How do I choose a dental marketing agency?

Choose a dental marketing agency by how well it can prove booked new patients, not by how polished its pitch is. Shortlist agencies that understand dental services and insurance mix, report bookings rather than clicks, sign a Business Associate Agreement, keep accounts in your name and name who runs your work. Score each one on the same criteria.

Guide for US practice owners · Published October 15, 2026

TL;DR
  • Write your goals first: which services, which locations and how many new patients you want each month.
  • Score every agency on the same criteria, so the comparison is about evidence rather than presentation.
  • Insist on booking-level reporting: calls and forms matched to appointments, not only clicks and impressions.
  • Check HIPAA readiness and ownership: a signed BAA, HIPAA-safe tracking, and every account in the practice's name.
  • Every practice welcome — retainers from $499/mo, Goals-Driven engagements, Performance-Linked Payout Models available.

“They were able to get all my ideas and work with me over a period of three to four months and create this amazing website. It's super customized, very modern, and it incorporates all the elements that I had wanted — the patient portal, nice pictures, a very interactive website, patient reviews. I would highly recommend their company to anyone who wants to make an excellent website.”

Before you shop

What should a practice decide before talking to agencies?

Decide what you want more of before any sales call. Name the services you want to grow, the locations involved, your capacity for new patients and the budget range you can commit. Without this, every agency will propose its own favorite channel, and you will be comparing pitches rather than plans.

  • Priority services: for example implants, clear aligners, emergency visits or family hygiene patients.
  • Patient mix: PPO, fee-for-service, membership plan or a blend. This changes which searches and channels matter.
  • Capacity: open chair time and how many new patients the front desk can schedule.
  • Budget range: the management fee and ad spend you can sustain for at least a few months.
  • Internal owner: the person at the practice who will approve work and review reports.

Write this on one page and send it to every agency you talk to. Good agencies will respond to it directly.

Scorecard

How should you score dental marketing agencies side by side?

Use one scorecard for every agency. Rate each criterion from weak to strong, based on evidence they show you rather than claims. The table below lists the criteria that matter most for a dental practice, what a strong answer looks like, and the question that tests it.

CriterionWhat a strong answer looks likeQuestion to ask
Dental experienceUnderstands case acceptance, insurance mix, recall and high-value services"How would you market implants differently from hygiene visits?"
ReportingShows calls, forms and booked appointments by channel and service"Can I see a sample monthly report with booking data?"
HIPAA readinessSigns a BAA and uses HIPAA-safe tracking on booking and intake pages"Will you sign a BAA, and how do you track conversions?"
Account ownershipAds, analytics, Google Business Profile and website sit in your name"Who owns the accounts and the website if we part ways?"
Team and accessNames the account lead and specialists; regular calls"Who will work on my account each week?"
Goals and accountabilityAgrees monthly goals in writing and reviews them openly"What happens if we miss the goal two months running?"
Contract termsClear scope, reasonable notice period, no hidden markups"What is the minimum term and how do we cancel?"

Total the scores only after every agency has been through the same questions.

First call

What should you listen for on the first sales call?

Listen for questions about your practice. A strong agency asks about your services, chair time, front desk and current numbers before it proposes anything. A weak one leads with packages and promises. The more specific their questions, the more likely the plan will fit your practice.

Good signs

  • They ask how calls are answered and how fast forms get a reply.
  • They want to see your Google Business Profile and current ad accounts.
  • They explain what they would not do yet, and why.
  • They give a realistic view of timing for each channel.

Warning signs

  • Guaranteed rankings or a guaranteed number of patients.
  • Reluctance to show a sample report or name who will do the work.
  • A one-size package with no questions about your goals.

Compliance

How do you check an agency's HIPAA and advertising readiness?

Ask three things: will the agency sign a Business Associate Agreement, how does it track conversions on booking and intake pages, and how does it handle reviews and testimonials. Clear, specific answers show the agency has done this before. Vague answers mean your practice carries the risk.

