By Raman Soni, Head of Performance Marketing at ICG.

TL;DR

  • Meta Ads generates 45% of IVF digital conversions by volume - primarily affordable single cycles for lower-SEC patients. Google Search Ads generates 20% of conversions but at higher ARPU (2-3 cycle packages). Organic channels drive 35% with the highest revenue per patient. The right channel mix depends on your ARPU target. Google converts 12.16% of raw leads; Meta converts 1.36% - but Meta generates 40x the volume. ICG Report 2025.

The channel split for IVF in India: what the data shows

<a href=Meta Catalyst IQ long-term comparison view charting Meta Ads performance across quarters with spend, CPQL and volume overlaid" width="1200" height="675" loading="lazy" decoding="async" style="width:100%;height:auto;display:block;">

The debate between Google Ads and Meta Ads for IVF marketing is usually framed as a binary choice. ICG's research shows it is not a choice at all - it is a strategic allocation decision based on patient persona and revenue target.

In ICG's analysis of 12 IVF centres, the digital channel split for conversions breaks down as follows: Social Media Ads (primarily Meta/Facebook/Instagram) drive 45% of total IVF conversions by volume, Organic channels (Google My Business, website, YouTube) drive 35% with the highest average revenue per patient, and Google Search Ads drive 20% - the smallest volume share but the highest per-lead conversion quality.

IVF Marketing Insights Report 2025

India's only data-backed IVF research. 10,000+ seekers, 12 centres, 34 pages.

The strategic framework: if your IVF centre targets high-ARPU patients (₹2.5-3 lakh per cycle and above), focus on Organic and Google Search where the highest-intent, highest-income patients research and convert. If you operate a high-volume affordable-package model, Meta Ads is the primary acquisition engine. Most centres run a mix - but the allocation percentages matter.

The 12.16% end-to-end conversion rate from Google Ads raw leads to treatment is exceptional for a high-ticket medical procedure with a 1-3 month decision cycle. The 41.89% qualification rate confirms the channel's intent quality - Google Search captures patients who are actively searching for IVF treatment, not patients who happen to be scrolling a social feed.

The Meta Ads IVF funnel: why 3,008 leads produces 41 conversions

The Meta Ads funnel data tells a very different story - and one that most IVF clinic owners misinterpret. The raw numbers look alarming: 3,008 leads generating only 41 conversions (1.36%). But this misreads the channel economics.

[Source: ICG IVF Marketing Insights Report 2025, Page 18]

The critical comparison: Google converts 12.16% of raw leads; Meta converts 1.36%. But Meta generates 40x the volume. In this dataset, Meta produced 41 conversions against Google's 9 - from different spend levels. If the cost per raw Meta lead is low enough (typically ₹200-₹500 for IVF Meta leads in India), the economics work despite the lower per-lead conversion rate.

The Meta funnel also reveals where the quality gap materialises: the Appointment Visit → Conversion stage. 112 Meta-sourced patients attended a consultation; only 41 converted (36.6%). For Google-sourced patients: 21 attended, 9 converted (42.9%). The conversion rate at consultation is also higher for Google patients - confirming that intent quality differences persist all the way to the treatment decision.

What the "visit attendance" gap reveals about Meta leads

The single most diagnostic number in the Meta funnel: of 233 patients who confirmed their appointments, only 112 actually attended (48%). In the Google funnel, 27 confirmed and 21 attended (78%). This 30-percentage-point gap in visit attendance is where Meta lead quality is most visible.

Meta leads who book and confirm but do not attend are often: lower-intent patients who enquired impulsively, leads who have simultaneously booked at multiple clinics and attended a closer one, or patients whose cost anxiety resolved itself before the appointment (they decided they cannot afford it without the counsellor's help).

The fix: pre-visit engagement. Patient Education AI - delivering WhatsApp content between the appointment confirmation and the visit day - reduces no-show rates by 18-22% for Meta-sourced leads by maintaining engagement and addressing the most common pre-visit doubts. The visit attendance rate gap between Meta and Google narrows significantly when pre-visit automation is in place.

Organic channels: the highest ARPU engine

The organic channel - Google My Business, website traffic, YouTube - drives 35% of IVF digital conversions at the highest ARPU and fastest post-consultation decision speed. An organic patient who reaches your clinic has typically researched for weeks or months, has already formed a positive opinion based on what they read and watched, and arrives at the consultation partially decided.

ICG's strategic recommendation: for IVF centres with ARPU targets above ₹2 lakh per cycle, organic investment (SEO, YouTube authority, AEO for AI search) is the highest long-term ROI channel. It takes 6-12 months to build - but once established, organic CPQL falls below ₹500 for established terms, against ₹1,500-₹2,500 for paid channels.

