  • Absolute lift from no-steps to full schema-with-images: +17.7 percentage points. One of the largest single-page schema lifts observed in the ICG AIO study series.

Methodology

1,404 URLs from the ICG SIE dataset were classified into four cohorts based on the procedural structure of the page: no numbered steps at all, numbered HTML steps without HowTo schema, HowTo schema with 5-7 steps, HowTo schema with 8 or more steps plus an image attached to every step. Cohorts were balanced on baseline organic authority (Domain Rating band), page-intent classification (pre-procedure prep vs post-procedure recovery vs booking-and-onboarding vs internal workflow) and category concentration before analysis.

Each URL was probed against a matched keyword set — typically 4 to 12 keywords per URL, selected as the top-intent queries the page targets — on a weekly AIO probe crawl. Citation rate is defined as the share of AIO-carrying result pages, for the matched keyword set, in which the URL appears as a listed citation. 16,800 citation checks were executed across the four-month window.

Step attribute analysis. Every numbered step inside every cohort was scored on six attributes: step number in the H3 or step-title, verb-first imperative step name, single-action opening sentence, image attached to the step, time-or-tool metadata declared in schema, and step count between 5 and 12 for the parent procedure. Attribute lift is the ratio of citation rate for steps carrying the attribute versus steps in the same cohort without it. Effects are broadly additive.

Anonymisation. No client names, page titles or URL strings appear anywhere in this report. Cohort figures are aggregate sample means.

Which healthcare page types benefit most

Pre-procedure preparation pages. "How to prepare for an MRI", "how to prepare for an endoscopy", "how to prepare for an IVF cycle". Procedural intent is unambiguous, steps are naturally ordered, and image-per-step is achievable. Highest observed lift in the study.

Post-procedure recovery pages. "First 48 hours after implant", "post-op care for cataract surgery". Same procedural intent, image-per-step less common but time-and-tool metadata are strong.

Booking and onboarding pages. "How to book a consultation", "how to submit insurance pre-authorisation". Operator-facing rather than patient-facing but still qualifies as a procedure — image-per-step becomes screenshot-per-step.

Internal workflow pages. "How to file a compliance report", "how to onboard a new patient into the practice". Team-facing content that also earns AIO citations on operator queries.

What to keep off HowTo. Product pages, service overviews, category pages, list-of-clinics pages, diagnostic-condition pages. Misclassification risks a structured-data spam manual action.

What to change on your existing HowTo pages this week

1 · Put the step number inside the H3 or step-title

"Step 3: Clean the site" not "Clean the site". +34 percent lift and takes about five minutes per page.

2 · Rewrite every step name to open with a verb

"Prepare the equipment" not "Equipment preparation". Verb-first imperative reads as a procedure to Google\'s classifier.

3 · Rewrite the first sentence of every step body as a single action

Is HowTo schema still worth using in 2026?
Yes, for procedure and workflow pages. The classic HowTo rich result was significantly deprecated in mid-2023, but the underlying schema still functions as a procedure-classification signal for AI Overviews, PAA, image carousels and third-party AI assistants. Pages carrying HowTo schema with 5-7 well-structured steps cite in AIO at 19.4 percent in the sample; the same pages with numbered HTML steps but no schema cite at 12.8 percent; pages with no numbered steps at all cite at 6.9 percent. The 12.5 percentage-point absolute lift from no-steps to schema-5-7-steps is one of the largest observed in the ICG AIO study series.
When should I NOT use HowTo schema?
When the page is not a procedure. Product pages, service overview pages, category pages, list-of-clinics pages — none of these are HowTo-eligible and marking them up as HowTo risks a manual action for structured-data spam. HowTo schema is for pages that describe a real, ordered procedure the reader can follow: how to prepare for an MRI, how to submit an insurance pre-authorisation, how to book a consultation, how to file a compliance report. If your page is not walking the reader through numbered steps, use Article or FAQPage instead.
How many steps is the right number?
Five to twelve. Below five, the page reads as a shallow procedure and both Google and the AIO citation model tend to discount it. Above twelve, the page reads as a chapter list and the procedural coherence breaks — the AIO citation model prefers a coherent linear procedure it can quote a step from. The efficient operating point is 6-10 steps for most healthcare workflow pages.
Do I need an image per step?
Not required, but a strong signal when present. The 8+ step cohort with image-per-step cites at 24.6 percent, versus 19.4 percent for the 5-7 step cohort without image-per-step — a 5.2 percentage-point difference. Image-per-step also unlocks image-carousel eligibility on mobile search results and improves click-through when it does render. If step imagery is achievable (photos, diagrams, screenshots), use it; if not, the procedure still qualifies without it, just at a lower citation rate.