  • BAA: an agency that handles patient information, such as call recordings or form submissions, should sign one. Our glossary entry on the business associate agreement explains what it covers.
  • Tracking: HHS Office for Civil Rights guidance on online tracking technologies still applies to patient portals and other authenticated pages. Third-party pixels on booking and intake flows are a known risk, so ask how the agency avoids them.
  • Reviews: the FTC rule banning fake reviews and testimonials took effect in October 2024. Among other things, it bars fake reviews, payment for reviews that must be positive, and suppressing negative reviews through threats or false accusations. Ask how the agency requests and answers reviews.
  • State rules: state dental boards and state laws set advertising rules, such as how specialties and credentials may be described. Rules differ by state.

This is a high-level summary, not legal advice. Confirm the details with your counsel.

Fit

Does the agency's model fit the size of your practice?

Match the agency's model to your practice structure. A single-location practice needs a close partner who will handle the Google profile, website and local search with little internal support. A multi-location group or DSO needs location-level reporting, brand controls and campaigns that scale. Ask each agency which of these it is built for.

Questions that reveal fit

  • Single location: "Who will update our Google Business Profile, answer reviews and write service pages, and how often?"
  • Several locations: "Can you report bookings by office, and how do you stop our locations competing with each other in search?"
  • New practice: "What would you do before opening day, and what would you leave until we have reviews?"
  • Established practice with an in-house coordinator: "Which tasks would you take on, and which stay with our coordinator?"

If you are weighing whether to hire at all, our comparison of a dental marketing agency versus an in-house coordinator sets out the trade-offs.

An agency that serves every practice the same way will rarely fit yours well. The best sign of fit is a proposal that reflects the one-page brief you sent, rather than a standard package with your logo added.

References

How do you verify an agency's claims before signing?

Ask for evidence you can check. Request a sample report with names removed, a walkthrough of a live dashboard, and a call with a current dental client. Look at the agency's own website and Google presence too. An agency that cannot market itself well will struggle to market you.

  1. Ask for a redacted monthly report from a dental account and check whether it shows bookings.
  2. Ask for a reference you can call, and ask that practice what changed after onboarding.
  3. Ask the agency to audit one thing for free, such as your Google Business Profile, and judge the quality of the findings.
  4. Read the full contract before the final call, not after.

What to ask a reference practice

  • How quickly did the agency fix tracking and the Google profile after onboarding?
  • Do the Monday or monthly reports show booked appointments, and do they match the practice's own schedule?
  • When something went wrong, how fast did the account lead respond, and what changed?
  • Would they sign the same contract again, and what would they negotiate differently?

A short reference call often tells you more than the sales meetings do. If an agency cannot arrange one, ask why.

FAQ

Common questions

What is the most important factor when choosing a dental marketing agency?

Whether the agency can show booked new patients by channel and service, not just clicks or leads. Booking-level reporting proves the work is paying off and lets you compare agencies fairly. Ask every agency for a sample report before deciding, and check that it tracks appointments.

Should a dental marketing agency sign a BAA?

Yes, if the agency will handle patient information, such as call recordings, form submissions or appointment data. A Business Associate Agreement sets out how that information is protected under HIPAA. Ask for it before work starts, and confirm your obligations with your own counsel.

Is a dental-only agency better than a general marketing agency?

Not automatically. What matters is whether the team understands dental services, insurance mix, case acceptance, recall and healthcare rules on privacy and advertising. Test this on the first call by asking how they would market a high-value service differently from routine hygiene visits.

How many dental marketing agencies should I talk to before choosing?

A shortlist of two to four agencies is usually enough. Send each one the same written brief, ask the same questions, and score them on the same criteria. More calls rarely help once you have a clear scorecard and comparable proposals.

Can a dental marketing agency guarantee new patients or rankings?

No agency controls Google's rankings or patient decisions, so guarantees should be treated with caution. A credible agency agrees measurable monthly goals, reports progress openly and explains what it will change if goals are missed. Performance-linked payment can be agreed, but only with clear definitions in writing.

A note on this guide: it explains marketing practice, not legal advice. Rules on privacy, advertising and insurance change and vary by state, so confirm anything compliance-related with your own counsel.
