Form ID analysis methodology has updated for two changes since the article was first published:
Change 1 — DPDP-compliant lead syncing. Meta Lead Ads forms now require explicit DPDP consent capture before lead data can be transferred to a CRM. ICG's deployments include a structured consent field on every Lead Ads form. The compliance discipline preserves Lead Ads economics while satisfying DPDP requirements.
Change 2 — Form ID quality scoring. Each Form ID's downstream lead quality (qualification rate, show-up rate, booking conversion) is tracked through Hawk's CRM integration. The Form ID quality score becomes a Meta optimization signal — Meta's algorithm learns which Form IDs produce booking-quality leads and biases delivery toward those.

What Form ID Analysis Is
Every Meta Lead Ad has a unique Form ID. The form ID carries all the configuration choices: which fields are pre-populated, which are required, how many fields total, whether disclaimers appear, how the thank-you message reads. Each of these choices affects two things — completion rate (do they finish the form?) and quality rate (was the patient actually qualified?).
Standard Meta reporting shows campaign-level CPL. It does not natively break down by Form ID. You have to specifically run the analysis.
The Three Form Architecture Levers
1. Field count vs quality
Shorter forms have higher completion rates and lower quality. A 2-field form (name + phone) completes at 35–50%; the leads convert downstream at 8–12%. A 5-field form (name + phone + email + city + condition) completes at 12–18%; the leads convert downstream at 22–35%. The 5-field form often produces 2–3× the qualified-patient throughput per ad rupee despite the lower completion rate.
Pattern to look for: Most healthcare accounts default to 2-field forms because completion rate looks better in reports. The structural waste is enormous.
2. Question framing — closed vs open
A form asking "Which treatment are you interested in?" with closed options ("IVF / Egg Freezing / Sperm Freezing / Not Sure") versus an open text field changes the downstream conversation dramatically. Closed options qualify the lead for the telecaller (they know which department to route to). Open text gives noise the telecaller has to re-qualify on the call.
3. Disclaimers and trust signals
A line above the submit button — "We will call you within 30 minutes" or "Your information is confidential and HIPAA-equivalent" — affects completion rate measurably. The right disclaimer lifts completion 8–15% on healthcare forms. The wrong one (or a generic one) drops it.
Running Form ID Analysis
The procedure:
- In Meta Business Suite → Ads Reporting → Customise columns: add Form ID as a dimension
- Pivot CPL, completion rate, and downstream conversion rate by Form ID
- Identify which Form IDs over-index on volume but under-index on quality (and vice versa)
- Decide which form architecture to retire, which to scale
The Conjoint Analysis Approach
When you have multiple form variations running, treat the choice combinatorially:

That is 81 possible combinations from four binary-to-ternary levers. Most accounts never test more than 2–3 combinations. Conjoint analysis identifies which lever combinations drive both completion AND quality simultaneously — usually a non-obvious 3-way interaction that you would not find with one-variable-at-a-time testing.
A Real Example
An IVF chain in Delhi NCR ran three forms across one Meta campaign:

Why Most Accounts Miss This

- Completion rate looks like a clean success metric; nobody wants to scale forms that "complete worse"
- Downstream quality data lives in the CRM, not in Meta — without integration, the full picture is invisible
A campaign with three forms is three campaigns. Treating it as one hides the structural waste in two of them and the structural win in the third.
