Which marketing KPIs should a fertility clinic track?
A fertility clinic should track the KPIs that link marketing to consultations and treatment starts. The core set is qualified enquiries by source, speed to first response, consultation booking rate, show rate, consult-to-treatment rate, cost per consultation and cost per treatment start. Because treatment often begins months after a first enquiry, measure these by monthly cohort, not just by calendar month.
- Cost per treatment start is the KPI that best shows whether marketing is building the patient base.
- Front-desk KPIs such as answered-call rate and response time often explain weak results better than ad metrics.
- Use monthly cohorts so that slow fertility decisions are credited to the campaign that started them.
- Join three data sources: ad platforms, call and form tracking, and the CRM or practice records.
- Every practice welcome — retainers from $499/mo, Goals-Driven engagements, Performance-Linked Payout Models available.

“They were able to get all my ideas and work with me over a period of three to four months and create this amazing website. It's super customized, very modern, and it incorporates all the elements that I had wanted — the patient portal, nice pictures, a very interactive website, patient reviews. I would highly recommend their company to anyone who wants to make an excellent website.”
What are the core fertility marketing KPIs?
The core KPIs follow one person from first enquiry to treatment start. They measure how many qualified enquiries each source brings, how fast the clinic responds, how many enquiries book and attend a consultation, how many consultations lead to treatment, and what each step costs. The table below defines each one.
| KPI | Definition | How to calculate | Review |
|---|---|---|---|
| Qualified enquiries by source | Calls and forms from people seeking a service you offer, in your area | Count by channel, excluding spam, wrong numbers and existing patients | Weekly |
| Speed to first response | Time from enquiry to first real contact | Median minutes during opening hours | Weekly |
| Answered-call rate | Share of inbound calls answered live | Answered calls divided by total calls | Weekly |
| Consultation booking rate | Share of qualified enquiries that book a first consultation | Consultations booked divided by qualified enquiries | Monthly |
| Show rate | Share of booked consultations that are attended | Attended divided by booked | Monthly |
| Consult-to-treatment rate | Share of attended consultations that start a treatment cycle | Treatment starts divided by attended consultations, by cohort | Monthly and quarterly |
| Cost per consultation | Marketing cost for each attended consultation | Media plus agency or staff cost divided by attended consultations | Monthly |
| Cost per treatment start | Marketing cost for each new treatment start | Same cost divided by treatment starts in the cohort | Quarterly |
Agree the definitions with the front desk and clinical team before you start reporting. Decide what counts as a qualified enquiry, when a consultation counts as attended, and what marks a treatment start. Write the definitions down so the numbers mean the same thing every month, even when staff change.
Why is cost per treatment start better than cost per lead?
A lead only matters if it becomes a patient. Cost per lead rewards cheap enquiries, including people outside your area, people seeking a service you do not offer, and people years from a decision. Cost per treatment start rewards the channels and campaigns that actually bring new patients into care.
Cost per lead still has a use: it is an early warning that a campaign has changed. Read our note on fertility cost per lead for how to interpret it. But budget decisions should rest on cost per consultation and cost per treatment start, because those numbers include the quality of the lead and the clinic's ability to convert it.
Report both costs by service line where you can. IVF, egg freezing and fertility testing attract different people at different stages, and a blended figure can hide a service that is struggling. Even a simple split between IVF and egg freezing usually gives a clearer picture than one clinic-wide number.
How do you measure KPIs when treatment starts months later?
Use cohorts. Group enquiries by the month they arrived, then follow each group forward as people book, attend and start treatment. This credits a campaign with patients who took months to decide, instead of making last month look weak and a later month look strong for reasons unrelated to marketing.
- Give every enquiry a source and an arrival month the moment it is logged.
- Update its status as it moves: booked, attended, treatment started, or closed.
- Each month, report each cohort's cumulative bookings and starts to date.
- Compare cohorts at the same age, for example each cohort three months after arrival.
Cohort reporting needs a CRM that keeps the original source attached to the patient record, which our guide to fertility lead reporting covers in more detail.
Here is how it looks in practice. The January cohort might show its bookings within weeks, its consultations over the next month or two, and its treatment starts across the following months. If February's cohort is compared with January's at the same age, a change in campaign quality shows up clearly. Comparing calendar months instead mixes old and new enquiries and hides the effect.
