The starting position
DermaClinix is a multi-location aesthetic dermatology chain across Gurgaon (Sector 29 and DLF Cyber City proximity) and South Delhi (Greater Kailash). At ICG diagnostic start in October 2025, the Gurgaon location was generating 110-130 enquiries per month from Meta Ads at a CPL of ₹420. The Google Ads programme added a further 55-70 enquiries at ₹610 CPL. Combined: approximately ₹2.8 lakh per month on paid media generating 165-200 monthly enquiries. The CPL looked efficient. The CPQL — once ICG measured it — did not. Enquiry-to-consultation attendance rate was 31%, meaning only 1 in 3.2 enquiries became an attended consultation. The true CPQL on Meta was ₹2,800. For a ₹780-2,400 aesthetic dermatology procedure, that economics was unsustainable. ICG's diagnostic identified three root causes: Meta Event Match Quality at 4.2 (structurally below the 4.5-5.0 threshold for reliable optimisation), 28% of monthly lead volume (46-56 leads per month) in CRM limbo-status, and Google Ads attribution capturing only ~22% of actual consultations.
The root cause
ICG's Week-1 diagnosis meeting presented three findings to DermaClinix's marketing lead and clinic director. **Finding 1 — The programme's efficiency is an illusion.** The ₹420 CPL was generating the wrong kind of lead. ICG's national aesthetic-derm benchmark (43 clients, Q2 2026) is ₹950 CPQL; the Gurgaon-specific benchmark is ₹980. DermaClinix's ₹2,800 CPQL was nearly 3× the ICG benchmark for their own city. CPL is irrelevant; CPQL is the number that needs to change. **Finding 2 — There is a recoverable patient pool of ~180 leads already in the system.** The 46-56 limbo leads per month had been accumulating for 3-4 months. Approximately 180 patients had expressed interest, provided contacts, and been abandoned by the follow-up system after 1-2 calls. These were warm leads, not cold prospects. Hawk could re-engage them within 14 days. **Finding 3 — The technical infrastructure fix will drive the majority of CPQL improvement.** The EMQ correction (from 4.2 to a target 7.5+) would reduce Meta CPM and improve targeting precision without changing creative, audience, or budget — a technical fix with direct commercial impact. ICG estimated a 25-35% CPQL reduction from the EMQ correction alone, before any campaign restructuring. The clinic's marketing lead asked the right question: 'If the EMQ fix drives CPQL down, why has no one fixed this before?' The answer: previous agency was measuring CPL. The CPQL of ₹1,080 at Week 28 represents a 61% reduction from the ₹2,800 baseline. The media budget was unchanged throughout the engagement — the improvement came entirely from infrastructure (EMQ fix), re-engagement (Hawk), and organic growth (YODA YouTube).
Three transferable lessons
Lesson 1: CPL is the wrong metric. Always. The ₹420 CPL that looked efficient was masking a ₹2,800 CPQL that was unsustainable. CPQL (attended consultation cost) is the only metric that connects media spend to clinic revenue.
