What is show rate?
Show rate is the percentage of booked appointments where the patient actually attends. It equals attended appointments divided by booked appointments for a period, times 100. Its opposite is the no-show rate. For marketing, show rate is the bridge between booked appointments and acquired patients, so a weak show rate quietly raises the real cost of every new patient.
- Formula: attended ÷ booked × 100, for a defined period.
- Decide how to treat late cancellations and reschedules, and keep it consistent.
- Track new-patient show rate by marketing source, not only overall.
- Reminders, deposits and clear expectations usually help; check TCPA and state rules for each.
- Every practice welcome — retainers from $499/mo, Goals-Driven engagements, Performance-Linked Payout Models available.

“They were able to get all my ideas and work with me over a period of three to four months and create this amazing website. It's super customized, very modern, and it incorporates all the elements that I had wanted — the patient portal, nice pictures, a very interactive website, patient reviews. I would highly recommend their company to anyone who wants to make an excellent website.”
How show rate is calculated
Take all appointments booked for a period, say a month, and count how many the patient attended. Divide attended by booked and multiply by 100. Practices need a clear rule for edge cases. A same-day cancellation is often grouped with no-shows. A rescheduled appointment is often removed from the first count and counted again when it happens. Whatever rule you choose, apply it the same way every month.
Show rate is most useful when tracked separately for new and existing patients. Existing patients usually behave differently from first-time patients, and marketing mainly affects new ones. Within new patients, splitting by source shows whether some channels bring people who book but rarely attend.
Show rate is a link in the funnel. Leads become bookings at the booking rate, bookings become visits at the show rate, and visits become treatment at the case acceptance or conversion rate.
How practices use and improve it
- Source quality: if one channel has a low new-patient show rate, review its targeting, offer and landing page.
- Speed of booking: long gaps between enquiry and appointment date often lower attendance.
- Reminders: confirmations and reminders by text, email or call are standard. Texts need TCPA-appropriate consent.
- Set expectations: tell new patients what to bring, how long the visit takes, and the cost or deposit policy where relevant.
- Deposits: common for some elective services; state consumer rules and clear policies apply.
For tracking, see the KPI dashboard guide and marketing ROI guide. Reminder texting rules are covered in the TCPA SMS guide.
Common mistakes and limits
A common mistake is judging marketing only on bookings while the front desk sees empty slots. Another is blaming a channel for low attendance when the real cause is a long wait for the first appointment. Compare show rate against the lead time to the appointment before moving budget.
Show rate benchmarks vary by specialty, payer mix and patient population, and published figures are often from small or unclear samples. Use your own history as the baseline. Also note that some patients who miss a visit rebook later. Tracking rebooked no-shows gives a fuller picture than one month's show rate. It also helps to review a sample of no-shows each month: was the reminder sent, was the appointment far out, and did the person understand the cost? Patterns there point to fixes faster than the headline rate does.

Common questions
How do you calculate show rate?
Divide the number of attended appointments by the number of booked appointments for a period, then multiply by 100. Decide in advance how to count late cancellations and reschedules, and apply the same rule every month so the trend is meaningful.
Why does show rate matter for marketing?
Because marketing pays for bookings that only turn into patients when people attend. A low show rate raises the true cost of acquiring each patient. Tracking it by source shows whether some channels bring bookings that rarely turn into visits.
How can a practice improve its show rate?
Common steps include booking new patients sooner, sending confirmations and reminders, explaining what to expect and any costs, and using deposit or cancellation policies for some elective services. Reminder texts need appropriate TCPA consent.
What is the difference between show rate and no-show rate?
They are opposites. Show rate is the share of booked appointments attended. No-show rate is the share missed without notice. If late cancellations are grouped with no-shows, the two rates still add up to 100 percent for the period.
