TL;DR — six findings from the token appearance scan
- Average healthcare GBP renders 4.1 of 14 tracked tokens per Maps card. The top-decile listing renders 8-10. Every card that shows more tokens out-clicks the same card with fewer, holding rank position constant.
- Wheelchair-accessible entrance is the most-appeared token at 71.3 percent of the sample. It is a direct attribute toggle in the Business Profile Manager, and it renders on almost every applicable card as soon as the attribute is enabled.
- Free parking is second at 62.5 percent — same mechanism.
- Booked appointments is the hardest to earn at 6.4 percent. Requires either Google-integrated appointment booking or heavy review-mining evidence.
- Same-day availability appears on 11.3 percent of the sample. It is unlocked primarily through review-mining, which means it responds to structured review-request prompts asking about appointment turnaround.
- Attribute completion is the single strongest lever on token count. Profiles with 100 percent attribute completion averaged 6.8 tokens per card; profiles below 50 percent attribute completion averaged 2.4.

Five numbers to anchor the report
Methodology
For every profile in the 328-listing Angryturtle portfolio, we ran a standard specialty query set at rendering-time via a headless-browser Maps render pipeline and captured the token strings that appeared on the profile's local-pack card. Query set: three standard queries per specialty (a category-name query, a service-name query, a "near me" query at a mid-density city grid point). Each query was rendered from a neutral IP inside the profile's own metro. Token strings were parsed from the rendered card DOM and grouped into the 14-token tracking set.
The 14 tracked tokens. Wheelchair-accessible entrance, Wheelchair-accessible parking lot, Free parking, Free street parking, Restrooms accessible, LGBTQ+ friendly, Ladies-only-hours (India-specific), Female-friendly, Booked appointments, Same-day availability, Telehealth, Insurance accepted, Cashless (India-specific), In-network insurance. The 14 tokens were selected because they are the tokens most frequently observed on Indian healthcare cards in a preliminary 100-render sample; a broader token universe exists but appears very rarely on healthcare cards specifically.
Eligibility analysis. For every token, we correlated appearance rate with three profile-level variables: whether the token-matching attribute is enabled in the Business Profile Manager, whether the service list contains a token-matching entry, and whether the review corpus contains token-matching vocabulary at least once. This lets us decompose token appearance into attribute-driven, service-driven, and review-driven eligibility.
Anonymisation. No listing identifiers, per-profile token lists, or per-query render captures appear in this report. All figures are portfolio-level or specialty-level aggregates.
Finding 1 · Average healthcare GBP shows 4.1 of 14 tokens per card
The number: across the sample and query set, the mean number of tokens rendered on any single Maps card is 4.1. The median is 4. The 25th percentile is 2; the 75th percentile is 6; the 95th percentile is 9.
The 14-token universe is aspirational for any single profile. No listing in the sample rendered all 14 on any single query because several tokens are mutually exclusive by category (Ladies-only-hours does not apply to a Hospital; Telehealth does not apply to a Diagnostic centre). The realistic ceiling for a healthcare profile is 8-10 tokens, and the top-decile achieves that consistently. The gap between 4.1 and 8-10 is the addressable operational leverage in the study.
Finding 2 · The three easy-attribute wins account for most of the sample average
The three tokens: Wheelchair-accessible entrance 71.3 percent, Free parking 62.5 percent, Restrooms accessible 58.9 percent. Together these three account for roughly 45 percent of the average card's 4.1 tokens.
All three are direct attribute toggles in the Business Profile Manager. Enabling the attribute is a two-click operation. The token renders on the profile's local-pack card within days of the attribute being switched on. This is the fastest available lift on any healthcare GBP — a 30-minute session enabling all applicable attributes typically adds 2-3 tokens to the render before any other work is done.

Finding 3 · Attribute completion is the single strongest lever on token count
The correlation: profiles with 100 percent attribute completion averaged 6.8 tokens per card. Profiles below 50 percent attribute completion averaged 2.4 tokens per card. The gap of 4.4 tokens is almost the entire spread between average and top-decile in the study.
Only 80.2 percent of the sample has any attributes populated at all. Only 12 percent has 100 percent completion. This is the single fastest and cheapest lever available on any healthcare GBP. The full attribute list for a specific primary category runs to between 15 and 40 fields; enabling every applicable field takes 15-30 minutes. The token payoff is roughly 4-5 additional tokens per Maps card and a click-through lift on the local pack that shows up within two weeks.
