What is patient lifetime value?

Patient lifetime value (LTV) is the total revenue, or better the gross profit, that a practice can expect from one patient across the entire relationship. It includes repeat visits, additional services and, in some models, referrals. Practices compare LTV with patient acquisition cost to decide how much they can afford to spend to win a new patient.

Guide for US practice owners · Published October 11, 2026

TL;DR
  • Basic formula: average revenue per visit × visits per year × years retained.
  • Using gross margin instead of revenue gives a more honest number.
  • Compare with patient acquisition cost; the gap funds growth.
  • Varies sharply by specialty, service and payer mix; calculate your own.
  • Every practice welcome — retainers from $499/mo, Goals-Driven engagements, Performance-Linked Payout Models available.

“They were able to get all my ideas and work with me over a period of three to four months and create this amazing website. It's super customized, very modern, and it incorporates all the elements that I had wanted — the patient portal, nice pictures, a very interactive website, patient reviews. I would highly recommend their company to anyone who wants to make an excellent website.”

How patient lifetime value is calculated

A simple version multiplies three figures from the practice's own records: average revenue per visit, average visits per patient per year, and average number of years a patient stays. A better version multiplies by gross margin, the share of revenue left after direct costs such as clinical staff time and supplies, so the result reflects profit rather than billing.

Many practices calculate LTV by segment. A patient who comes for a one-time procedure has a very different value from one in ongoing care or a membership. Splitting by service line, payer type or acquisition channel makes the number more useful.

Some models add a referral component, crediting a patient with a share of the value of people they refer. That is harder to measure, so keep it separate from the core figure.

In marketing

How practices use it

  • Set acquisition limits: knowing LTV tells a practice the most it can pay to acquire a patient and still profit.
  • Compare channels: some channels bring patients who stay longer or use more services, which can justify a higher acquisition cost.
  • Invest in retention: raising retention or follow-through on treatment plans increases LTV, often more cheaply than new marketing.
  • Plan memberships: recurring models change LTV, so track churn alongside it.

Limits

Common mistakes and limits

A common mistake is using revenue instead of margin. That overstates value and can justify spending that loses money. Another is assuming every patient stays as long as the most loyal ones. Use real retention data, and update it as the practice changes.

Insurance mix matters. Reimbursement rates, collection rates and write-offs can make two patients with the same visit history worth very different amounts. Lifetime value is also a forecast, not a promise. Treat it as a planning range and revisit it at least yearly.

Finally, avoid marketing that pushes services patients do not need to raise lifetime value. Beyond ethical problems, it can breach state board rules and harms reviews and retention. The healthier way to raise lifetime value is better care experience: easy scheduling, clear communication about costs and follow-up that patients actually find useful.

FAQ

Common questions

How do you calculate patient lifetime value?

Multiply average revenue per visit by average visits per patient per year and by the average number of years a patient stays. For a more accurate figure, multiply the result by gross margin so it reflects profit rather than billed revenue.

Why does patient lifetime value matter for marketing?

It tells a practice how much a new patient is worth, which sets the upper limit on what it can spend to acquire one. Comparing lifetime value with patient acquisition cost shows whether marketing is profitable.

Does patient lifetime value differ by specialty?

Yes, a great deal. Specialties with ongoing or recurring care tend to have higher lifetime value than those with one-time visits, and payer mix changes the result further. Practices should calculate their own figure from their own records.

Should referrals be included in patient lifetime value?

They can be, but they are hard to measure. Many practices keep a core lifetime value based on the patient's own visits and track referrals separately, so the main number stays reliable.

A note on this guide: it explains marketing practice, not legal advice. Rules on privacy, advertising and insurance change and vary by state, so confirm anything compliance-related with your own counsel.
