Ichelon Consulting US · Cost answer

How much does chiropractic marketing cost per month?

Chiropractic marketing costs whatever the scope requires: a management fee for the work, plus ad spend paid to platforms, plus any one-time setup. The monthly total moves with how many services you buy, how competitive your city is, and how many offices you run. There is no single market rate, so judge any quote by what it covers and by your cost per new patient.

TL;DR
  • Three parts: agency or staff fees, media spend paid to Google or Meta, and one-time setup such as a website rebuild.
  • Ask for ad spend to be shown separately from management fees so you can see what reaches the platforms.
  • Judge the spend by cost per new patient, not by clicks or leads, and compare it with the value of a typical care plan.
  • Ichelon Consulting US retainers start from $499/month, custom-scoped, with a BAA and a report every Monday.
  • Every practice welcome — retainers from $499/mo, Goals-Driven engagements, Performance-Linked Payout Models available.

“They were able to get all my ideas and work with me over a period of three to four months and create this amazing website. It's super customized, very modern, and it incorporates all the elements that I had wanted — the patient portal, nice pictures, a very interactive website, patient reviews. I would highly recommend their company to anyone who wants to make an excellent website.”

Cost drivers

What decides how much a chiropractic practice spends on marketing each month?

Scope decides most of the bill. A practice that only needs its Google Business Profile, reviews and a few local pages pays far less than one running paid search, social ads, video and a new website at once. After scope come market competition, the number of locations, and how much creative and content work is needed each month.

Cost driverWhy it moves the priceWhat to ask the provider
Services in scopeEach channel (local SEO, paid search, social, email, website) adds hours every monthWhich channels are included, and which are add-ons?
Ad spendPaid to Google or Meta directly; it scales with your goals and your marketIs media spend billed separately, and who owns the ad account?
Market competitionDense metro areas usually mean more practices bidding on the same searchesHow many practices compete for my core searches within driving distance?
Number of locationsEach office needs its own profile, location page, reviews and trackingIs pricing per location or for the group?
Content and creative volumeVideos, blog posts and ad creative take production timeHow many pieces per month, and who records the video?
Tracking and reportingCall tracking and new-patient attribution need setup and upkeepWill you report new patients, or only clicks and calls?

Your patient mix matters too. A practice built on cash-pay wellness plans, one focused on personal injury, and one with many Medicare patients value a new patient very differently. CMS limits Medicare coverage of chiropractic care to manual manipulation of the spine to correct a subluxation, so practices with a large Medicare share often market non-covered services with clear self-pay pricing. That changes which searches are worth paying for.

Pricing models

How do agencies price chiropractic marketing?

Most agencies use one of five models: a flat monthly retainer, a retainer plus a percentage of ad spend, project fees for one-time work, pay-per-lead, or a performance-linked arrangement. Each shifts risk differently. Ask which model applies to each service, because many quotes mix two or three of them.

The common models

  • Flat retainer: a fixed fee for an agreed scope. Easy to budget; make sure the scope is written down.
  • Retainer plus a share of ad spend: the fee rises as your budget rises. Ask where the percentage starts and whether it is capped.
  • Project fees: used for websites, brand work or a one-time audit. Confirm who owns the site and its content afterwards.
  • Pay-per-lead: you pay for each form or call. Define what counts as a lead, and check duplicates and wrong numbers are excluded.
  • Performance-linked: part of the fee depends on agreed outcomes. Agree the measurement method before the work starts.

A caution on per-patient fees

Some state rules restrict paying a third party for each patient referred to a licensed provider. Before agreeing to any fee tied to booked or seen patients, check your state chiropractic board's rules with your counsel. Fees tied to agreed marketing goals are usually structured differently from payment per referred patient.

Budget split

Where should a chiropractic marketing budget go first?

Start where patients already look for you: your Google Business Profile, reviews, and a website that loads fast and books easily. Add paid search once the basics convert, then social and video to build demand. Spending on ads before the front desk can answer calls and book quickly wastes most of the budget.

  1. Foundations: an accurate Google Business Profile, a steady flow of genuine reviews, and one page per service and per office.
  2. Conversion: online booking, click-to-call on mobile, clear new-patient information and posted self-pay pricing where you offer it.
  3. Paid search: tightly targeted campaigns for high-intent searches near each office, with call and booking tracking.
  4. Social and video: short educational videos and posts that show the doctor, the office and what a first visit is like.
  5. Retention: recall and reactivation messages to past patients, sent only with documented consent for texts.

A new practice usually spends more of its budget on foundations in the first months. An established practice often shifts spend toward paid search and reactivation. A group with several offices needs consistent profiles and tracking before any channel can be compared fairly. The chiropractic practice marketing guide covers each channel in more depth.

Return

How do you tell whether chiropractic marketing spend is working?

Divide total monthly marketing cost, including fees and ad spend, by the number of new patients who actually came in. That cost per new patient is the number that matters. Compare it with the revenue from a typical care plan or membership, after your clinical costs, to see whether the spend pays back.

  • Track calls and bookings by source. Use call tracking on ads and your profile, and ask every new patient how they found you.
  • Separate new patients from returning ones. Reactivated patients are valuable, but report them separately.
  • Watch the front desk. Missed calls and slow callbacks raise the cost of every patient, whatever the channel.
  • Give channels time. Paid search shows results quickly; local SEO and reviews build over months.

Our cost per new patient calculator does the arithmetic once you have the figures. If your agency reports only impressions and clicks, ask for new patients by source every month.

FAQ

Common questions

How much should a chiropractor spend on marketing per month?

There is no single right figure. Set the budget from your goal: how many new patients you need each month, what each new patient is worth over a care plan, and what your market costs. Start with foundations such as your Google profile and reviews, then add paid channels once your front desk converts calls into visits reliably.

Is ad spend included in a chiropractic marketing agency's fee?

Usually not. Most agencies charge a management fee and bill ad spend separately, or ask you to pay Google and Meta directly. Ask any agency to show both figures on its quote, confirm who owns the ad accounts, and request monthly reports that show spend, calls, bookings and new patients side by side.

Is pay-per-lead a good pricing model for chiropractors?

It can be, if a lead is defined precisely and duplicates, spam and wrong numbers are excluded. Be careful with any fee tied to each patient seen, because some state rules restrict paying for patient referrals. Check your state chiropractic board's rules with your counsel before signing a per-patient arrangement.

How long before chiropractic marketing pays for itself?

Paid search can bring calls within weeks of launch once tracking and landing pages are ready. Local SEO, reviews and video usually take months to build. Measure cost per new patient monthly, compare it with care-plan value after clinical costs, and judge each channel over a fair period rather than a single month.

Does Medicare coverage affect chiropractic marketing?

It can. CMS limits Medicare coverage of chiropractic care to manual manipulation of the spine to correct a subluxation, so other services are typically self-pay for Medicare patients. Practices with many Medicare patients often promote non-covered services with clear pricing, which changes the searches and messages worth paying for.

A note on this guide: it explains marketing practice, not legal advice. Rules on privacy, advertising and insurance change and vary by state, so confirm anything compliance-related with your own counsel.
