Which kind of CRM do clinics and hospitals in India use?
Clinics and hospitals in India mostly use one of four kinds of CRM: a healthcare-specific CRM, a general business CRM adapted for healthcare, the patient-relations module inside their hospital management system, or a WhatsApp-led messaging tool. Many smaller clinics still run on spreadsheets and phone call logs. The right choice depends on enquiry volume, number of locations and how much of the follow-up happens on calls and WhatsApp, which is the norm in India.
The four kinds of CRM in Indian healthcare
| CRM type | Strengths | Limits | Typical fit |
|---|---|---|---|
| Healthcare-specific CRM | Built for enquiries, appointments, calling, WhatsApp follow-up and recall; healthcare reports out of the box | Smaller ecosystem of add-ons than general CRMs | Clinics and hospitals running paid ads and handling daily enquiries |
| General business CRM | Flexible, many integrations, familiar to sales teams | Needs customisation for appointments, specialties and consent; often priced per user | Large groups with in-house tech teams |
| HMS patient-relations module | Data sits next to billing and clinical records | Usually weak on lead capture from ads, call tracking and WhatsApp | Hospitals focused on existing patients rather than new enquiries |
| WhatsApp-led messaging tool | Fast to set up, good for broadcasts and chat | Limited pipeline tracking, calling and source reporting | Single clinics with modest enquiry volume |
What a clinic CRM actually has to do
Indian patients enquire through many doors: Google Ads forms, Meta lead forms, Google Business Profile calls, Instagram DMs, WhatsApp, walk-ins and doctor referrals. A clinic CRM earns its place when it pulls these into one list and makes sure no enquiry is left uncalled. The essential jobs are:
- Lead capture from every source, with the source and campaign recorded automatically.
- Calling and WhatsApp in one place, so the front desk does not juggle personal phones.
- Follow-up reminders for patients who did not book on the first call.
- Appointment booking and reminders to reduce no-shows.
- Recall for reviews, repeat sessions and check-ups, with consent recorded.
- Reports on response time, conversion by source and conversion by staff member.
Response speed matters more than most clinics expect. Our analysis of lead response time in healthcare explains why the first call often decides whether a lead converts.
How the choice changes with size
Single clinic. A healthcare CRM with calling and WhatsApp built in is usually simpler than adapting a general CRM. If enquiries are few, a structured WhatsApp tool may be enough for now, as long as sources are tracked.
Specialty chains. IVF, dental, dermatology and hair transplant groups have long decision cycles and multi-step journeys. They benefit from pipelines matched to the specialty, such as consultation, investigation, counselling and treatment start. See our pages on CRM for IVF clinics and similar specialty set-ups.
Hospitals. Hospitals typically keep the HMS for clinical and billing work and add a CRM for enquiries, call centre work and campaigns. The two should share a patient identifier so marketing reports can match leads to billed admissions.
Why spreadsheets stop working
Many clinics start with a shared spreadsheet and a WhatsApp group. It works while enquiries are few and one person handles them. It breaks when a second receptionist joins, when ads start bringing leads at night and on weekends, or when the owner asks which campaign produced last month's procedures. Common symptoms include duplicate calls to the same patient, leads never called at all, and no record of why an enquiry did not book.
Moving to a CRM is less about software and more about process. Decide who owns each lead, how fast the first call must happen, how many follow-up attempts are made and when a lead is closed. Then configure the CRM to enforce those rules. A CRM set up without agreed rules simply becomes a more expensive spreadsheet.
Data protection and WhatsApp rules
Any CRM holding patient names, phone numbers and health interests is processing personal data under the Digital Personal Data Protection Act, 2023. Record consent for marketing messages separately from consent for care, let patients withdraw it, and restrict who can export data. WhatsApp Business messaging also follows Meta's template and opt-in rules. Our guide to DPDP consent for WhatsApp marketing covers both.
What to measure once the CRM is live
A CRM is only useful if someone reads its reports. Review a short set of numbers every week: enquiries by source, time to first call, share of enquiries reached, appointments booked, show-ups and conversions to treatment. Compare staff members and sources side by side. Over a few months these figures show which campaigns bring real patients and which only bring phone numbers, so the marketing budget can follow results rather than lead counts.
Questions to ask before you buy
- Does it capture leads from Google Ads, Meta, your website and Google Business Profile without manual entry?
- Can staff call and WhatsApp from inside the CRM, with calls recorded and logged?
- Can it report conversion by source, by campaign and by staff member?
- Does it connect to your HMS or billing system?
- How does it store consent, and where is data hosted?
- What does onboarding and training include?
