The remaining 76 delay - and most of them never convert, not because they chose a competitor, but because no system kept them engaged through the decision lag.

This number reframes the entire IVF marketing problem. The 4 delay reasons: what is actually stopping qualified patients

ICG's research identifies four categories of delay reason for middle-income IVF patients who qualify but do not start treatment:

42% - Cost and hidden cost anxiety

Cost anxiety is the single biggest delay driver - affecting nearly half of all delayed middle-income IVF leads. The anxiety has three components: the high upfront cost of a cycle (typically ₹1-2 lakh), fear of hidden charges that are not disclosed at enquiry, and difficulty planning a budget across potentially multiple cycles.

The operational fix: transparent pricing at every touchpoint. Show the total cost of a cycle - including medications, diagnostics, and freezing fees - in ad creative, landing pages, and the first counselling call. Hidden cost anxiety is created by opacity. Transparency resolves it without reducing your pricing.

23% - Outcome uncertainty

"What if it fails?" is the question that keeps 23% of qualified IVF leads from committing. They have the money. They want the treatment. But the fear of spending ₹1-2 lakh and not having a child - and having to face that outcome - creates a paralysis that generic follow-up calls cannot resolve.

The fix: medical precision and outcome clarity. Not success rate claims (those are restricted under Indian Medical Council guidelines), but specific, honest clinical communication - what the doctor would tell a patient in a consultation. "At your AMH level, your response to stimulation is likely to be X. Here is what we would do in cycle 1 and how we would adjust in cycle 2 if needed." Specificity builds confidence. Vague reassurance does not.

15% - Medical fear and past trauma

15% of delayed leads have a specific medical fear barrier: fear of injections and procedures, or emotional and physical trauma from a previous failed attempt. This segment needs a fundamentally different communication approach - one that acknowledges the fear explicitly rather than minimising it.

ICG's research shows that video consultation is particularly effective for this segment: a 60-minute video call with the fertility specialist, in a private, low-pressure environment, where the patient can ask the questions they are afraid to ask in a clinical setting, produces a 25% uplift in lead-to-visit conversion.

20% - Practical constraints

One in five delayed qualified leads is blocked by practical constraints: the spouse or family is not yet aligned, logistics for a multi-cycle treatment are not sorted, or the patient wants a second opinion from another clinic before committing. These delays are real but time-limited - the patient will eventually decide.

The operational approach: CRM tagging. Tag each QBNI (Qualified But Not Interested) lead with the specific constraint - "Spouse not aligned", "Second opinion pending", "Timing - next month" - and automate a different follow-up sequence for each tag. A lead tagged "Second opinion pending" should receive clinic authority and comparison content. A lead tagged "Spouse not aligned" should receive content designed for couples to discuss together.

The QBNI lead: your biggest hidden revenue asset

QBNI stands for Qualified But Not Interested - the CRM status for leads who have been qualified as appropriate candidates but have not yet expressed active interest in booking. Most IVF centres either ignore these leads or send them the same generic follow-up as everyone else. High-performing centres treat them as the most valuable segment in the CRM.

The logic: you have already paid to generate a QBNI lead. The marketing cost is sunk. Every conversion from this pool is incremental revenue at near-zero acquisition cost. The cadence works because it does not push - it nurtures. Each touchpoint delivers something the patient finds useful (information, a story, a low-commitment consultation offer) rather than repeating the same booking ask. The patient feels accompanied through the decision, not pressured into it.

Medical profiling: the qualification rate multiplier

Most IVF counsellors use a generic sales script in the first call: name, location, budget. ICG's research shows that centres using a medical profiling approach - where the counsellor requests AMH and hormone levels, asks about years of trying, and details any prior IUI or IVF attempts - achieve a qualification rate of 15-20%, versus 6-8% for standard scripts.

Why does medical profiling improve qualification? Because it filters out the "just searching" personas immediately (a patient who deflects all medical questions is researching, not buying), while simultaneously establishing the clinic's authority with patients who are ready to engage clinically. The counsellor who asks "Can you share your AMH test results?" signals that this clinic takes a diagnostic approach - which is exactly the signal high-intent patients are looking for.
