Google Review Schema - Definition, Formula, and Why It Matters for Healthcare Marketers
Google Review Schema is JSON-LD structured data that marks up individual reviews or aggregate rating data on a page, making the site eligible for star-rating enhancements in Google Search results. Star-rating enhancements typically lift SERP CTR by 20-35%.
How it works.
Two schema types apply — Review for a single review with author, ratingValue and reviewBody, and AggregateRating for a page's overall score derived from multiple reviews. Google validates the schema, cross-references the review count and star rating against on-page visible content, and — for eligible page types (LocalBusiness, MedicalBusiness, Product, Service) — surfaces the yellow stars and review count directly in the SERP result. Google's policy since 2019 restricts self-serving reviews (a business marking up its own homepage with its own aggregate rating). Third-party review aggregation on a services page is the compliant pattern.
Why it matters for healthcare businesses.
For a clinic ranking at position 3-5, star ratings in the SERP are often the single largest CTR lever available. A 4.8-star listing next to a 4.2-star listing takes disproportionate share of clicks even when both rank similarly. It is also a policy-sensitive area — a manual action for fabricated reviews is a permanent scar on the domain. Correct implementation with third-party review aggregation is the safe path.
Example - Aesthetic clinic services page.
An aesthetic clinic in Bengaluru had 4.7 stars on Google Business Profile but no review schema on the services page. Adding AggregateRating schema pulling from a Google Business Profile API integration, plus 8 individual Review schema blocks on the treatment pages, lit up star ratings on 34 SERP results inside 60 days. Organic CTR on those queries rose from 3.1% to 5.2%. Inquiries increased 27% with no ranking movement.
Common mistakes.
- Self-referencing reviews. A business marking up its own homepage with its own aggregate rating is a policy violation. Use third-party aggregation.
- Fabricated review counts. Google cross-references against Google Business Profile and other public sources. Fabrication triggers manual action.
- AggregateRating without visible reviews. Schema without matching visible content is invalid. Show the reviews on the page.
- Sitewide AggregateRating. One review score for the whole site is often invalid. Score per service or per doctor page, not per homepage.
Frequently asked questions.
Can I mark up my own homepage with my own rating?
No. Google prohibits self-serving reviews. Aggregate reviews from third-party sources like Google Business Profile.
How many reviews do I need for AggregateRating?
No official minimum. Under 3 reviews looks thin and often fails validation. 10+ is safe.
Do star ratings appear on every eligible page?
No. Google awards the enhancement at its discretion. Valid schema is necessary but not sufficient.
Should I use Review or AggregateRating?
AggregateRating for the summary score with a review count. Review for showcasing 3-5 specific testimonials. Both can co-exist on the same page.
