TL;DR — six findings from the 328-profile correlation set
- Top band beats bottom band by 3.1x. Profiles carrying 100 or more photos average 3.1x the monthly direction requests of profiles carrying 20 or fewer, adjusted for specialty and city density.
- The lift is not linear. The first 40 photos deliver roughly 65 percent of the total lift. Adding photos 41-100 delivers the next 25 percent. Adding photos beyond 100 delivers the last 10 percent.
- Owner-uploaded and customer-uploaded photos both count. The profile-level total is what correlates. A 60 owner + 40 customer split behaves the same as 100 owner + 0.
- Diagnostic centres, Hospitals and Dental clinics show the strongest correlation. Cosmetic surgery, Dermatology and Fertility show weaker correlations — the decision to visit is driven more by consultation content.
- Cover photo and logo carry disproportionate weight. Both are inputs into the Maps card render. Missing either caps every downstream photo lift by roughly 40 percent.
- Photo freshness matters. Profiles that add at least 5 new photos monthly show 1.9x the direction requests of profiles that ship a batch and then stop, at the same total photo count.

Five numbers to anchor the study
Methodology
Every profile in the 328-listing Angryturtle portfolio was audited monthly for photo count (owner-uploaded, customer-uploaded, total), cover-photo presence, logo presence, and photo freshness. For every profile that had GBP Insights available across the reporting window, the four public interaction metrics — profile views, search views, direction requests, website clicks — were captured at month-end. Direction requests are used as the primary outcome because they are the highest-intent action Google exposes to profile owners.
Banding. Profiles are grouped into four photo-count bands: 0-20 photos, 21-40, 41-100, and 100+. Bands are based on total published photos, both owner and customer-uploaded. Bands are recomputed monthly, so a profile that ships new photos and crosses into a higher band is measured in the higher band from the following month onward.
Correlation. Pearson correlation between photo band and monthly direction requests is calculated within specialty and city-density strata to control for confounds. City density is defined as the count of competing healthcare listings in the same primary category within a 5-km radius, normalised into three density tiers (low, medium, high). Specialty is the profile's primary GBP category.
Exclusions. Profiles without GBP Insights access, profiles active for fewer than 90 days inside the window, and profiles where the primary category changed mid-window (which triggers a category-migration audit under a separate study) are excluded from the correlation calculation. The unweighted correlation-eligible sample is 287 profiles.
Anonymisation. No listing identifiers, addresses, phone numbers, photo content, or per-profile Insights numbers appear in this report. All figures are band-level or specialty-level aggregates.
Finding 1 · Top photo band earns 3.1x the direction requests of the bottom band
The numbers: normalised to a base of 100 direction requests for the 0-20 photo band, the 21-40 band averages 178, the 41-100 band averages 265, and the 100+ band averages 310. The 3.1x ratio holds across every specialty and city density stratum after adjustment.
The lift is not evenly distributed across the count range. Going from 20 to 40 photos delivers a 78-index-point lift. Going from 40 to 100 delivers 87 more. Going from 100 to well past 200 delivers only another 45. In other words, the first 40 photos are worth almost twice as much per photo as the next 60, and the photos past 100 are almost cosmetic.

Finding 2 · The first 40 photos deliver 65 percent of the lift
The rule of thumb: a profile at 20 photos capturing 100 direction requests per month lifts to 265 at 40-100 photos, and then only to 310 past 100. Roughly 65 percent of the total possible lift is unlocked between photos 1 and 40.
This is the single most operationally useful finding in the study. Photo shoots budgeted at 40 owner-uploaded images cover the interior, exterior, staff-at-work (no faces), signage, key equipment, waiting area, procedure rooms, and the immediate street context. That count is achievable in a single half-day session at a single location. Anything past 40 is worth doing but does not warrant a second dedicated session — it should be filled in through monthly incremental captures and customer-upload prompts.
Finding 3 · Owner and customer uploads correlate almost identically at portfolio level
The finding: comparing 20 profiles at 100 owner-uploaded + 0 customer-uploaded against 20 profiles at 60 owner + 40 customer, direction request means differ by less than 4 percent. The correlation is with total published photo count, not with the origin of the upload.
Practically, this means encouraging customer photo uploads via post-visit prompts is a legitimate path to the 100-photo band. Angryturtle-managed listings that add a "photo of your visit" prompt to the review-request SMS earn between 2 and 6 customer photo uploads per week in high-throughput specialties. Over 12 months, this can carry a listing from 40 owner-uploaded to 120 total without a second dedicated shoot.
Finding 4 · Correlation strength varies by specialty
The pattern: photo-to-direction-request correlation is strongest in Diagnostic centres, Hospitals, Dental clinics and Skin care clinics, and weakest in Cosmetic surgery, Fertility, Dermatology, and Plastic surgery.
Diagnostic and dental patients arrive at the physical location for the service; they look for confirmatory visual proof that the location matches the online branding before they leave home. Fertility, cosmetic surgery and plastic surgery decisions are made in the consultation, not in the search — the direction-request decision follows the appointment booking, not the other way around.

