Evaluate clinic Meta Ads with six healthcare metrics — CPL, CPQL, appointment rate, LTV, ROAS and cohort quality. Real Indian benchmarks. Talk to ICG.
TL;DR
The 6 Meta Ads Metrics for Clinics
Metric 1: Cost Per Lead (CPL) — but contextualised
CPL is the starting point, not the endpoint. Benchmarks for Indian healthcare Meta Ads:
- IVF / Fertility: ₹300–800 per lead
- Dermatology / Aesthetics: ₹150–500 per lead
- Hair Transplant: ₹200–600 per lead
- Dental: ₹100–350 per lead
- Mental Health: ₹150–500 per lead
A CPL within or below these ranges is acceptable. A CPL above them requires investigation — most often a creative quality issue (low CTR leading to poor delivery cost) or audience size/targeting issue.

Metric 2: Form Completion Rate
For Meta Lead Ads (forms that open within Facebook/Instagram), form completion rate = people who opened the form / people who submitted it.
Benchmark: >60% completion rate. If your form gets opened but not completed, it is asking too many questions (keep it to 3–4 fields: name, phone, WhatsApp, area of interest).
Metric 3: Lead Quality Score (requires CRM correlation)
Pull your last 90 days of Meta leads. For each, record the funnel outcome: Connected / Conversation / Appointment Set / Attended / Treatment Started.
Calculate the lead quality score: Treatment-starting patients / total Meta leads.
Benchmark: 5–10% of Meta leads should start treatment in a well-run account. Below 3% indicates either audience quality is poor (wrong targeting) or the calling team is not handling Meta leads effectively.
Important context: Meta leads are inherently lower-intent than Google Ads leads (they did not search — they were shown). A 7% Meta treatment conversion rate is comparable to a 12% Google Ads treatment conversion rate in terms of effort required to convert.
Metric 4: Creative Performance — CTR and Frequency
Click-Through Rate (CTR) tells you whether the creative is compelling enough to generate a click.
Benchmark: >1.5% CTR for healthcare Lead Ads; >2% for Instant Experience.
Frequency tells you how many times the same person saw your ad.
Threshold: When frequency exceeds 3.0 for the same audience, performance typically begins to drop (audience fatigue). At this point, creative must be refreshed or audience must be expanded.
Metric 5: Audience Overlap With Google Ads
A patient who saw your Meta ad and then searched for your clinic on Google and clicked your ad is attributed to both. Without deduplication, you are crediting both channels for the same patient.
In Agency OS, the Google Ads and Meta Ads spend and conversion data are visible side by side. The Beacon deduplication layer identifies leads that entered from both channels and attributes them correctly — typically giving primary credit to the touchpoint closest to the conversion decision.
Metric 6: Video View-Through Rate (for video creatives)
If you are running video ads (Reels, Story formats), the 3-second view rate and 25% completion rate are key quality signals.
3-second view rate benchmark: >25% (meaning 1 in 4 people who see the video watch at least 3 seconds)
25% completion rate benchmark: >40% for 15-second videos
Low view-through rates indicate the first 3 seconds of the video are not compelling. The opening frame and first line of dialogue determine whether the patient watches or scrolls.
The Monthly Meta Ads Review Checklist
Ask your agency for:
- CPL by campaign and by creative, month-over-month
- Form completion rate by campaign
- Frequency by ad set (flag any above 3.0)
- Lead quality breakdown: Connected / Conversation / Appointment / Attended / Treatment
- CTR by creative (which specific ad images/videos performed best)
- Best-performing and worst-performing audience segments
If your agency cannot produce this breakdown, they are not monitoring Meta Ads at the level your spend warrants.

Five mistakes that make Meta Ads performance evaluation useless
Most Indian clinic marketing teams are not short of dashboards. They are short of a scoring rubric that tells them which metric to fix first. Over 300+ Meta Ads audits, ICG has seen the same five evaluation errors turn a healthy account into a false negative — or a leaky account into a false positive.
- Judging by CPL alone. A ₹120 CPL on an aesthetics campaign looks cheap until you find only 4% of those leads actually book a consult. Always pair CPL with CPQL (cost per qualified lead) and appointment-show rate. Our Meta Catalyst IQ platform surfaces both in a single view.
- Reading a 3-day window. Healthcare buyers take 7–21 days to convert. Evaluating a campaign before 14 days of spend at 3–5× your target CPL is guesswork, not analysis.
- Ignoring competitor context. Your CTR dropping 0.4% is not a creative problem if three competing clinics in your city just launched fresh angles. Use Prism Spy to see the Meta Ads Library moves that are cannibalising your reach.
- No cohort tagging on the CRM side. If your front-desk team cannot tell you which Meta Ads campaign a walked-in patient came from, your ROAS number is fiction. Every ICG engagement wires source-of-truth cohort tagging into the intake form in week one.
- Optimising for lead volume, not lead quality. Lead-form campaigns look cheap; conversation-optimised WhatsApp campaigns look expensive but close 3–5× better in dental, IVF and dermatology. Evaluate cost-per-conversation, not cost-per-lead-row.
If any two of these five apply to your account, your current performance evaluation is telling you the wrong story. A one-hour Client Elevation Programme diagnostic will surface exactly where the leak is — no obligation, no pitch deck.
