ORM Crisis Playbook — the 24-Hour Response Framework
What actually triggers a crisis workflow
Not every negative review is a crisis. Not every unhappy comment is a crisis. A crisis, in the ReputationShield definition, is when one or more of the following breaks:
- Cross-surface correlation — three or more surfaces (Google, Reddit, Quora, YouTube, Meta) show correlated new negative activity inside 72 hours.
- Rank threshold — a negative Reddit thread or Quora question crosses into Google's top 3 for the doctor or clinic name query.
- Viral velocity — a thread crosses 500 upvotes or 100 comments in 48 hours, or a Meta post crosses 100 negative comments in 24 hours.
- Legal or regulatory claim — any review, comment or thread makes a specific claim of malpractice, billing fraud, ethics violation or licensure issue.
- Media pickup — a journalist or blog picks up the story with published or scheduled coverage.
Any single one of these opens the room. Two or more, and the SLA compresses.
The 24-hour framework — five phases
Phase 1 · Detection to Response (Hour 0-1)
Detection lives inside monitoring — the surface-specific tools (Angryturtle, YODA, Prism Pulse, Reddit and Quora sweepers) trigger alerts. Reputation lead confirms the trigger inside 15 minutes. Alerts fan out to clinic principal, marketing head and (where legal is required) counsel. Crisis room opens on WhatsApp or Zoom inside 60 minutes.
Phase 2 · Containment (Hour 1-6)
Objective: stop the crisis from acquiring new visibility while the response gets drafted. Actions:
- Pause any live ads pointing to the affected surface (a Meta ad running to a doctor's channel with a live crisis is just amplifying it).
- Hide (not delete) coordinated-attack comments on Meta.
- Flag rule-violating content on Reddit and Quora.
- Snapshot every surface — screenshots, URLs, thread IDs — for evidence and post-mortem.
- Cross-reference the crisis pattern with any prior reviews, complaints or internal-record events.
Phase 3 · Response (Hour 6-24)
Cross-surface response gets drafted, sign-off happens, replies get published. The rule is one surface, one response mode, one owner:
| Surface | Response mode | Owner |
|---|---|---|
| Google Business Profile reviews | Empathy + private-channel outreach | Angryturtle ops |
| Disclosed reply (if sub allows) + displacement plan | Reputation writer | |
| Quora | Better disclosed expert answer | Quora ops |
| YouTube comments | Public education reply + hide brigade | YODA ops |
| Meta comments | Empathy public + DM private + hide-rule set | Prism Pulse ops |
| Media / journalist | Written statement + spokesperson | Clinic principal + PR |
Phase 4 · Displacement (Hour 24-72)
Displacement content plan drafted and started. This is the long-play recovery: publish long-form authority content under the brand's own domain that answers the same query as the negative surface, targeted to outrank it inside 60-120 days. Two to five pieces get commissioned depending on the query cluster.
Phase 5 · Post-mortem (Hour 72 to Day 30)
Once the acute phase settles — usually inside 5 days for a well-managed crisis — the post-mortem runs. What was the root cause? What was the detection delay? What worked in the response? What did not? What changes to the monitoring or the operational SLA come out of it? The post-mortem is not blame — it is calibration.
The crisis room — who is in and what each does
- Clinic principal — final decision authority on public statements, private-channel outreach and any legal escalation.
- Marketing head or agency lead — runs the response workflow, coordinates owners, tracks SLA.
- Reputation manager — surface-specific tactical execution, drafting, publishing.
- Senior content writer — drafts displacement content, writes the private-channel messages.
- Legal counsel — where a claim, a threat or a media pickup requires counsel review.
- Internal comms lead (large groups only) — manages internal staff communication so the response is consistent.
What the response never does
- Never argue with a commenter or reviewer publicly.
- Never reveal specific patient information — even to rebut a claim.
- Never threaten legal action publicly (private counsel letter, if warranted, sits on a different rail).
- Never use language that admits fault on unverified specifics.
- Never delete criticism en masse — hide it, flag it, respond to it. Deletion is a compliance and algorithm penalty on almost every platform.
- Never coordinate fake positive reviews to counter negative ones. It gets caught, the profile suspends, and the crisis triples.
The displacement content plan — what and how
Displacement content is the long-play. Two to five long-form authority pieces published under the brand's own domain, each targeting one specific negative-surface query. Structure per piece:
- Title matched to the negative-surface query intent.
- Content that is genuinely better than the negative surface — more useful, more up-to-date, more specific.
- Author disclosure — named practising doctor, credentials.
- FAQ schema on every piece.
- Internal links from three or more existing high-authority pages on the brand's domain.
- Sustained content freshness — quarterly refresh minimum.
Median displacement window in the ReputationShield portfolio: 68 per cent of targeted negative surfaces pushed below Google position 10 inside 120 days.
