The debate between Google Ads and Meta Ads for IVF marketing is usually framed as a binary choice. ICG's research shows it is not a choice at all - it is a strategic allocation decision based on patient persona and revenue target.
In ICG's analysis of 12 IVF centres, the digital channel split for conversions breaks down as follows: Social Media Ads (primarily Meta/Facebook/Instagram) drive 45% of total IVF conversions by volume, Organic channels (Google My Business, website, YouTube) drive 35% with the highest average revenue per patient, and Google Search Ads drive 20% - the smallest volume share but the highest per-lead conversion quality.

The Meta Ads funnel data tells a very different story - and one that most IVF clinic owners misinterpret. The raw numbers look alarming: 3,008 leads generating only 41 conversions (1.36%). But this misreads the channel economics.

The critical comparison: Google converts 12.16% of raw leads; Meta converts 1.36%. But Meta generates 40x the volume. In this dataset, Meta produced 41 conversions against Google's 9 - from different spend levels. If the cost per raw Meta lead is low enough (typically ₹200-₹500 for IVF Meta leads in India), the economics work despite the lower per-lead conversion rate.
The Meta funnel also reveals where the quality gap materialises: the Appointment Visit → Conversion stage. 112 Meta-sourced patients attended a consultation; only 41 converted (36.6%). For Google-sourced patients: 21 attended, 9 converted (42.9%). The conversion rate at consultation is also higher for Google patients - confirming that intent quality differences persist all the way to the treatment decision.
What the "visit attendance" gap reveals about Meta leads
The single most diagnostic number in the Meta funnel: of 233 patients who confirmed their appointments, only 112 actually attended (48%). In the Google funnel, 27 confirmed and 21 attended (78%). This 30-percentage-point gap in visit attendance is where Meta lead quality is most visible.
Meta leads who book and confirm but do not attend are often: lower-intent patients who enquired impulsively, leads who have simultaneously booked at multiple clinics and attended a closer one, or patients whose cost anxiety resolved itself before the appointment (they decided they cannot afford it without the counsellor's help).
The fix: pre-visit engagement. An organic patient who reaches your clinic has typically researched for weeks or months, has already formed a positive opinion based on what they read and watched, and arrives at the consultation partially decided.
ICG's strategic recommendation: for IVF centres with ARPU targets above ₹2 lakh per cycle, organic investment (SEO, YouTube authority, AEO for AI search) is the highest long-term ROI channel. It takes 6-12 months to build - but once established, organic CPQL falls below ₹500 for established terms, against ₹1,500-₹2,500 for paid channels.
The 5-minute response rule and its impact on both channels
One of the most impactful operational findings in ICG's research applies equally to both Google and Meta leads: response time cannot exceed 5 minutes during working hours. A lead who does not receive contact within 5 minutes of enquiry is 3x more likely to book with a competitor who responds first.
Meta leads are particularly time-sensitive because the enquiry is often impulsive - triggered by an ad seen during casual browsing, not by a deliberate search. The patient's intent window is narrower. A 30-minute response to a Meta lead often arrives after the patient has moved on. The 5-minute rule applies everywhere; it matters most for Meta.