Finding 4 · Same-day availability responds to review-request prompts
The pattern: Same-day availability is a review-text-mined token — Google infers eligibility from reviewer language ("Doctor saw me the same day I called", "walked in without appointment"). The mining is passive; the operator cannot toggle it directly. But reviewer language is influenceable through the post-visit review-request prompt.
In a sub-sample of 42 profiles that added an explicit "was your appointment easy to book" prompt to their SMS review-request flow, Same-day availability token appearance rose from 8 percent to 34 percent inside 90 days. The prompt does not ask reviewers to lie — it simply steers those who had a same-day experience to mention it, and Google's mining then picks up the vocabulary.
Finding 5 · Booked appointments requires integrated booking almost always
The pattern: Booked appointments token appears on 6.4 percent of the sample. Almost every appearance is on profiles that either use Google's integrated appointment booking or a Google-partner booking provider. Review-text mining alone rarely triggers the token; the integrated-booking signal is close to prerequisite.
Google integrated booking is still rare in Indian healthcare because the domestic booking-provider ecosystem is not yet fully wired to Google's partner API. The token is therefore a leading indicator of a specific operational choice rather than of general listing quality. Operators can leave the Booked appointments token as a lower-priority target and focus on the tokens with faster mechanisms.
Finding 6 · Token count correlates directly with click-through rate
The finding: holding rank position and time-of-day constant, cards with 6+ tokens rendered showed a click-through rate 34 percent higher than cards with 2 or fewer, across a matched sub-sample of 74 profile pairs. The lift held across specialties and city densities.
Over 90 days, the higher click-through feeds back into the ranking model as a positive engagement signal, and profiles with higher token counts showed measurably improved pack presence — 1.4 additional grid points on the top-3 pack scan across a matched 30-profile sub-sample. Tokens are therefore both a conversion lever (higher CTR at existing rank) and a ranking input (higher pack presence over 90 days).
Finding 7 · Specialty determines which tokens are addressable
Not every token is a fair target for every specialty. Ladies-only-hours is a meaningful token for a general practice or a diagnostic centre that runs a dedicated women's slot; it is not a meaningful token for a pediatric dentist or a cosmetic surgeon. Telehealth has become a natural fit for dermatology, endocrinology and psychiatry follow-ups; it is a stretch for hair transplantation or physiotherapy where the visit itself is the service.
The addressable token set for a given profile is typically 8 to 10 of the 14 tracked. Operators should map the addressable set at onboarding rather than trying to earn every token — the ranking model rewards coverage of eligible tokens, not coverage of the full universe. A hair transplantation clinic that has enabled Wheelchair-accessible entrance, Free parking, Restrooms accessible, Female-friendly, Insurance accepted, Cashless, Free street parking, and In-network insurance is at 8/8 of its realistic addressable set — and will out-rank a listing that has tried to force Telehealth and Booked appointments without the underlying operational fit.
Finding 8 · Token coverage compounds with review-response quality
In a matched sub-sample of 38 profiles paired by primary category and city density, listings that ran a 72-hour review-response SLA and had 6+ tokens rendering on their card showed a pack-position gain of 1.6 grid points over 90 days compared to matched listings with the same token count but a slower response SLA. The two signals appear to compound: high token coverage plus fast response signals a well-operated listing to both searchers and to Google's ranking model.
The compounding also runs the other way. A listing with high token coverage but a slow response SLA shows a smaller lift than the raw token count would predict. Tokens are ingredient-quality, not the whole recipe — the operational discipline behind them is what earns the ranking payoff.
What this means for healthcare operators
1 · Complete the attribute list first, always
Fastest, cheapest, and highest-yield GBP investment available. 15-30 minutes per listing. 4-5 additional tokens on the card render within days. Click-through lift within two weeks.
2 · Populate the service list to the field maximum
Insurance accepted, Telehealth, In-network insurance and Cashless all lean on service-list content. Enter every applicable service, every accepted insurer, and every payment mode.
3 · Add a same-day availability prompt to the review-request flow
Same-day availability token rose from 8 percent to 34 percent inside 90 days across 42 profiles that added the prompt. Zero-cost change to an existing message.
4 · Do not chase Booked appointments token until integrated booking is realistic
Rarest token in the sample. Almost always requires Google-integrated appointment booking. Not the fastest lift on the table.
