TL;DR
Author: Abhash Kumar · Co-Founder, ICG · IIT BHU + IIM Bangalore · July 2026
Meta's Health and Wellness advertising policy is one of the most technically complex advertising policy environments in digital marketing. It intersects with Indian healthcare regulation (NMC Section 6, Schedule J, ART Act 2021), with Meta's own global advertising policies, and with the specific technical restrictions around audience targeting, creative formats, and conversion tracking for healthcare advertisers.
ICG manages 43+ live healthcare Meta accounts simultaneously. This guide reflects what we actually encounter in the policy environment — not a theoretical reading of the policy document.
What Meta's Health and Wellness Policy covers
Restricted healthcare categories in India (require adherence to Meta's health policies): Pharmaceutical and supplement products, online pharmacies, clinical trials, addiction services, cosmetic procedures and surgeries, fertility services and treatments, sexual health services, weight management services and programmes, mental health services.
Key restrictions:
Condition-implying language in ad copy: Meta prohibits ad copy that implies knowledge of or targets a user's health condition. "Struggling with infertility?" is non-compliant — it implies the ad is targeted to people Meta has identified as dealing with infertility. "IVF consultation — [City]" is compliant — it describes the service without implying condition targeting.
Before-and-after imagery: Meta restricts before-and-after imagery for cosmetic procedures and medical treatments. This aligns with NMC Section 6's prohibition on identifiable patient before-and-after imagery. ICG's creative framework avoids before-and-after for both compliance reasons simultaneously.
Personalized advertising based on health conditions: Meta restricts using health-condition-based interest categories for targeting healthcare ads. Custom audiences from first-party data (website visitors, CRM customer match) are permitted. Interest-based targeting using inferred health conditions is restricted.
Special ad categories: Ads for certain healthcare services (credit, housing, employment, and — added in recent policy updates — certain health and wellness services) must be run under "Special Ad Categories" which restricts demographic targeting options.
ICG's compliant Meta creative framework for healthcare
| Non-compliant approach | ICG compliant alternative |
|---|---|
| "Are you struggling with hair loss?" | "Understanding hair restoration — what you need to know before deciding" |
| "Dealing with fertility challenges?" | "IVF consultation — specialist fertility evaluation in [City]" |
| Patient before-and-after (identifiable) | Educational procedure Reel (mechanism of action, NMC carve-out) |
| "Guaranteed LASIK results" | "LASIK — advanced refractive surgery with [technology] at [Clinic]" |
| Health-condition interest targeting | First-party lookalike + demographic + location targeting |
| Advantage+ creative with healthcare | Manually controlled asset groups with compliance-reviewed creative only |
CAPI and Meta's evolving privacy architecture
Meta's Conversions API (CAPI) is now essentially mandatory for healthcare advertisers who want to maintain campaign performance as Meta's pixel-based tracking degrades.
Why CAPI matters for Meta healthcare compliance: CAPI sends server-side conversion events that bypass browser tracking restrictions. From a privacy perspective, CAPI with proper hashing (SHA-256 of phone and email) is more privacy-protective than pixel-based tracking — the data sent to Meta is one-way hashed and cannot be used to identify individuals. This makes CAPI not just a performance tool but a privacy-compliance tool aligned with DPDP Act 2023.
ICG's Beacon platform implements CAPI with:
- Data hashing before transmission (SHA-256 of phone, email, first name + last name)
- Event data minimisation (only the data necessary for conversion matching is sent)
- Audit logging of all conversion events transmitted (for DPDP data processing documentation)
FAQ
Q1: Does Meta's health policy affect organic Instagram posts (not paid ads)? Meta's Health and Wellness advertising policy applies specifically to paid ads. Organic Instagram posts are governed by Meta's Community Standards and Instagram's Community Guidelines — which are less specifically restrictive than the advertising policy. However, NMC Section 6 still applies to organic posts by registered medical practitioners regardless of Meta's organic post policies.
Q2: What is the "Special Ad Category" for health services and how does it affect targeting? Special Ad Categories on Meta restrict demographic targeting options: gender, age, and location targeting are limited for special ad category campaigns. For most healthcare specialties, standard campaigns (not Special Ad Category) are appropriate — Special Ad Category is primarily required for credit, housing, employment, and certain social issue ads. Specific healthcare categories (e.g., some addiction treatment services) may be subject to Special Ad Category requirements — ICG assesses this at account setup.
Q3: How does Meta's Advantage+ Creative interact with NMC compliance for healthcare? Advantage+ Creative uses AI to test creative variations, including modifying text and imagery. For healthcare, this creates a compliance risk — AI-generated creative variations may introduce non-compliant language that hasn't been reviewed. ICG's Meta healthcare campaigns use controlled asset groups with fixed, compliance-reviewed assets. Advantage+ Creative is disabled for healthcare accounts where compliance accuracy is critical.
Compliance note: Meta Health and Wellness Advertising Policy (current as of July 2026), NMC Section 6, DPDP Act 2023.