Finding 5 · Cover photo and logo are pre-requisites for the lift
The pattern: profiles missing a cover photo or a logo capture only about 60 percent of the direction-request lift their photo count would otherwise predict. Both fields are visible on the Google Maps card that a searcher sees before they reach the profile page — a card missing a logo or a cover reads as incomplete and reduces the click-through into the profile that photos would then influence.
In the broader Angryturtle portfolio, only 52.1 percent of listings had a logo and 53.0 percent had a cover photo. Nearly half the sample is capping its own photo-investment ROI by leaving one of the two most visible fields blank.
Finding 6 · Freshness matters — 5 photos/month adds 1.9x to a static portfolio of the same size
The finding: compare two 60-photo profiles — one that shipped the 60 in a single onboarding session and stopped, versus one that continues to add 5 new photos per month. The monthly-refresh profile averages 1.9x the direction requests of the static profile at month 12.
Google's ranking model treats photo freshness as a "still active" signal. A profile that stops shipping content is a profile the model gradually deprioritises against actively-managed competitors, regardless of total photo count. Five owner-uploaded photos per month is enough to hold the signal; it does not require a monthly shoot — a single staff member with a phone camera can produce five in an hour.
Finding 7 · The photo-mix matters more than the raw count above 40
Once a profile clears the 40-photo threshold, the composition of the next 60 photos matters more than the incremental count. In a sub-sample of 46 profiles that added 60 owner-uploaded photos over 90 days, we tracked the composition of the batch: interior versus exterior, staff-at-work versus static equipment, procedure rooms versus waiting areas, signage versus street context. Profiles whose 60-photo batch included at least 8 exterior street-context images showed a direction-request lift 27 percent higher than profiles whose batch was heavily interior-weighted.
The likely mechanism is search-intent match. A searcher tapping "directions" on a Maps card is a searcher who has already made the visit decision and is now confirming the physical location. Street-context images (approach shots, signage from the road, the entrance from the parking area) are exactly the visual proof that decision-stage searchers look for. Interior images matter for the earlier discovery-stage decision, but the conversion payoff on the direction tap comes from the last-mile confirmation. Any 40+ owner-uploaded photo batch should carry 8-15 street-context images by default; most current healthcare portfolios do not.
What this means for healthcare operators
1 · Fix logo and cover photo first, always
Both are one-hour uploads. Both are near-zero cost. Both are prerequisites for any downstream photo-count lift. If either is missing, no photo investment converts at full ratio.
2 · Ship 40 owner-uploaded photos in the first 90 days
The 20-to-40 jump unlocks a 78 percent direction-request lift. The 40-to-100 jump adds another 87 index points. Prioritise the first 40 in a single dedicated session; fill the next 60 through monthly increments and customer-upload prompts.
3 · Add a "photo of your visit" prompt to the review-request flow
Customer photo uploads correlate as strongly as owner uploads at the portfolio level. The prompt is a single sentence in the SMS or WhatsApp review-request message. Over 12 months it can carry a listing from 40 to 100+ photos without a second dedicated shoot.
4 · Ship 5 new photos monthly, indefinitely
Freshness is worth almost 2x the direction-request ratio at the same total photo count. Assign the task to a single staff member with a phone camera. One hour a month.
