TL;DR — velocity is category destiny
- Category median velocity ranges about 100x across all 24 healthcare categories. From 0.08 reviews/week (Eye Care Clinic) to 7.92/week (Pediatric dentist). Among categories with 3+ profiles the spread is about 60x (0.10 to 6.18/week). Fertility clinics sit at 0.48/week across 28 profiles. Dermatologists at 0.40/week across 28 profiles.
- Structurally fragile categories: 8 of 24. Fertility, Plastic surgery, Dermatology, Urology, Cosmetic surgery, Mental Health, Eye Care, Obstetrics-Gynecology all average under one review per week per profile.
- Structurally resilient categories: 5 of 24. Pediatric dentist, ENT specialist, Diagnostic centre, Skin care clinic, Family practice physician all clear three per week at the median.
- Post-visit review-request automation is the only lever that moves the ceiling. Every top-decile location runs SMS or WhatsApp within 24 hours of the appointment.
- Velocity floor for reputation defence: 1 review/week. Below the floor, recovery from a bad-faith review takes months and a single negative review compounds into permanent rank ceiling.
- Velocity ceiling for AIO citation eligibility: 3 reviews/week. Below this ceiling, the AIO readiness sub-score for freshness of user-generated content is capped.

Methodology
Every review earned by every profile in the sample between 1 January 2026 and 31 August 2026 was date-stamped, counted per week and per profile, and aggregated at category level. Category floors, medians and top-decile marks are the 10th, 50th and 90th percentiles of the per-profile weekly average within the category. Categories with fewer than three profiles are shown in the table for range reference but are not treated as reliable benchmarks in the takeaways. Cross-category comparisons use the median as the primary point of reference; top-decile marks are the operating ceiling that trained review-request automation reliably produces.
Resilience banding. Very high = median velocity above 5.0/week. High = 3.0 to 5.0. Medium = 1.0 to 3.0. Low = 0.5 to 1.0. Fragile = below 0.5. The bands set the recovery time for a bad-faith review — a Very high category recovers in under two weeks; a Fragile category recovers in six months or more.
Anonymisation. No client names, doctor names, business names, addresses, phone numbers or reviewer identifiers appear anywhere in this report. Every figure is aggregated at category level with a minimum of one profile per category shown for reference and a footnote flag on any category with fewer than three profiles.

Median weekly velocity · resilient vs fragile categories
Median reviews per week per profile
Operating lines · floor, ceiling and target
Reputation-defence floor: 1.0 reviews/week. Below this line, recovery from a single bad-faith review takes more than ten weeks and the negative review compounds into permanent rank ceiling. Fertility, Dermatology, Plastic surgery, Urology, Mental Health, Cosmetic surgery and Eye Care all sit below the floor at the median — every one of these categories has a structural need for post-visit review-request automation.
AIO citation-eligibility ceiling: 3.0 reviews/week. Google's AI Overview citation model appears to weight recency of user-generated content — the freshness sub-score on the AIO readiness composite for the sample rises materially once the per-week velocity clears three. Six categories in the report sit above this line at the median: Pediatric dentist, ENT specialist, Diagnostic centre, Skin care clinic, Family practice physician, Hospital.
Reasonable target: category top-decile mark. The top-decile column in the benchmark table is not a stretch — it is the level that trained post-visit review-request automation reliably produces across the sample. A Fertility clinic at the top-decile mark of 1.50 reviews/week clears the reputation-defence floor. A Dental clinic at the top-decile mark of 1.40 reviews/week does the same. A Dermatologist at 1.10/week is still below the AIO ceiling but well clear of the compounding-risk zone.
Four actions worth taking this quarter
1 · Score your practice against its category median first
The benchmark table lets you place every location in the portfolio into one of five resilience bands within thirty seconds. Locations in the Fragile band are the highest priority regardless of specialty.
2 · Automate post-visit review requests in the fragile categories
SMS or WhatsApp within 24 hours of the appointment. One soft nudge at 72 hours. Nothing else. This is the single lever that has moved the top-decile mark in every fragile category in the sample.
3 · Set a velocity SLA and monitor it weekly, not monthly
A Fragile-band listing that misses the floor for two consecutive weeks needs an intervention that week, not a month later. Monthly rollups obscure the compounding risk.
4 · Pair velocity with response rate
Neither number is meaningful on its own. Every top-decile-velocity category in the sample also carries a response rate above 89 percent. Together they set the recovery time for a bad-faith review.
