Short answer: moderately competitive, with a lower entry bar than the southern metros. In ICG's October 2026 study of the 35 most-reviewed healthcare brands in Kolkata, the median leader had 1,465 Google reviews (Hyderabad: 2,649) and averaged 4.73 stars, the lowest of seven cities. But leaders still add about 23.7 reviews a month, so the gap does not close by itself.
Kolkata is an unusual healthcare market to read from the outside. Word of mouth and doctor referrals still carry a lot of weight, yet patients increasingly check Google before they call. This article uses the Kolkata figures from ICG's Kolkata healthcare Google review study to answer a practical question for clinic owners and hospital marketing teams: how high is the bar on Google, and where is it lowest?
Kolkata against six other metros
We searched Google Maps in each city for seven specialties (dental, IVF, dermatology, hair transplant, eye, orthopaedic and multispeciality hospitals) and took the five most-reviewed brands in each search. That gives 35 leading brands per city. Here is how Kolkata's leaders compare on the two numbers that best describe how hard it is to catch up: how many reviews they already have, and how fast they add more.

Two readings matter. First, the stock of reviews is lower than in most metros: only Mumbai's leaders have a smaller median. A Kolkata clinic does not need to match a Hyderabad hospital's five-figure profile to look established. Second, the flow is not low. Kolkata's leaders add reviews faster than the national median and faster than Delhi, Pune or Chennai. The market is not standing still; it is just starting from a lower base.
What that means in months
A clinic that starts from almost nothing and collects 25 reviews a month, which is ambitious but achievable with a disciplined request routine, would take about five years to reach today's median Kolkata leader. By then the leader will have moved on. That is the honest arithmetic, and it is why the city-wide leaderboard is the wrong target for most clinics. The useful target is the three profiles that appear next to yours when someone searches your specialty in your neighbourhood. Those are often far smaller than the city leaders.
The rating is not where Kolkata leaders pull away
Kolkata's leaders average 4.73 stars, the lowest of the seven cities but only 0.13 below Mumbai. In practice, a well-run clinic at 4.7 is level with them. Only 8.6% of reviews with text were negative (111 of 1,294), close to the 7.8% national figure.
What stands out in Kolkata is what patients write about. Results and treatment outcomes were the main theme of 36.0% of written reviews, the highest share of any city in the study (national: 29.4%). Staff and bedside manner was still the most common theme at 46.4%, but that was the lowest share of the seven cities. Kolkata patients, on this evidence, talk more about whether the treatment worked than patients elsewhere.
Waiting is the complaint to plan around

Waiting and scheduling accounted for 23.4% of negative reviews in Kolkata, the highest share among the seven cities and well above the 18.8% national figure. Add booking and communication and almost a third of Kolkata complaints are about time and access rather than medicine.
That is good news for a marketing team, because expectations are something marketing controls. Patients complain less about a wait they were warned about than one that surprised them.
A practical plan for Kolkata clinics and hospitals
- Pick your battlefield. Search your specialty plus your area (Salt Lake, New Town, Ballygunge, Behala and so on) on Google Maps and write down the review count, rating and reply habits of the top three. That is your benchmark, not the city median.
- Ask every patient, every visit. A post-visit WhatsApp or SMS with your review link, sent to everyone, is the only reliable way to build volume. Google's policy forbids offering incentives and says businesses must not "selectively solicit positive reviews from customers", so do not filter for happy patients.
- Set time expectations in writing. Put typical visit length in the booking confirmation and on the Google profile, and send a WhatsApp update when a doctor is running late. This targets the city's most distinctive complaint directly.
- Reply more, and better. Kolkata leaders replied to 57.7% of reviews on average, below the 60.5% national figure. Reply to complaints quickly, take the details offline, and never confirm a patient relationship or treatment in public. See should a clinic reply to every Google review.
- Explain outcomes carefully. Because Kolkata reviewers write so much about results, describe what a treatment involves and how progress is measured, without promising outcomes. NMC professional-conduct rules and ASCI codes restrict outcome claims, and reviews will test anything you overstate.
- Pair Google with referrals, not instead of them. Many Kolkata patients arrive through a doctor or a relative and then check Google. A strong profile protects those referrals as much as it wins new searches.
