Methodology
This report aggregates three first-party ICG datasets over the window 1 January 2026 to 31 August 2026.
- GBP layer (Angryturtle): 36 IVF Google Business Profiles under active management, grouped by primary category (Fertility clinic / Fertility Doctor) and city cluster. Metrics: portfolio health score (0–100 composite of NAP consistency, review velocity, media freshness, category completeness, Q&A coverage, post cadence and citation coverage), rank score (weighted local-pack visibility across a proprietary keyword basket), and AI Overview citation count per listing.
- SEO layer (SIE — Search Intelligence Engine): Google Search Console queries pulled from 12 ICG-verified IVF properties. Query rows containing branded terms, doctor names or business names were stripped from the report set at extract time; only informational, procedural and geo-doctor queries are reported here.
- YouTube layer (YODA): 223 IVF vertical videos across the 2026 YTD window (142 Shorts, 81 long-form), pooled from client channels ICG operates. Metrics: lifetime views at snapshot, average duration in seconds, publish-month cohort.
Anonymisation: No client names, no doctor names, no business names, no addresses. Every number in this report is either a portfolio aggregate (n ≥ 2) or a category benchmark; singleton city rows are reported at the city name only, never at the listing level. City clusters with fewer than three listings are labelled as such so readers can weight the signal appropriately.
Finding 1 · Zero suspensions across 36 listingsThe IVF GBP portfolio is more suspension-resistant than the vertical's regulatory profile predicts.
IVF is a high-risk vertical for Google Business Profile takedowns. ART (Regulation) Act 2021 constraints on advertising, DPDP consent handling on the review layer, and Google's own healthcare policy overlay together make fertility one of the two or three verticals where a routine listing edit can trigger a review. Against that regulatory profile, the ICG managed IVF portfolio posted zero suspensions across 36 listings in the January–August 2026 window.
The composition is 30 listings tagged Fertility clinic and 6 tagged Fertility Doctor. The zero-suspension result is not a random draw — it is the compounding effect of a monitoring layer that flags category-completeness drift, NAP conflicts across the citation graph, and review-velocity anomalies before they escalate to a review by the platform. The finding is not "our listings never trigger flags" — they do — the finding is that the escalation pipeline is short enough to catch a flag before it becomes a suspension in a vertical where the median resolution time on an escalation, industry-wide, is measured in weeks.
Implication for operators: a fertility clinic operator running two or more GBP listings without a monitoring layer for NAP, category, and review velocity is quietly one policy edit away from a two-week revenue hole. The IVF vertical is not the one to run GBP on manual quarterly audits.
Finding 2 · The Gurugram–Delhi cluster gapGurugram IVF listings average 91.0/100 GBP health; New Delhi averages 81.8/100.
In the ICG-tracked IVF portfolio, the two largest metropolitan clusters are New Delhi (n = 8) and Gurugram (n = 4). Gurugram outperforms New Delhi on portfolio-average GBP health by 9.2 points — 91.0 vs 81.8 on a 100-point scale that composites NAP consistency, review cadence, media freshness, category completeness, Q&A coverage, post cadence and citation coverage.
The 9-point gap is real and it is not an artefact of city, competition or catchment. In every case where ICG has been able to A/B compare a Delhi and a Gurugram IVF listing on matched category and matched review velocity, the delta collapses — meaning the cluster average is picking up on operator behaviour (review-response cadence, post frequency, media upload discipline) more than on any structural advantage of the Gurugram catchment.
Implication for operators: the ceiling for a metro fertility listing is 100, not 82. An 18-point delta to the ceiling is roughly six months of disciplined review + post + media cadence work — not a replatform, not a rebuild.
Finding 3 · AI Overview citations are bimodalGurugram cluster earns 8.8 AIO citations per listing; a long tail sits at zero.
Across the 36 IVF listings tracked, the Gurugram cluster averages 8.8 AI Overview citations per listing, New Delhi averages 6.3, and Chandigarh's single listing peaks at 11 citations. Distribution is bimodal — a top quartile of listings pulls double-digit citation counts while a long tail sits at zero. The gap is not primarily explained by geography; it is explained by three signals the AIO extraction layer treats as high-weight for local intent:
- Structured data completeness on the listing itself — full MedicalBusiness or MedicalClinic schema, opening hours, specific service list, price range where possible.
- Review response velocity — not review count, response cadence — which correlates strongly with AIO citation probability on category-level "best IVF in [city]" queries in the tracked window.
- A first-party Q&A block on the listing that mirrors the top People-Also-Ask surface for the query — the AIO layer reads this as a candidate answer pool.
Implication for operators: AI Overview citation growth on a GBP listing is a 30–60 day workstream, not a content-marketing quarter. If a listing is at zero citations, the fix is on the listing — not on a new blog series.
Finding 4 · The "cost" cluster paradox25,957 impressions on "ivf treatment cost" — 0.13% CTR — nearly all volume landing on page two.
The largest single impression pool in the ICG IVF query set for 2026 YTD is the cost cluster. Three queries carry it:

Two things are happening in that table simultaneously. First, the informational cost cluster carries orders of magnitude more impression volume than any commercial geo-query. Second, the CTR is being suppressed by average position sitting between 12 and 24 — the properties are being served, they are not being clicked, because they are not on page one.
The implication is arithmetic: a fertility centre property that moves "ivf treatment cost" from avg position 23 to avg position 6 does not need to change conversion, does not need a new page, and does not need more content velocity. It needs a page-two-to-page-one push on a query that already has 25,957 impressions of demand it is currently missing.

That is the achievable ceiling for a well-optimised commercial geo-query in this vertical.

The Mumbai row is the interesting one. Average position 26.47 — comfortably page three — and it still delivers 3.19% CTR and 38 clicks. That is a demand-density signal, not a ranking-quality signal. Buyers of an IVF cycle in Mumbai are scrolling past page one to look at more options, which means the CTR floor on this query in Mumbai is materially higher than the CTR floor on the same query pattern in a lower-density city. Ranking a Mumbai property from position 26 to position 5 is not a linear return — it is a step-change return.

First, the average positions are page-one — three of the six queries sit at position 4 or better, which is what we would expect for competition-thin Hindi content when the English equivalent surface is crowded. Second, CTR peaks at 7.83% on the "ivf ke injection kahan lagte hain" query — well above the CTR ceiling for any English cost or process query in the same pool. Hindi patient-education content earns the click when it ranks.
Implication for operators: a bilingual (English + Hindi) patient-education hub for the IVF injection / embryo-transfer / process sub-topic is not a translation exercise. It is two parallel first-page ranking lanes with distinct query volume, distinct CTR ceilings and distinct on-page structure.

For the 2026 YTD window, ICG's IVF YouTube corpus reports 528,452 total lifetime views across 223 videos. When split by format, the delta is meaningful:
Two caveats sit on top of that headline. First, the distribution is heavily skewed by breakout Shorts — a single April 2026 cohort of ten Shorts delivered 190,279 lifetime views, which materially pulls the average up. Median performance is lower than the mean on both formats. Second, "lifetime views" understates long-form value on downstream signals — subscriber conversion, watch-time contribution to channel authority, and comment velocity are all higher per-video on long-form. The Shorts advantage is a top-of-funnel discovery advantage, not a full-funnel one.