Where does the KPI data come from?
Three systems hold the data: the ad platforms, the call and form tracking layer, and the CRM or practice records that show bookings and treatment starts. The KPIs only work when these are joined by source. Reporting problems usually trace back to a missing link between tracking and the CRM, not to the ad platforms.
- Ad platforms: spend, impressions, clicks and conversions for each campaign.
- Call and form tracking: which source produced each enquiry. Use call tracking built for fertility clinics and a vendor that signs a BAA.
- CRM and practice records: bookings, attendance and treatment starts, held under HIPAA.
Keep tracking pixels off patient portals and authenticated pages, and be careful on booking and intake flows. HHS guidance on online tracking still covers authenticated pages after the 2024 court ruling on public pages. Ask counsel to confirm your setup.
Add one more source: ask every new patient how they heard about the clinic, and record the answer in the CRM. Referrals from physicians, friends, employers and online communities are often invisible to tracking tools. Self-reported source fills that gap and helps you see the value of reviews, content and referral relationships. Our guide to tracking referral-partner leads explains how to log them.
Assign one owner for data quality. Someone should check each week that new enquiries have a source, that statuses are updated, and that duplicates are merged. Small gaps compound quickly in a long funnel.
Which fertility marketing metrics can mislead?
Metrics that count activity rather than patients can mislead: impressions, clicks, followers, raw lead volume and sessions. They are easy to grow and easy to report, but none of them shows whether more people are attending consultations or starting treatment. Use them to diagnose, never as the measure of success.
- Raw lead volume can rise while qualified leads fall.
- Platform-reported conversions often count form views or button clicks, not real enquiries.
- Blended averages hide differences between services; report IVF, egg freezing and other services separately.
- Short windows undercount slow decisions; one month of data is rarely enough to cut a channel.
Watch vanity metrics only when they explain a change in a core KPI. If consultations fall, impressions and click-through rate may show whether visibility dropped or whether the ads stopped persuading. If cost per enquiry rises, auction metrics may show new competition. Used this way, activity metrics answer a specific question instead of filling a report.
How often should a fertility clinic review its KPIs?
Review enquiries, response speed and answered calls weekly, conversion rates and costs monthly, and budget shifts quarterly using cohort data. A steady rhythm stops the clinic from overreacting to one strong or weak week, while still catching front-desk problems early enough to fix them before they cost patients.
Keep the report short: one page with the core KPIs, the trend, and one or two decisions for the next period. In our fertility marketing engagements, the clinic and the US team agree monthly goals on these KPIs, track them live on Ichelon Agency OS and review a report every Monday. Common questions
What is the single most important fertility marketing KPI?
Cost per treatment start, measured by monthly cohort. It shows how much marketing spend it takes to bring a new patient into a treatment cycle, and it accounts for lead quality and the clinic's conversion. Other KPIs, such as response speed and show rate, explain why it moves.
Should a fertility clinic track cost per lead?
Yes, but as a warning signal, not the main goal. Cost per lead shows when a campaign changes, but cheap leads are often unqualified. Budget decisions should rest on cost per attended consultation and cost per treatment start, which reflect whether leads actually become patients.
How do fertility clinics track marketing without breaking HIPAA?
Use call and form tracking from vendors that sign a Business Associate Agreement, keep tracking pixels off patient portals and other authenticated pages, and be careful on booking and intake flows. Pass only the minimum data needed back to ad platforms. Have counsel review the tracking setup.
Why do fertility marketing results look different month to month?
Because treatment often starts months after the first enquiry. A calendar-month report credits treatment starts to the wrong period. Cohort reporting, which follows each month's enquiries forward, shows the true yield of each campaign and makes month-to-month comparisons more reliable.
Which KPIs show a front-desk problem rather than a marketing problem?
Answered-call rate, speed to first response and consultation show rate. If enquiries rise but these fall, the clinic is losing people it already reached. Fixing call coverage, callback rules and reminders often improves cost per treatment start more than changing campaigns.
A note on this guide: it explains marketing practice, not legal advice. Rules on privacy, advertising and insurance change and vary by state, so confirm anything compliance-related with your own counsel.
