Missed-call escalation: the patient who rang at lunch gets a call back
A patient rings your clinic during the lunch rush. Nobody picks up. They put the phone down, try the next clinic on the map, and book there. In the IVR report the next morning it is one line among dozens, and by then it does not matter.
AtomCRM treats a missed call as work that someone owns right now. The handset reports it within seconds, and the call-back is offered to colleagues one at a time, each with a short countdown, until someone takes it.

Forwarding carries the live call; the ladder catches the rest
A GSM call in progress belongs to the mobile network. No server can move it and no app can answer it on somebody else's phone. So AtomCRM uses two mechanisms that work together.
Carrier call forwarding, for the live call
Conditional call forwarding has been part of every GSM network for decades. You draw one forwarding ladder in AtomCRM, each handset asks for its own next hop, and the app sends the code to the carrier by itself. Chained across handsets, a patient's call walks the ladder while they are still on the line.
Escalation ladder, for the call that still got missed
If the call walks the whole chain and nobody answers, the handset reports the missed call and AtomCRM offers the call-back to colleagues in turn, each with a deadline. If one does not take it, it moves to the next.
Forwarding is fast but blind: it cannot skip a colleague who is offline or already on a call, and it leaves no record. The ladder is slower, but it is the only one of the two that can tell a manager on Monday which calls were lost on Saturday.
What the telecaller sees when a call is offered
A rung lasts about twenty seconds. A full-screen takeover would interrupt work that is also worth money, such as a note being typed after another call. So the offer appears as a red strip visible from every screen of the app. The telecaller can take it while reading a lead or halfway through a note, and taking it dials the patient straight away.
The countdown is drawn from the server's deadline and corrected for the phone's clock. A handset whose clock runs fast does not lose the call early, and one that lags does not get extra time.
The Missed tab has three buckets. Waiting is what is being offered to this telecaller right now. Live is every ladder still walking. Done shows rescued and lost calls side by side, because how many patients got a call back and how many did not is one question, not two.
Why the ladder log matters more than the call-back
Open any escalated call and AtomCRM shows the whole ladder as it actually walked: who was offered it, who was skipped and why, and who finally took it or whether nobody did.
That record turns a vague complaint into a fact. "Escalated three times, nobody answered" is an impression. "Three people offline at 10:32" is a staffing problem with a time on it, and a manager can fix it by moving a lunch break or adding cover on a busy morning.
Designing a ladder that works for your clinic
- Put your most available people first. The first rung should be someone whose job is the phone, not a counsellor who spends half the day in consultations.
- Keep it short enough to finish. Each rung is about twenty seconds. A ladder of three or four people rescues a call while the patient is often still holding the phone.
- Map numbers to ladders. A handset can be mapped to its own number, so calls on a branch or campaign number can go to the ladder for that team.
- Check your carrier permits forwarding. If the SIM does not allow call forwarding, the escalation ladder still works; only live forwarding does not.
Setting up the forwarding and escalation ladders is part of the one-time setup fee. You can change them later in the desktop CRM without touching the phones.
What the app will not claim
The network reports whether forwarding is switched on through a dialog the phone shows to the user and hands to no app. So AtomCRM never shows a confident "Forwarding: ON" it cannot check. The Me screen shows what the app sent to the carrier and when, which is what it actually knows.
We think that matters. A status light that was never verified is the kind of thing that looks fine for months and then fails quietly the day a carrier drops the rule.

How quickly is a missed call escalated?

Does the app need to be open to catch a missed call?
No. A background service catches missed calls while the app is closed, which is when most of them happen, and a notification tells the telecaller a patient is waiting.
What if nobody on the ladder takes the call?
The call is marked lost and shows in the Done bucket next to the rescued calls. The ladder log shows who was offered it and why anyone was skipped.
Do we need a telephony provider for forwarding?
No. Conditional call forwarding is a standard feature of GSM networks. The app sends the forwarding codes to the carrier itself. Your carrier must allow call forwarding on the SIM.
Can different branches have different ladders?
Yes. Handsets can be mapped to their own numbers, and routing rules can send calls on a given number to a given ladder.
