What is llms.txt?
llms.txt is a proposed standard for a Markdown file placed at a website's root, at /llms.txt, that gives large language models a short, curated guide to the site's key content. It was proposed in 2024 as a way to help AI tools find the most useful pages. It is not an official web standard, and major search engines have not said they use it for ranking or AI answers.
- A Markdown file at the site root listing key pages with short descriptions.
- Proposed in 2024; voluntary and not an official standard.
- Different from robots.txt, which controls crawling; llms.txt does not block or allow anything.
- Low effort to add, but no proven effect on Google rankings or AI citations.
- Every practice welcome — retainers from $499/mo, Goals-Driven engagements, Performance-Linked Payout Models available.

“They were able to get all my ideas and work with me over a period of three to four months and create this amazing website. It's super customized, very modern, and it incorporates all the elements that I had wanted — the patient portal, nice pictures, a very interactive website, patient reviews. I would highly recommend their company to anyone who wants to make an excellent website.”
What llms.txt is
The proposal suggests a simple structure: a title with the site or project name, a short summary, then sections of links to important pages, each with a one-line description. Some sites also publish a fuller version with page content included, often named llms-full.txt. The idea is that an AI tool, especially one reading a site on demand, can use the file to find clean, relevant content without crawling navigation menus and scripts.
It differs from robots.txt and sitemaps. Robots.txt tells crawlers what they may access. An XML sitemap lists URLs for search engines to crawl. llms.txt is a human-readable summary intended for language models. It grants no permissions and blocks nothing. To control AI crawlers, sites use robots.txt rules for the specific user agents those companies publish.
Adoption so far has been strongest among software documentation sites, where developers paste docs into AI tools.
How it applies to practice marketing
- Optional, low cost: a practice can add one in under an hour, listing service pages, locations, providers and key policies.
- Keep it accurate: treat it like any public page. Outdated services or providers there send the wrong signal.
- Do not rely on it: crawlable pages, clear answers, structured data and consistent listings matter far more.
- No PHI or private links: it is a public file.

Common mistakes and limits
The main mistake is believing llms.txt is required or will raise visibility in Google or ChatGPT. There is no public evidence that major AI search products use it as a ranking or citation signal, and Google has said that normal SEO practices are what matter for its AI features. Vendors selling llms.txt as a breakthrough are overselling it.
Another mistake is confusing it with AI crawler control. If a practice wants to allow or block specific AI crawlers, that is done in robots.txt, and the choice has trade-offs: blocking may keep content out of some AI answers. Review the decision with whoever manages your website and SEO, and revisit it as the proposal and AI products change.

Common questions
What is llms.txt?
It is a proposed Markdown file placed at a website's root, at /llms.txt, that gives large language models a short, curated list of the site's most important pages with descriptions. It was proposed in 2024 and is voluntary, not an official web standard.
Does llms.txt help a practice rank in Google or ChatGPT?
There is no public evidence that it does. Major search engines have not said they use llms.txt for ranking or AI answers. Crawlable pages, clear answers, structured data and consistent listings matter far more.
Is llms.txt the same as robots.txt?
No. Robots.txt tells crawlers which parts of a site they may access. llms.txt is a summary for language models and does not grant or block access. To control AI crawlers, sites use robots.txt rules for those crawlers' user agents.
Should a medical practice add an llms.txt file?
It is optional. It is quick to create and harmless if accurate, but it should not replace core SEO and content work. If added, list only public pages, keep it current and never include patient information.
