointment, then patient acquisition cost.
How practices use it
- Early campaign signal: CPL moves quickly, so it helps spot broken campaigns or landing pages.
- Channel comparison: compare CPL and booking rate together. A cheaper lead that rarely books is not a bargain.
- Budget planning: estimated CPL times target leads gives a rough budget, refined later with booking data.
- Specialty context: CPL varies widely by specialty, city and service value.

Common mistakes and limits
The biggest mistake is optimizing for CPL alone. Ad platforms will find cheap leads if told to, often from people with low intent or outside the service area. Bidding on form fills without quality feedback can make CPL look great while bookings fall.
Other mistakes include counting existing-patient calls as leads, comparing CPL across specialties, and mixing periods. Published CPL benchmarks often use different definitions, so compare your own numbers over time first. Pair CPL with booking rate, show rate and patient acquisition cost to see the full picture.
The fix is a feedback loop. Tag each lead with its outcome in the CRM, review a sample of calls and forms every week, and send qualified or booked outcomes back to ad platforms where that can be done without sharing health details. Over time, the campaigns learn to find people who book, not just people who fill in forms.

Common questions
How do you calculate cost per lead?
Divide the marketing spend for a channel and period by the number of leads it produced in that period. A lead is an enquiry from a potential patient, such as a call, form or message. Decide whether spend includes fees and tools, and use the same definition consistently.
What counts as a lead for a medical practice?
A lead is a unique enquiry from a potential new patient about a service the practice offers. Spam, duplicates, existing-patient calls, misdials and job enquiries should be excluded. Many practices track qualified leads separately for fairer comparisons.
What is a good cost per lead for a practice?
It depends on specialty, city, service value and how leads are defined. A high-value elective procedure can justify a far higher cost per lead than routine care. Practices should judge cost per lead alongside booking rate and patient acquisition cost.
Why can a low cost per lead be misleading?
Because cheap leads may not book. If a channel produces many low-intent enquiries, its cost per lead looks good while cost per booked appointment and patient acquisition cost rise. Always check what happens after the lead.
