Does a hospital or clinic website need an llms.txt file?
A hospital or clinic website does not need an llms.txt file to appear in ChatGPT, Gemini or Google's AI Overviews, as of October 2026. It is a proposed standard, not a requirement, and Google has said its AI features rely on normal search indexing. Adding one is quick and harmless. But crawl access, clear treatment pages, structured data and consistent clinic details matter far more.
What is an llms.txt file?
An llms.txt file is a plain-text, Markdown-formatted file placed at the root of a website, for example yourhospital.in/llms.txt. It gives large language models a short summary of the site and a curated list of its most useful pages. It was proposed publicly in 2024 as a voluntary convention.
It is often confused with robots.txt, but the two do different jobs. Robots.txt tells crawlers what they may and may not fetch. A sitemap lists every URL you want indexed. An llms.txt file is closer to a reading guide: "here is who we are, and here are the pages worth reading". Nothing forces any AI system to read it.
| File | Purpose | Who acts on it | Priority for a clinic |
|---|---|---|---|
| robots.txt | Allows or blocks crawling | Search and AI crawlers that respect it | High: check it is not blocking search or AI crawlers by mistake |
| XML sitemap | Lists URLs for indexing | Google, Bing and others | High |
| Schema markup | Labels the clinic, doctors, services and FAQs | Google and other systems that parse it | High |
| llms.txt | Summarises the site for language models | Only tools that choose to read it | Low: nice to have |
Can ChatGPT do SEO, and does it read llms.txt?
ChatGPT can help draft and research SEO work, but it does not rank your site. When ChatGPT answers with web results, it relies on its search crawler and search index, not on llms.txt. As of October 2026 we are not aware of any official statement from OpenAI, Google or Perplexity that their answers depend on the file.
What these tools do depend on is access and clarity. If your robots.txt or firewall blocks their crawlers, your pages cannot be fetched. Common user agents to check include Googlebot, which covers Search and AI Overviews; OAI-SearchBot, used for ChatGPT search; and PerplexityBot. Google-Extended is a separate control that affects use of your content for Gemini models, not your Search ranking. Hospitals sometimes block all bots after a security scare and never notice. What actually gets a clinic cited by AI answers?
AI tools cite pages that answer a question directly, come from a source they can verify, and agree with what the rest of the web says about you. For a clinic, that means focused treatment pages, named doctors with qualifications, consistent clinic details, and genuine reviews, all reachable by crawlers.
- One page per treatment or department, opening with a plain two-sentence answer.
- Doctor profiles with qualifications and registration numbers, which the NMC guidelines of 6 October 2026 expect for doctors' educational content.
- Schema markup for MedicalClinic or Hospital, Physician and FAQPage, matching what is visible on the page.
- Consistent name, address and phone across your site, Google Business Profile and listings.
- FAQs written from real questions your front desk hears.
- No restricted claims. Testimonials and "best" language are not allowed in promotion and can make content less trustworthy to quote anyway.
What are the four types of SEO, and where does llms.txt fit?
The four types usually listed are on-page, off-page, technical and local SEO. An llms.txt file is a small technical item. It sits alongside robots.txt and sitemaps, but it carries far less weight than page speed, indexing, schema or your Google Business Profile in deciding whether a clinic is found.
Answer engine optimisation, or AEO, now runs across all four. On-page work writes answer-first content. Technical work keeps crawlers in. Off-page work builds the mentions and reviews AI tools use to check facts. Local work makes sure the clinic shows up for "near me" questions. Read more in the four types of SEO for clinics.
Should we add one anyway, and what goes in it?
If your web team can add it in an hour, go ahead. It costs little and may help tools that do read it. Keep it short, factual and in line with NMC and ASCI rules, list your key pages, and update it when departments or doctors change. Do not treat it as an AI-visibility strategy.
- A one-line description of the hospital or clinic, its city and its specialities.
- Links to the main department or treatment pages.
- Links to doctor profile pages and the contact page.
- No patient data, no offers, no outcome claims.
