IVF Chandigarh: 67% enquiry growth in a Tier 2 market — with a media budget of ₹1.2 lakh per month
CPQL ₹1,840→₹980 · Consultations +66% · 20 weeks
The starting position
Single-location IVF clinic in Chandigarh — 3-cycle-room, 2-embryologist setup with a senior reproductive endocrinologist (10+ years IVF experience, fellowship from a Mumbai fertility hospital) as the primary clinician. Four years of operation at the time of ICG engagement, patient base built on referrals from the senior clinician's prior practice network. The Chandigarh IVF market context: Chandigarh Tricity (Panchkula, Mohali) has 8-12 IVF clinics. But the significant market is not just Chandigarh city — it is the inbound catchment from Punjab (Ludhiana, Amritsar, Jalandhar, Patiala, Bathinda) and Haryana (Ambala, Kurukshetra, Hisar, Rohtak) that lacks adequate local IVF infrastructure. A well-positioned Chandigarh IVF clinic is a regional fertility hub for North India. This regional catchment dimension was not reflected in the clinic's marketing. The existing paid search programme was geo-targeted to Chandigarh city only (5km radius). Campaign keywords were generic IVF searches without city or region modifiers. The clinic was spending ₹1.2 lakh per month and generating 38-45 monthly consultations at a CPQL of ₹1,840 — above the national market median (₹2,400) but significantly above ICG's Chandigarh benchmark (₹980).
The root cause
ICG's diagnostic covered three areas: paid media architecture, CRM performance, and the organic content surface. **Paid media finding — The geo-targeting issue was the primary culprit.** Location-intent keyword analysis for IVF in North India showed: 'IVF clinic Chandigarh' and variants generated 320 monthly searches from within Chandigarh Tricity, but 180 monthly searches from Punjab (Ludhiana 60, Amritsar 45, Jalandhar 35, Patiala 25, others), plus 120 monthly searches for 'IVF hospital Chandigarh' from Punjab, and 90 monthly searches for 'Best IVF doctor Chandigarh' from Punjab and Haryana. The Punjab inbound search volume (300+ monthly searches for Chandigarh-intent IVF from outside Chandigarh) was being entirely missed by the 5km radius geo-target. At a 35-40% conversion rate, that's 105-120 potential enquiries per month from an audience already looking for a Chandigarh IVF clinic. **CRM finding — 10-day follow-up architecture for a 90-180 day decision cycle.** Hawk's audit found 31% of the clinic's existing leads (40-48 per month) in limbo status at the 30-day mark. The clinic's follow-up system: 3 WhatsApp messages + 2 phone calls within the first 10 days, then nothing. Patients who didn't book within 10 days were effectively abandoned — despite IVF having a 90-180 day consideration cycle. A 10-day follow-up sequence is appropriate for an aesthetic derm clinic. It is completely wrong for IVF. **Content finding — Near-zero competitive YouTube content from Chandigarh IVF providers.** YODA's analysis of the Chandigarh IVF YouTube landscape found Chandigarh's IVF clinics had collectively produced fewer than 20 YouTube videos. National chains (Nova IVF, Indira IVF) dominated whatever YouTube content existed. A Chandigarh-specific doctor-fronted IVF YouTube channel had essentially no competition.