The 5-minute response rule and its impact on both channels

YODA · Topic × Format MatrixCross-tab of every healthcare topic × every video format · heatmap shows which combinations compound (green) and which fatigue (red). The single most-actionable YODA view.

One of the most impactful operational findings in ICG's research applies equally to both Google and Meta leads: response time cannot exceed 5 minutes during working hours. A lead who does not receive contact within 5 minutes of enquiry is 3x more likely to book with a competitor who responds first.

Meta leads are particularly time-sensitive because the enquiry is often impulsive - triggered by an ad seen during casual browsing, not by a deliberate search. The patient's intent window is narrower. A 30-minute response to a Meta lead often arrives after the patient has moved on. The 5-minute rule applies everywhere; it matters most for Meta.

2026 update: budget allocation math and the three attribution mistakes that break this comparison

The channel-mix picture above holds for 2026, but two things have shifted since we first published this data: Meta CPL for IVF has risen roughly 18-24% year-on-year in Tier-1 metros, and Google Search CPCs on fertility keywords are now competitive with dental implant CPCs. That means the raw funnel percentages stay the same, but the rupee cost per confirmed visit now decides the split - not the click cost.

Working budget split we recommend for 2026

Centre stageMeta AdsGoogle SearchYouTube / Organic
New centre (0-12 months)55-65%20-25%15-20%
Established centre (12-36 months)30-40%35-45%20-30%
Premium / high-ARPU centre15-25%30-40%35-50%

The three attribution mistakes that hide the truth in your dashboard

  1. Attributing walk-ins to Direct or Organic when the patient actually saw a Meta reel three weeks earlier. This inflates organic ARPU and starves Meta of credit. Fix it with a first-touch question on every enquiry form and IVR: How did you first hear about us?
  2. Reporting Meta Leads as conversions instead of confirmed appointments. As the funnel above shows, only 41 of 3,008 Meta leads convert - so a CPL of INR 400 is really a cost-per-confirmed-visit of INR 29,000+. Always compare channels on visit or consult cost, never lead cost.
  3. Ignoring the 5-minute response gap between channels. Meta leads decay 4x faster than Google leads. If your CRM shows Meta underperforming, check response time before you cut the budget.

We run this exact attribution audit inside every IVF engagement using Meta Catalyst IQ for the paid-social side and Prism Spy to benchmark against the three closest competitor centres in your city. For the organic + YouTube layer, YODA handles the ARPU-building content engine. If you want us to model your centre's specific budget split against your current ARPU and CAC, book a Client Elevation Programme diagnostic.

Frequently asked

Questions readers ask
about this topic.

Both serve different roles. Google Search Ads captures patients who are already searching for IVF treatment - producing a 41.89% qualification rate and a 12.16% raw lead to conversion rate. Meta Ads reaches a broader audience and generates 40x the volume at a 1.36% raw conversion rate. The right channel depends on your ARPU target: high-ARPU centres prioritise Google and Organic; volume-oriented affordable centres prioritise Meta. Source: ICG IVF Marketing Report 2025.

ICG's actual MIS data from active IVF Google Ads campaigns shows: 41.89% of raw Google leads qualify, 28.38% attend a consultation, and 12.16% convert to treatment. These are the only publicly available actual funnel data points for IVF Google Ads in India. Source: ICG IVF Marketing Insights Report 2025, Page 17.

ICG's MIS data shows: 24.10% of raw Meta IVF leads qualify, 3.72% attend a consultation, and 1.36% convert to treatment. Of the 3,008 Meta leads in this dataset, 41 converted. The economics work because Meta CPLs are low and volume is high. The biggest Meta conversion challenge is the appointment visit rate (48% of confirmed appointments attend, vs 78% for Google). Source: ICG IVF Marketing Report 2025, Page 18.

Based on ICG's analysis of 12 IVF centres, approximately 50% of all IVF cycles are influenced by a digital-first patient journey. Of this digital share, Meta Ads drives 45% of conversions, Organic channels 35%, and Google Search Ads 20%. Extrapolated nationally, this represents 1 lakh+ IVF cycles per year influenced by digital marketing. Source: ICG IVF Marketing Report 2025, Pages 6-7.

Meta Ads intercepts patients who are not actively searching - creating a lower-intent lead pool at the point of enquiry. Meta's 24.10% qualification rate vs Google's 41.89% reflects this intent gap. The most visible quality difference: 78% of Google-confirmed appointment patients attend, vs only 48% of Meta-confirmed patients. Pre-visit WhatsApp engagement reduces this gap significantly.

Organic channels (Google Business Profile, website, YouTube) drive 35% of IVF digital conversions - with the highest average revenue per patient (ARPU) of any channel. Organic patients have typically researched for weeks before contacting a clinic and arrive with high intent and partially-formed trust. Google Search ranking for IVF-related queries and YouTube authority content are the primary organic acquisition drivers.
