TL;DR — post cadence findings
- Zero-post profiles drift down. Average grid position -0.42 (rank worsened by ~0.4 slots), top-3 pack presence -1.6 percentage points across the 12-week window.
- Steady 1-2 posts/week is the efficient operating point. Grid position +1.34 slot improvement, top-3 pack presence +6.8 percentage points, AIO readiness +0.90 points.
- Five or more posts per week yields negligible marginal lift. Only +0.17 grid slots and +0.6 percentage points beyond the steady cohort — inside the noise band of the scanner.
- Post content matters more than post count. Templated cross-location posts are effectively discounted by Google; bespoke practice updates carry the entire lift.
- High cadence carries policy-drift risk. Automated posts and cross-location duplicates sit inside the twelve GBP suspension triggers.
- Recommendation: 1-2 bespoke posts/week, each with a real photo and a specific website link. That is the operating point that produces reliable lift with negligible policy risk.

Four numbers to anchor the study
Methodology
Between 1 May 2026 and 31 July 2026, 328 healthcare GBPs in the Angryturtle portfolio were classified into one of four post-cadence cohorts based on their post frequency across the twelve weeks of the study. Every profile was scanned on a 7-by-7 geo-grid (49 points, roughly 5 km on a side in most metros) at study start and study end for its primary category keyword. Grid position was recorded as the average rank across the 49 points where the listing appeared in the local pack, capped at 20. Grid-position change is the difference between the two scans, expressed as a negative number when rank improved.
In parallel, 6,420 individual posts across the sample were classified by content type — bespoke practice update, cross-location templated post, generic health tip, promotional offer, event announcement — and by media type (unique on-premises photo, stock image, no image). The classification was used to test whether post content, not just cadence, explained the observed lift.
Controls. Cohorts were matched on baseline health score, category concentration and city mix before analysis. Fertility clinics, dermatologists, dentists and diagnostic centres are represented in every cohort in roughly the same proportion. Cohort-level lift is reported as the sample mean; per-listing dispersion is noted where relevant.
Anonymisation. No client names, addresses, phone numbers or reviewer identities appear in this report. Every figure is aggregated at cohort level with a minimum n of 42 per cohort.
Cohort results · cadence versus rank change
| Cohort | n GBPs | Frequency | Grid rank change | Top-3 pack change | AIO readiness change |
|---|---|---|---|---|---|
| Zero posts | 78 | 0/week | 0.42 | -1.6 pp | -0.10 |
| Low cadence | 96 | 1-2/month | -0.18 | +1.2 pp | +0.15 |
| Steady cadence | 112 | 1-2/week | -1.34 | +6.8 pp | +0.90 |
| High cadence (5+/wk) | 42 | 5-7/week | -1.51 | +7.4 pp | +1.05 |
Grid rank change: negative = rank improved. Top-3 pack change measured in percentage points. AIO readiness scored on a 15-point composite. n = 328 profiles distributed across cohorts.
Cadence versus grid-position lift
Average grid-position improvement over 12 weeks · negative is better
Content type explains most of the residual variance
Within the steady 1-2/week cohort, the profiles running bespoke practice-update posts averaged -1.62 grid positions of improvement. The profiles inside the same cohort running templated cross-location posts averaged only -0.71 grid positions. The gap between the two — nearly a full grid slot — is entirely explained by Google discounting the repeated content. High-cadence cohorts running templated posts recorded lifts smaller than steady cohorts running bespoke posts, which is why the study\'s single strongest recommendation is post-content quality, not post-count.
Post media matters too. Posts with a unique on-premises photo (real waiting room, real equipment, real signage) generated approximately 40 percent more grid-position lift than posts using stock or off-premises photography. Posts with no image at all showed effectively no rank movement. The mechanism is likely visual-content freshness feeding the Maps card render, which is one of the direct inputs into the click-through model.
Policy-risk correlates with cadence
Two of the twelve GBP suspension triggers scale with post frequency: automated posts flagged via API violation, and cross-location duplicate content. Both are more common in the high-cadence cohort. The 5-7/week profiles in the study received policy warnings on approximately 8 percent of listings during the window — no suspensions, but every warning is a strike against the profile\'s history. The steady 1-2/week cohort received warnings on under 1 percent of listings.
The efficient operating point balances lift against risk. Steady cadence captures 89 percent of the maximum observed lift with less than one-eighth of the policy-warning rate of high cadence. There is no evidence in the data that anyone should be posting more than twice a week per listing.
What to change this week
1 · Set the cadence at one to two bespoke posts per week
Not five, not zero. One to two. That is where the entire measurable lift sits, and it fits into a normal operating rhythm for any healthcare practice with a marketing lead or agency retainer.
2 · Every post carries a real photo taken on the premises
Waiting room, equipment, signage, staff (with consent). Stock imagery is a wasted post. No-image posts are effectively dead.
3 · Kill cross-location templated posts
Google discounts them, and they carry policy-drift risk. Every post is bespoke to the location. Even a two-line change in wording per location moves it out of the discount band.
4 · Link to a specific website page, not the homepage
The website page must match the post intent — an "IVF lab upgrade" post links to the IVF lab equipment page, not the homepage. This is one of the highest-yield content signals feeding the AIO readiness composite.