What ICG built
**Phase 1 (Weeks 1-3) — Campaign expansion.** ICG rebuilt the geo-target from 5km Chandigarh radius into a 3-tier geography: Tier 1 (Chandigarh Tricity, 10km radius) with highest bid adjustments for proximity-intent patients; Tier 2 (Punjab ring — Ludhiana, Amritsar, Jalandhar, Patiala within 150km) with moderate bid adjustments and 'in Chandigarh' keyword modifiers; Tier 3 (Haryana ring — Ambala, Kurukshetra, Hisar within 100km) with lower bid adjustments. New keyword campaigns per tier with city-specific ad copy: Tier 2 ad copy specifically referencing 'Patients from Ludhiana and Amritsar welcome.' Beacon deployed simultaneously — server-side consultation-booking events replacing form-submission pixel events. Initial Chandigarh EMQ was 3.9 (lower than DermaClinix's 4.2 baseline — common in Tier 2 markets where pixel setups receive less agency attention). Beacon corrected EMQ to 6.8 by Week 3, 7.4 by Week 6. **Phase 2 (Weeks 2-4) — Hawk 90-day follow-up architecture.** Follow-up system rebuilt from 10-day sequence to 90-day sequence. Days 1-3: immediate WhatsApp response via 4-Bot (within 4 minutes), followed by personalised follow-up call. Days 7-10: second WhatsApp with FAQ content package. Days 21-28: soft re-engagement from the doctor personally. Days 45-60: seasonal content message. Days 75-90: final personal outreach offering video consultation as alternative to in-person visit. Punjab-specific personalisation throughout: CRM captured patient's city at enquiry, follow-up messages referenced the patient's specific geographic situation, acknowledged travel considerations, offered logistical support. **Phase 3 (Weeks 5-10) — YODA YouTube content programme.** 6-video Chandigarh-first IVF YouTube content calendar produced over 6 weeks: 'IVF in Chandigarh — what to expect at your first consultation' (14 min, 8,400 views by Week 20), 'IVF cost in Chandigarh — what's included and what's not' (10 min, 12,300 views), 'Travelling from Ludhiana/Amritsar for IVF — how we make it easier' (8 min, 4,200 views disproportionately from Punjab geo), 'What is ICSI and when is it recommended?' (12 min, 5,600 views), 'Male infertility and IVF — a conversation many couples avoid' (9 min, 3,800 views), 'IVF after 38 — what the data says about older patients and success' (11 min, 4,900 views). All NMC-compliant: outcome statistics quoted as ranges from published peer-reviewed data (ICMR, SART), not as clinic-specific guarantees.
**Platforms deployed:** - **Beacon** — Meta CAPI + Google conversion signal correction. EMQ lift 3.9 → 7.4. - **Hawk** — 90-day follow-up sequence architecture (rebuilt from previous 10-day system), Punjab-specific personalisation layer. - **YODA** — Chandigarh-first IVF YouTube content programme. 6 videos. 39,200+ combined views by Week 20. - **Agency OS** — Weekly reporting dashboard with Punjab inbound share tracking.

The ₹980 CPQL at Week 20 is at the national ICG median — achieved in a Tier 2 market on a ₹1.2 lakh media budget. The 38% Punjab inbound share at Week 20 means more than one-third of all consultations now come from outside the Tricity — patients willing to travel 100-200km for this clinic's care. These are high-commitment patients with above-average conversion rates and above-average treatment completion rates.
Three transferable lessons
Lesson 1: Tier 2 IVF clinics are regional hubs, not local clinics. A well-positioned Tier 2 IVF clinic serves a regional geography. The Punjab inbound catchment (3-4× the size of Chandigarh Tricity in population) was accessible at the same media cost as the local catchment — it just required the campaign geo-target to open. ICG now applies regional-catchment analysis as a standard diagnostic step in all Tier 2 city IVF engagements.
Lesson 2: The IVF follow-up sequence must be calibrated to the IVF cycle, not the clinic's capacity. A 10-day sequence abandons every IVF patient who hasn't booked in 10 days. ICG's 90-day Hawk sequence recovered 31% of limbo leads — a 10-day sequence would have recovered none.
Lesson 3: First-mover YouTube content in a Tier 2 city compounds faster than in a metro. The Chandigarh IVF YouTube landscape had essentially zero competitive content. 6 videos generated 19 monthly organic enquiries by Week 20 — the equivalent of ₹18,620 in paid media at the Week-20 CPQL, at zero incremental cost.
"We used to think of our clinic as a Chandigarh clinic. Now I think of us as North India's regional fertility practice. The Punjab numbers proved we had a catchment we weren't using."
