What is the local pack?
The local pack is the block of map results Google shows for searches with local intent, typically a map plus a short list of nearby businesses, often three. Each listing shows the Google Business Profile name, rating, reviews, hours and buttons to call, get directions or visit the website. For practices, the local pack is often where patients choose whom to call.
- Appears for local-intent searches, such as specialty + city or "near me".
- Draws from Google Business Profiles, not regular web page rankings.
- Google names three ranking factors: relevance, distance and prominence.
- Ads can appear in or above it; organic placement cannot be bought.
- Every practice welcome — retainers from $499/mo, Goals-Driven engagements, Performance-Linked Payout Models available.

“They were able to get all my ideas and work with me over a period of three to four months and create this amazing website. It's super customized, very modern, and it incorporates all the elements that I had wanted — the patient portal, nice pictures, a very interactive website, patient reviews. I would highly recommend their company to anyone who wants to make an excellent website.”
What the local pack is
When Google detects that a searcher wants a local business, it shows a map with pins and a short list of businesses. Clicking through opens the fuller Google Maps list. The listings come from Google Business Profiles, so a practice needs a verified, complete profile to appear.
Google says local results are based mainly on three factors. Relevance is how well the profile matches the search, influenced by categories, services and descriptions. Distance is how far the business is from the searcher or the location in the query. Prominence is how well known the business is, drawing on reviews, links, mentions and its overall web presence. Google also says that paying for ads does not raise local ranking.
Because distance matters, results change from street to street. Two people searching the same term a few miles apart can see different practices.
How practices improve local pack visibility
- Complete the profile: correct primary category, services, hours, photos, booking link and providers.
- Steady reviews: ask every patient, respond carefully under HIPAA, and keep reviews coming over time.
- Strong location page: link the profile to a detailed page for that location.
- Consistent listings: name, address and phone match across directories.
- Local prominence: community involvement, local news and association mentions.

Common mistakes and limits
A practice cannot control distance, so it will not appear in the pack everywhere in a city. Rank tracking from a single point gives a misleading picture; grid-based checks across the service area show more. Some practices try to fix distance with virtual offices or fake addresses, which breaks Google's guidelines and can get profiles suspended.
Other mistakes include stuffing keywords into the business name, choosing categories that do not match, and ignoring the profile after setup. Google Business Profile features and policies change often, so check the current guidelines and treat the profile as an ongoing channel.
Measure what matters. Local pack position is a means, not the goal. Track calls, direction requests and website clicks from the profile in its performance report, and link them to booked appointments where your call tracking allows it.

Common questions
How does Google decide which practices appear in the local pack?
Google says local results depend mainly on relevance, distance and prominence. Relevance is how well the profile matches the search, distance is how close the practice is to the searcher, and prominence reflects reviews, links, mentions and overall reputation.
Can a practice pay to appear in the local pack?
Not in the organic listings. Google says paying for ads does not affect local ranking. Practices can run ads that appear in or near local results, such as Local Services Ads or location-based search ads, but those are labeled as ads.
Why does my practice appear in the local pack in some places but not others?
Because distance is a major factor. Results change based on where the searcher is, so a practice usually ranks well near its office and less well farther away. Checking rankings across a grid of points shows the real pattern.
How many businesses show in the Google local pack?
Usually three, alongside a map, though Google tests and changes layouts. Searchers can click to see more results in the full Google Maps list. Ads may also appear in or above the pack.
