Fractional CMO vs marketing agency: what's the difference?

A fractional CMO decides what your marketing should achieve and holds everyone to it; a marketing agency runs the campaigns. The CMO owns strategy, budget, priorities and reporting to leadership. The agency builds and manages ads, SEO, content and design. Many healthcare companies need both: the agency for capacity, the CMO to judge whether it pays off.

TL;DR
  • Strategy versus execution: the fractional CMO sets direction and budget; the agency produces and manages campaigns.
  • Accountability runs one way: the CMO briefs, reviews and judges the agency, not the reverse.
  • An agency alone rarely challenges its own scope, which is why a separate senior owner helps once spend grows.
  • Both may need a BAA if they could access protected health information on your behalf.
  • Every practice welcome — retainers from $499/mo, Goals-Driven engagements, Performance-Linked Payout Models available.

“They were able to get all my ideas and work with me over a period of three to four months and create this amazing website. It's super customized, very modern, and it incorporates all the elements that I had wanted — the patient portal, nice pictures, a very interactive website, patient reviews. I would highly recommend their company to anyone who wants to make an excellent website.”

How is a fractional CMO different from a marketing agency?

A fractional CMO is a part-time executive who sits on your side of the table. They own the plan, the budget and the results. A marketing agency is a vendor that supplies execution: campaign setup, ad management, SEO, content, design and reporting on its own work. One decides and judges; the other builds and runs.

The distinction matters because the two roles answer different questions. The CMO asks: should we be spending on this channel at all, and how much? The agency asks: how do we make this channel perform as well as possible? Both questions are important. But if only the agency is asking, nobody is checking whether the channel mix itself is right.

The roles also differ in who they serve. An agency works to a scope of work and must manage its own staffing and margins. A fractional CMO works to your company's goals, and part of their job is deciding which vendors you need.

Comparison

What does each one do, side by side?

The clearest way to see the difference is to compare who owns each part of the marketing cycle. The fractional CMO owns the decisions at the top and the judgment at the end. The agency owns the production and optimization in between. Overlap usually appears in reporting, where both look at results from different angles.

Healthcare companies with one channel and a small budget often manage with an agency alone. The case for a CMO grows with spend, channels and the number of vendors involved.

Incentives

Why can't the agency just act as your strategist?

An agency can give useful strategic advice, but it is hard for any vendor to recommend cutting its own scope. An agency that runs paid search is unlikely to suggest moving budget to referral programs it does not offer. A fractional CMO has no channel to protect, so their advice can follow the evidence.

There are practical limits too:

  • Partial view: an agency usually sees only the channels it manages, not sales follow-up, front-desk handling or other vendors' results.
  • Different success measures: agencies often report clicks, leads or rankings. Leadership needs booked appointments, revenue or pipeline.
  • No internal authority: an agency cannot fix a slow intake process or reassign staff.

None of this means agencies act in bad faith. It means the strategist and the executor work best as separate roles, with clear lines between them.

Signs your agency is filling a strategy gap

  • The agency sets its own goals, and nobody internal questions them.
  • Monthly reports show activity, such as impressions and clicks, but not booked patients or pipeline.
  • Budget increases are proposed by the agency and approved without comparison to other channels.
  • Nobody can say what would happen if one channel were paused for a month.
  • Different vendors report conflicting lead numbers, and nobody reconciles them.

Two or more of these usually mean the company needs a senior owner of marketing, whether fractional or full-time.

Combinations

Does a healthcare company need a fractional CMO, an agency, or both?

The answer depends on spend, number of channels and in-house capacity. A small practice with modest paid search can often work with an agency alone. A company running several channels, vendors or locations usually benefits from both. A company with a strong in-house team may need a fractional CMO and only specialist freelancers.

Your situationUsually enough

Several channels or vendors, no senior marketing ownerFractional CMO plus agency
Capable in-house marketers, no plan or budget disciplineFractional CMO, with freelancers for gaps
Startup preparing a launch or fundraiseFractional CMO first, then add execution
Large in-house department, several product linesFull-time CMO plus specialist agencies

The mix can change over time. Many companies start with a fractional CMO and one agency, then bring some work in-house as the team grows. Others keep a lean team and rely on specialist agencies for paid media, SEO and video. The CMO's job is to revisit that mix each quarter, based on cost, quality and speed, rather than leaving it fixed by habit.

Working together

How should a fractional CMO manage a marketing agency?

A fractional CMO manages an agency through written briefs, agreed goals, a fixed reporting rhythm and an approval step before launches. The aim is to turn the agency's work into business results the leadership team can see, without micromanaging how specialists do their jobs.

  1. Brief in writing: audience, offer, goal, budget and compliance limits for every campaign.
  2. Agree goals that match the business: booked appointments, qualified demos or revenue, not only clicks.
  3. Own the accounts: ad accounts, analytics and dashboards should belong to your company, with agency access granted.
  4. Approve before launch: claims, testimonials, landing pages and tracking get checked first.
  5. Review on a fixed schedule: a monthly review covers spend, results and next month's changes.
  6. Compare against alternatives: each quarter, ask whether the money would work harder elsewhere.

This structure also protects the agency. Clear briefs and agreed goals reduce disputes about what was promised.

It also helps the agency do better work. A CMO who shares sales outcomes, front-desk feedback and patient or customer insights gives the agency context it rarely gets on its own. Campaigns improve faster when the people building them know which leads actually turned into appointments or contracts.

Privacy and choice

What should healthcare companies check before hiring either one?

Check healthcare experience, how each party will handle patient information, and who owns your accounts and data. Under HIPAA, a vendor that creates, receives, maintains or transmits protected health information for a covered entity is a business associate and needs a Business Associate Agreement.

HHS explains these duties in its guidance on business associates. Whether a particular CMO or agency needs one depends on what data they touch, so confirm with your counsel. Also ask both parties how they handle tracking on booking and intake pages, and how they review health claims. Our overview of what practices should require from agencies and vendors lists the questions.

FAQ

Common questions

What is the difference between a fractional CMO and a marketing agency?

A fractional CMO is a part-time executive who owns marketing strategy, budget and results for your company. A marketing agency is a vendor that executes campaigns such as ads, SEO and content. The CMO decides what to do and judges whether it worked; the agency builds and runs the work.

Can a fractional CMO replace a marketing agency?

Not usually. A fractional CMO directs and reviews execution but does not normally run campaigns. Unless you have in-house marketers or freelancers to do the work, you still need an agency or other execution support. The CMO can help you choose, brief and manage that support.

Is it a conflict of interest if one firm provides both the CMO and the agency?

It can be if results are not visible to you. Reduce the risk by agreeing goals in writing, owning your ad and analytics accounts, and receiving regular reports on business outcomes such as booked appointments. Separate scopes and fees for strategy and execution also make it easier to judge each part.

Does a fractional CMO need to sign a BAA?

Under HIPAA, any vendor that creates, receives, maintains or transmits protected health information on behalf of a covered entity is a business associate and needs a Business Associate Agreement. Whether your fractional CMO needs one depends on the data they access. Confirm with your counsel.

Should a healthcare startup hire an agency or a fractional CMO first?

Most startups benefit from a fractional CMO first, because the plan, positioning and budget should exist before anyone is paid to execute. Without them, an agency runs campaigns against unclear goals. Once the plan is set, the CMO can brief and hire the right execution partners.

A note on this guide: it explains marketing practice, not legal advice. Rules on privacy, advertising and insurance change and vary by state, so confirm anything compliance-related with your own counsel.
