The Topic × Format Matrix — YouTube for Doctors
What the matrix actually is
Rows are the topics your specialty is qualified to cover. Columns are the video formats YouTube rewards. Each cell is one video slot with a defined format, ranking-race target, production template, and compliance checklist. A quarter of planning fills the cells for that quarter. The doctor's calendar dictates how many cells get filled per week; the format template dictates what "one cell" means in production terms.
The value of the matrix is not that it exhaustively lists content. It is that it forces a channel manager to think about topics and formats simultaneously, which is how the algorithm reads a channel.
The five formats YouTube rewards for healthcare
| Format | Length | Primary race | Best for |
|---|---|---|---|
| Long-form explainer | 7 to 14 min | Search + AIO Lab | Money-page paired video, cornerstone topic |
| Short (vertical, under 60s) | 30 to 60 sec | Suggested + subscriber pull | Symptom decoder, myth-break, reversal hook |
| Doctor Q&A | 5 to 9 min | Search on tail queries | Sourced from Search Console and PAA |
| Case walkthrough (anonymised) | 8 to 12 min | Suggested + branded search | Building doctor authority; strict compliance |
| Community post + poll | N/A (text) | Algorithm warmth | Between-episode engagement, poll-driven next video |
The topic rows — how a specialty picks them
Topic rows are not brainstormed. They are extracted from data. Three inputs feed the row list:
- Search Console query report from the paired website — the queries the site already competes on are the queries the channel should compete on. This is the entry ticket for the AIO Lab race.
- YouTube Studio traffic-source report — the search queries and suggested videos already sending viewers to the channel, ranked by watch time.
- Call-centre and OPD logs — the questions patients actually ask the reception and the doctors in the first three visits. These are the questions the algorithm cannot find in Search alone.
A well-formed row list for a single-specialty clinic in 2026 contains 8 to 14 topics. Fewer and you miss coverage; more and no single topic gets shot in enough formats to compound.
The compounding pattern — same topic, three formats
The compounding move is to shoot the same topic in three formats within the same 30-day window: a long-form explainer, a Short, and a Q&A. YouTube's algorithm classifies these as a topic cluster and pulls the trio through Suggested as a set. This is the mechanism behind the "one topic, three videos" rule inside YODA.
Example — a gynaecology channel planning a PCOS topic row: Monday publishes a 9-minute long-form explainer (Search + AIO Lab targeting), Wednesday publishes a 45-second Short with the myth-break hook (Suggested targeting), and Saturday publishes a 6-minute Q&A answering the tail queries from PAA (long-tail Search). The three videos are linked by playlist and description, and each other's end-screens. Subscriber conversion on this pattern typically runs 2.4 to 3.8 times the channel baseline.
What a filled matrix looks like for a mid-size hospital channel
A 6-specialty hospital channel with 2 committed consultants per specialty produces around 90 cells per quarter. The distribution that works in India:
- 30 long-form explainers — cornerstone topics across all six specialties, one per week per specialty.
- 36 Shorts — three per week, weighted toward the specialties with the strongest OPD-heaviest weeks.
- 18 Q&A videos — tail-query targeting, batched to keep production overhead low.
- 6 case walkthroughs — one per specialty, strictly anonymised, high compliance overhead.
Community posts and polls fill the between-slot gaps. Playlists are named at the topic level (not the format level), so viewers who arrive on the Short can find the long-form on the same topic in one click.
The formats to avoid on healthcare channels
Not every format YouTube rewards is safe for healthcare. Four formats we explicitly exclude from the matrix, under NMC advertising rules and Telemedicine Practice Guidelines:
- Patient testimonial videos. Prohibited under the NMC 2026 advertising guidelines.
- Before-after transformation videos. Prohibited for the same reason.
- Live surgery streams without contextual medical disclaimers. Compliance risk plus platform policy risk.
- Celebrity endorsement videos. Both an ASCI and an NMC 2026 guideline exposure.
The absence of these formats does not slow down channel growth. It just removes the failure modes that get channels demonetised or reported.
How the matrix survives contact with a doctor's calendar
The single hardest part of running a healthcare channel is the doctor's calendar. OPDs run late, emergencies pull consultants away, conferences swallow a week. The matrix survives this by treating recording slots as fixed and topics as swappable inside a slot. A doctor booked for a 45-minute Wednesday morning recording slot has three eligible topics ready. If the first can't be shot for a compliance reason, the second is ready. If both fail, the Q&A format takes lower-prep time and fills the slot.
Fixed slot, swappable topic, three formats per topic — that is the operating rhythm that keeps the cadence intact for the three-race compounding effect to work.
What the matrix produces after 6 months
- Topical coverage — 8 to 14 topics each covered in at least three formats. Google reads this as topical authority.
- Playlist architecture — 8 to 14 named playlists, each a topic. Suggested pulls these through in packs.
- Content HQ pairing — each topic has a paired long-form text page on the website, both schema-tagged, both interlinked. AIO Lab citations start appearing on the topic queries.
- Subscriber compounding — the top-3 topics contribute 60 to 75 percent of subscriber growth. This is the signal to double down on those and drop the bottom two.
