WhatsApp Business API setup guide for clinics in India
What the WhatsApp Business API actually is
WhatsApp offers three tiers of business messaging. The consumer app that comes on any phone is WhatsApp Personal. The small-business app is WhatsApp Business (WABA — the free green-icon app, tied to one number, no automations). The enterprise tier is the WhatsApp Business Platform, commonly called the WhatsApp Business API or BAPI. This is what clinics need if they want to automate reminders, run recall sequences, plug booking events into WhatsApp, or have more than one person handling the same number.
You cannot buy the BAPI directly from Meta as an SMB. You go through a Business Solution Provider (BSP) — companies like Wati, Interakt or AiSensy that are Meta-verified partners. The BSP handles account provisioning, template infrastructure and billing; Meta invoices the BSP, the BSP invoices you.
Step 1 · Meta Business Manager verification (7-14 days)
Before anything else can happen you need a verified Meta Business Manager. The clinic entity — not the doctor's personal Facebook — has to prove it exists. Documents required:
- GSTIN certificate in the legal name of the clinic entity
- PAN card of the clinic entity
- Business address proof — utility bill, lease deed or GST certificate address
- A domain email in the clinic's own domain (info@yourclinic.in works; gmail addresses do not)
- A live website on the clinic's own domain with visible contact details
The domain email is where 70% of Indian clinics get stuck. If your clinic runs on info.yourclinic@gmail.com you cannot verify. You need to add a professional mailbox — Google Workspace or Zoho Mail — before you begin. Budget ₹136 per user per month for Google Workspace Business Starter.
Step 2 · Pick a BSP (2-3 days)
Three categories exist. We stay neutral because we operate all three at PatientPulse:
- Template-first BAPI — clean template dashboard, quality-rating protection, stable webhooks. Suits outbound-heavy clinics (multi-city dental chains, high-volume aesthetic).
- Chatbot-first BAPI — visual flow builder, drag-and-drop qualification triage. Suits consultation-heavy specialities (IVF enquiries, aesthetic).
- API-first BAPI — REST-first, documentation-first, per-second rate limits. Suits hospital groups with an in-house tech stack.
Price bands are similar (₹2,500-₹8,000 per month subscription + Meta per-conversation pass-through). Feature parity is closer than sales calls suggest. The real differences are template UI quality, webhook reliability under load, and how good the support team is when Meta suddenly rejects a template category. Step 3 · Number registration and phone number decision (2-3 days)
You need a phone number that is not currently active on any WhatsApp account. Two paths:
- Fresh number — buy a fresh business SIM or virtual number. Cleanest option, keeps reception's existing number untouched, no migration headache. Downside: patients who already message the old number keep messaging the old number.
- Migrate existing number — port your existing WhatsApp Business number into the BAPI. You lose the personal green-icon app for that number (it becomes API-only). Uninstall the app on the phone first, then request migration through the BSP.
Step 4 · Display name and green tick submission
Meta requires your display name to match either your legal name or a variation Meta considers close enough. If your legal entity is "Bright Smiles Dental Private Limited" you can usually get "Bright Smiles Dental" approved. You cannot get "Bright Smiles" alone, and you cannot get "Dr Rakesh — Best Dentist Delhi." Meta's brand-name guidelines are the ceiling.
The green tick (official business account badge) is a separate submission. You need clean brand mentions across the internet — press coverage, verified social profiles, an Wikipedia entry helps. Most clinics do not qualify at Day 1. That is fine. Green tick is not required to send messages; it is a signal to patients that the account is verified. Revisit at month 6 once press coverage has built.
Step 5 · Author and submit the first 20 utility templates (7-10 days)
Templates are pre-approved message bodies Meta lets you send outside the 24-hour customer-service window. Every reminder, every recall, every campaign has to be a template. Meta rejects ~30% of first-round submissions for policy reasons.
The starter 20 for a clinic look like:
- Appointment confirmation
- Appointment reminder — 24 hour

- Cancellation acknowledgement
- Post-visit thank you
- Prescription ready alert
- Report ready alert
- Follow-up recall — 15 day

- Birthday wish (marketing — needs marketing opt-in)

Step 6 · Webhook wiring to your clinic management software
The templates are useless if no event fires them. A modern CMS (like the ones running Berkowits, Bhardwaj, Dermaclinix and other clinics on our stack) exposes appointment, prescription, invoice and package events over webhooks. The BSP dashboard has a webhook receiver. You point one at the other, map the events to templates, and the automation flows.
Legacy or closed CMS platforms need a bridge. Options:
- Zapier or Make.com — polling adaptors. Works for low volume, ₹2,000-₹5,000 per month.
- Google Sheet sync — cheap and cheerful. Reception enters bookings in a sheet, PatientPulse reads new rows and fires templates.
- Custom middleware — needed for high-volume hospital groups. Two to four weeks of engineering work.
Step 7 · Reception SOP and go-live drill
Every technical setup dies at the reception desk if the humans do not know what to do. Before go-live you need:
- Written SOP for who monitors the inbox, in which time window, and what the escalation path is
- A shared inbox tool (most BSPs include one; hospital groups sometimes plug in a dedicated inbox layer)
- A short list of doctor-approved reply templates for common enquiries (price, timings, insurance, procedure explainer)
- A rehearsal — send 20 test messages over an hour, measure who picks them up, iterate

Step 8 · Quality rating monitoring (forever)
Meta assigns your BAPI number a quality rating — Green, Yellow or Red. Green is normal. Yellow appears when users start blocking or reporting messages. Red is a suspension trigger and rate-limits your throughput. Common causes of a drop:
- Sending marketing templates to a list that never opted in
- Sending too many templates too fast to the same recipient
- Templates that are borderline promotional but submitted as utility
- Ignoring inbound replies for hours (patients then block)
The BSP dashboard shows quality rating. You need to check it weekly. Any drop below Green needs immediate root-cause action — this is where PatientPulse governance calls earn their fee.
The realistic 21-day timeline (Starter tier)
- Days 1-3 — Meta Business Manager submission, BSP selection, phone-number decision
- Days 4-10 — Meta verification in flight, domain email fixed, number provisioning, display-name approval
- Days 11-15 — First 20 templates authored, submitted, iterated to approval
- Days 16-19 — Webhook wiring, reception SOP, dry-run drill
- Days 20-21 — Soft go-live to real patients, monitoring, first governance snapshot
Hospital chains and full-library engagements run 30-45 days. Anyone promising a 3-day WhatsApp Business API setup is either lying, selling the WhatsApp Business (personal) app, or setting you up for a Yellow rating in month two.
