The metric at the centre of that argument has a name — deduplicated reach — and getting it right is the single fastest way to end the definition-vs-strategy conversation that eats the first 15 minutes of every monthly review. What deduplicated reach actually measures

Deduplicated reach is the count of unique Instagram accounts that saw at least one piece of your content across any surface — Feed, Reels, Stories, Explore — inside the reporting window. If the same person saw four of your Reels and two Stories in the same month, they count once. That is the deduplication.

This is a fundamentally different number from impressions. Impressions counts every view, so the same person watching a Reel four times shows up as four. Impressions is useful for creative testing but misleading for the audience-growth conversation that clinic directors want to have. Deduplicated reach answers a cleaner question: how many real humans did we get in front of this month.

Why the app number and the export number rarely match

The classic Monday-morning argument has three causes and it helps to know which one you are looking at.

Cause one is window mismatch. The Instagram app defaults to a 30-day rolling window on the account view. Meta Business Suite defaults to the last 28 days on the desktop dashboard. If your monthly report uses the calendar month, all three numbers can be different for the same account and none of them are wrong. Prism Pulse standardises on the calendar month by default and shows the rolling 30-day as a secondary view, so the client's default frame matches your default frame.

Cause two is paid stitching. When an account has active Meta Ads campaigns, Business Suite quietly blends paid reach into some of the account-level rollups. The app usually shows organic-only on the primary reach counter. If you export the CSV and the number is 30% higher than what the client sees, this is usually why. Prism Pulse defaults every reach view to organic-only and explicitly labels the blended view.

Cause three is surface toggle. Some Business Suite views exclude Stories from the reach rollup by default; others include them. The app almost always includes Stories in the primary counter. Absolute reach is a vanity number without a trend. The three trend questions worth asking every month, in order, are: is deduplicated reach growing month over month; if it is growing, is the growth coming from a small number of viral pieces or from a wider content-quality lift; and is the engaged-audience proportion of reach also growing, or are we just hitting more strangers who never come back.

A healthy healthcare account shows steady month-over-month reach growth in the 8-15% range for the first six months of active content, followed by a plateau that signals it is time for a format shift. A spike-and-crash pattern — one month up 60%, next month down 40% — usually means one viral Reel and no repeatable playbook, which is where the ideation view inside Prism Pulse and the Programming service-to-KPI map become the more useful conversation.

The reach numbers a clinic founder actually screenshots

Founders tend to check three specific views inside the Instagram app, and knowing which one they are looking at protects you from a bad conversation. The first is the account-level Insights tab, which shows accounts reached over the last 7 or 30 days depending on toggle. The second is the individual post view, which shows unique accounts reached by that single piece. The third is Stories archive, which is often forgotten because it disappears after 24 hours but generates its own reach line inside the app.
